Strengths and Limitations of this study: By surveying participants at different sites and times, representing categories of different perceived risk factors, this study obtained credible estimates for population-level prevalence across these sites and how that changed over the sampling period. This will allow future screening at these sites to determine the seroprevalence trends. However, most seroprevalence studies only reflect past disease burden. Using Markets and Lorry Stations enabled sampling of a broad cross-section of the Ghanaian populace. Phase I of the study was conducted in the region with the greatest burden of reported infections and it was expected that a country-wide survey would yield less seroprevalence. Despite these strengths, our study also has a number of limitations including the potential for self-selection bias and the low number of ancillary staff in the sample. Due to the rapidly evolving nature of the vaccination programme, questions relating to vaccination were changed midway through the baseline questionnaire rollout which could have impacted on outcome, although we have controlled for questionnaire version in the multivariable analysis. The relevant sections of the baseline questionnaire were not designed to capture actual vaccine uptake as an outcome but rather attitudes towards vaccination and thus we cannot determine whether access to vaccination could be a driver in vaccine hesitancy in our sample, however this will be captured in follow-up questionnaires. In summary, we have identified key predictors of SARS-CoV-2 vaccine hesitancy in HCWs and demonstrate clear ethnic differences in hesitancy levels. Importantly, we have established drivers behind vaccine hesitancy in HCWs, which include belief in COVID-19 conspiracies and mistrust (of vaccines in general, in SARS-CoV-2 vaccines specifically, in healthcare systems and research) and suggest that these factors may account for some of the observed ethnic differences in hesitancy. Our study has several limitations, some of which is inherent to real-world EHR data. In particular, because ICU care status was based on EHR location data, ICU misclassification may have occurred due to creation of COVID-19 isolation wards within ICUs, regardless of the necessity for ICU-level care. Additionally, the complexity of the RTRM reduces its current applicability to clinical trials and other healthcare systems; however, we have provided a full list of variables and their contributions to prediction of hospitalized mortality, for further scrutiny and to allow incorporation of top contributors in other research and clinical applications. Future directions could focus on both research and clinical applications. First, the depth of the cross-facility, harmonized EHR data allows for more robust risk adjustment in retrospective, matched cohort studies and provides a granular endpoint to evaluate interventions beyond general effectiveness towards optimal delivery, including timing and dosing. Our analysis comes with important assumptions and limitations. First, due to the nature of COVID-19 prevalence, serology studies, and differing testing algorithms by country, determining the true incidence of COVID-19 at any given instance is not feasible. To address this, we have assumed a maximum incidence of 2%, and provide an increasing range up to this point. Second, our model output directly relies on the 1% threshold calculated for each Rt in each individual country. Limitations of this study involve the heavy lifetime cannabis use levels within the cannabis group. Hence, it is unclear if similar effects would be found in people with intermediate cannabis use histories. Despite collecting a coarse measurement of recent cannabis use at the time of vaccine willingness questionnaire, a continuous measurement of cannabis use was only collected at baseline. Hence, this study is also limited by the time gap between measurement of those two features. Additionally, participants were not asked to elaborate on their reasons for or against becoming vaccinated, however, data collection occurred just as vaccines were becoming available and skepticism over a new vaccine may have been at its highest. Recent data from the CDC indicated that intent to become vaccinated against COVID-19 may increase with time3. Thus, it is possible that participants’ willingness may have evolved since data collection as they witnessed more people receiving a vaccine. Future studies are needed to continue to monitor COVID-19 vaccine intent and uptake, as well as infection rates, among substance users. These studies will help inform the epidemiological characteristics of COVID-19 within substance users, and provide insights into combatting the pandemic within vulnerable populations.

