Lossofthecost-sharingsubsidiesalonemeansthatpremiumswillrisebyanestimated19%in2018,varyingfrom7%to38%bystate[17]–andbyfarmore(between35%and90%)overathree-yearperiod[18].Thisisontopofotherpremiumincreasesresultingfromhigherthananticipatedserviceutilization.(Pre-miumincreasesfromrepealoftheindividualmandatewillnotoccuruntil2019.)Whilethesizeofpremiumincreasesvariesagreatdealbystate,theyweresubstantialduringthe2018openenrollmentperiod(whichendedon15December2017inthefed-eralmarketplace)–anaverageincreaseof34%forSilverplans

Regulationsgoverningsmallemployersthatgrouptogethertocreatetheirownplans(called“associationhealthplans”)wouldbemodifiedtopermitlesscomprehensivepolicies.Andthesaleofshort-termpolicieswouldbepermitted

(1)occasionallystatingthatitwouldnothavetheInternalRevenueServiceenforcethetaxpenaltiesforthosewhochoosetoremainuninsured–thisisrelevantto2018,whentheindividualmandateisstillinforce;(2)severelycuttingfundingforoutreachduringtheannualopenenrollmentperiod,aswellasthelengthoftheperiod;(3)providingnegativepublicstate-mentsabouttheACA;and(4)choosingnottofundtheso-called“cost-sharingsubsidies.

Onewouldexpecttoseeanuptickinthenumberofunin-suredfortworeasons:(1)thefinancialpenaltyforbeinguninsuredwillberemovedbeginningin2019,and(2)premiumswillriseduetoadverseselection,byanestimated10%,althoughmostpeopleareprotectedthroughrisingfinancialsubsidies.EstimatesbytheCongressionalBudgetOfficeprojectthatby2027,fivemillionAmericanswouldloseindividualcoverage;