4.1 Strengths and limitations: This study helps shed light on a novel situation that is still unfolding and offers insight into the challenges many children and young people are facing. One strength of this study is the diversity of professionals who completed the survey. This may help paint a fuller picture on which groups of children and young people may be suffering disproportionally and identify those at risk of developing symptoms stemming from stress and worry. Additionally, this data was collected during the height of the first lockdown, allowing us to gain insight into professionals’ concerns during the most restrictive phase of the pandemic. However, there are some limitations to this study. The first of these limitations is where the original data is taken. These data were obtained from the COVID-19 Information Page of the Republic of Turkey Health Ministry. This data can be included the possibility of false reporting and negligent reporting. The quality of the data can affect the build process and performance of the model to some extent. However, in order to obtain a better model, the best model was tried to be obtained by examining the p and q values from 1 to 10 instead of using an automatic model in the NNAR model. Strengths and Limitations: This is the first global survey, to our knowledge, to investigate the opinions of physicians about utility, trust, and risks of commonly used public health digital surveillance tools. Our survey data is from a relatively large and diverse sample of verified practicing physicians. Potential limitations include cross-sectional design, limited number of respondents from developing countries, inability to control for all possible confounding variables (e.g., personal medical history, socio-political beliefs, local data privacy regulations, knowledge about digital tools), and inability to deduce causality. Further, physician perceptions may change over time if infection risk and prevalence decrease, due to vaccination and herd immunity. Our findings should be interpreted within this context. Nevertheless, they provide a useful baseline for future surveys. However, we think that these cases are out of the mainstream of the epidemic, and they pose no serious limitation to the model. On the other hand, the proposed model is highly flexible, allowing for both completely asymptomatic and mild symptomatic cases, that are thought to play a significant role in the SARS-CoV2 epidemic. Among the limitations of our research, we have to mention the data quality. Changes in real-time data due to corrections, poor data-quality and slow reporting may affect therefore the assumptions of our model. Under-reporting due to slow data processing, restrictive testing policies and lack of testing availability impacted on the cumulative number of cases acknowledged by official data sources. While we use Russell’s method to correct for this, we are introducing potential limitations of Russell’s method into our model. Also regarding data quality, imported cases also introduce uncertainty in the model, and data is not as granular as it is required to account for that. Another limitation is that in our model, 81% of the infections are asymptomatic. While as mentioned previously, some local data shows that for every PCR-diagnosed case there were 9 IgG SARS CoV-2 positive individuals that had not been diagnosed during the outbreak in a very poor neighborhood in the City of Buenos Aires (30) reaching 50-60% seroprevalence, studies conducted in Europe after local outbreaks show 15 to 20% seroprevalence (31). Our study has several limitations, which include lack of viral culture data and samples at later timepoints. Additionally, to study the sgRNA decay kinetics, we targeted a single and highly abundant gene to compare and quantify gRNA and sgRNA, though we found high correlation between E-gene and N-gene sgRNA copy numbers. Analyzing additional targets would help us understand the stability of other sgRNAs. Another limitation our study is lack of unblinded data from the favipiravir study, precluding analysis by study arm. However, our findings of sgRNA persistence in clinical samples and lack of difference in its decline compared with gRNA are notable regardless of whether favipiravir reduced levels in one arm. For remdesivir, a RDRP inhibitor like favipiravir, we found no differential effect on sgRNA, compared with gRNA, in cell culture, despite effective reduction in viral replication and cytopathic effects. In summary, we found that SARS-CoV-2 sgRNAs are persistently detectable in clinical samples, correlate strongly with gRNA, and decline at indistinguishable rates in clinical samples and cell culture. We find little evidence to support the premise that sgRNA detection is a reliable marker of transcriptionally active virus or that it provides additional information beyond detection of gRNA in clinical samples. We advise caution against using sgRNA assays to inform decisions concerning treatment or medical isolation.

A further limitation was the inability to quantify infection risk associated with each activity due to insufficient data. This will be the focus of future work when additional virological and serological outcome data are available. There are several limitations to our study. First, our model assumed lockdown intensity could be precisely controlled by policy makers but in practice the actual extent of the contact reduction may not be obvious until a lockdown is in practice. Second, although the results of this study relied highly on parameters, especially contact rates, the contact rates referred to in the present study were based on data from [25], which did not take into account behavioral changes brought about by the COVID-19 pandemic. Third, our analysis excluded children aged from 0 to 15 years old. Several studies suggested children were less likely to contribute to epidemics [23,38–40], the number of children cases has been increasing due to a new variant of SARS-CoV-2. Finally, our model of economic loss considered only the impact of direct behavioral restraint due to the lockdown, and did not take into account the value that infectious disease victims were expected to produce in the future, which had been taken into account in several previous studies about economic losses by COVID-19 pandemic [18,41].

There are several limitations to the work presented here. This study was performed at a single site and our measured false positivity rate is likely dependent on our own testing processes. This analysis only covers high-throughput automated platforms. Other PCR methods that use a 96 well plate format which involve sample transfers and manipulation likely will experience a higher false positivity rate. The individuals tested here were not uniformly sampled in time after a positive result. Also, the specimen type, nasal swab, may have influenced the positive rate. Nasopharyngeal swabs are considered the gold standard, with a higher rate of positivity compared to other, less invasive sample types. There are several limitations to these conclusions. From a data perspective, we face the fundamental problem of correcting for systematic differences in testing that occur over long periods of time and across locations. Although our use of growth rates provide some improvement over unadjusted case data, such period-by-period estimates cannot capture longer term trends in differential testing. While modeling hospitalizations could have addressed such issues, these data are not widely available at the county level. Another limitation arises due to a lack of detailed mask behavior during the initial phases of the pandemic, making the task of disentangling the effect of mobility and masks very difficult. Additionally, observed data is often systematically missing and must be imputed, and checking the embedded assumptions in our imputation models is challenging. Many respondents to the survey reported lower quality of life on questions regarding limitations due to physical and emotional health, similar to a prior report indicating disrupted home, work, and social life for patients after acute COVID-19(6). We detected most cases in asymptomatic participants, highlighting the limitations of symptom screening for isolation of infectious individuals.16 While BinaxNOW is less sensitive than nucleic acid amplification tests, it appears effective at detecting infectious individuals.27 Detecting asymptomatic individuals is key to interrupting transmission chains as asymptomatic individuals may account for more than half of all transmission.34 We detected two of 40 individuals who were BinaxNOW-negative but rRT-PCR-positive. However, one of these was consistent with a prior infection35, leading to unnecessary isolation, a potential problem with highly sensitive RT-PCR testing. In addition, given the intensity of the pandemic and spread of misinformation, EHR data may represent confirmatory bias between providers and patients. Finally, we have excluded hospitalized COVID-19 patients. On the one hand, it would be difficult to match hospitalized Coronavirus patients during the COVID era with non-COVID hospitalized patients. On the other hand, the post-COVID syndrome can still be observed in patients who were never hospitalized.12,43–47 Regardless, future PASC studies should include hospitalized patients. Our understanding of COVID-19 and its chronic sequelae is evolving, and new risks are unknown. We do not know who might develop post-COVID syndrome, how long symptoms last, and whether COVID-19 prompts the presentation of chronic diseases. There is a unique opportunity today to understand the post-acute effects that can follow SARS-CoV-2 infection. The ever-increasing adoption and magnitude of clinical data stored in EHR repositories over the past decade provide exceptional opportunities for instrumenting healthcare systems to study evolving pandemic byproducts. Our approach avoids a flood of false positive discoveries, while offering a more probabilistic flexible criterion than the standard phenome-wide association study (PheWAS).

Limitations: This modeling approach relies on statistical relationships between wastewater primary sludge RNA concentrations and COVID-19 case rates, which are primarily based on diagnostic test results. The reported prediction intervals reflect the variability in these reported case rates. While commonly used as a proxy, reported cases are believed to underestimate infection due to asymptomatic COVID-19 infections.20 Clinical testing volumes in the municipalities considered were dynamic over the study period; responding to events such as school openings, holidays, and shifting of resources to locations with increasing case rates. The measured RNA concentrations in wastewater are not subject to such variation in testing policy and should therefore exhibit a more direct relationship to the unobservable changes in SARS-CoV-2 infection in the community. In addition to being unable to comment on community-level infection metrics, there are a few other limitations associated with our results that we must acknowledge. Aside from the ABM design limitations outlined in Farthing et al. (2021), we make a number of assumptions in our simulations. Most of these assumptions are directly tied to our parameter space detailed in Table 1, and include such things as: in simulated gatherings only one asymptomatic individual was in attendance, no individuals wear masks with exposure-reduction efficacies > 50% and therefore we are not simulating the use of N95 or similar masks, and there is no simulated forced-air ventilation or infectious individuals that produce superspreader-level of contaminated aerosols (e.g., 970 quanta (Miller et al. 2020)). Additionally, we do not simulate activity-specific behaviors and individuals in our simulations were unmoving. Finally, we based the infectiousness of asymptomatic individuals on the estimate given by Buonanno et al. (2020) (i.e., 142 quanta/hr), and to relate this estimate to ABM parameters we used the linear model described in Farthing et al. (2021). However, this parameterization procedure may have over-inflated virion transmissibility in certain scenarios because quanta-estimates are room-size specific, and the Farthing et al. 4.1 Strengths and Limitations: This study is the first systematic analysis of news coverage of COVID-19 related suicides in the UK at a time of heightened concern and speculation over the effects of the pandemic and associated restrictions on mental health (Brooks et al., 2020; Holmes et al., 2020), in the country with the highest overall death toll during the first wave of the pandemic relative to comparable countries (Raleigh, 2020). Our findings are based on a well-established, evidence-informed media monitoring database which captures all media reports of suicides and attempted suicides in the UK (Fraser et al., 2017). It may not, however, capture all important aspects of reporting, particularly in relation to the novel and rapidly evolving challenges precipitated by the pandemic and social distancing restrictions. Our understanding of what constitutes ‘positive’ and ‘negative’ news coverage of COVID-19 related suicidal behaviour may well require further refinement as time - and the pandemic - progress. Importantly, the perspectives of those most likely to be affected by such coverage should also be reflected in this process. 4.2 Future research: Ongoing research is needed to assess the quality and content of possible COVID-19 related suicide reporting over the longer-term, with feedback to those responsible where this deviates from recommended standards.
