This paper analyzes a large family with Von-Hippel Lindau disease. Clinical manifestations consistent with VHL were found in 13 members of this family and all 5 affected family members who were genetically tested were positive for the Tyr112Asn (T547A; assumed to be reported according to AF010238.1 and mapped to c.334T>A [NM_000551.3]) germline mutation of the VHL gene. The other affected individuals were unavailable for testing or died before testing could be completed. Interestingly, 3 family members tested positive without clinical manifestations, but may be too young to present with features. Clinical manifestations in this family included: CNS hemangioblastoma, renal cell carcinoma (7/13 members), retinal hemangioblastoma, renal cysts, and pheochromocytoma. ACMG evidence codes: 'PP1' based on segregation results, 'PP2' because they observe a missense variant in genes where missense variants are common mechanisms of disease, 'PP4' because the patient's phenotypes and family history are highly specific for a disease with single genetic etiology.

KindredID: 1
Case: F, 20Y0M, Ethnicity Unknown
DiseaseAssertion: Asymptomatic
FamilyInfo: 13 affected individuals in this family were found. The p.Tyr112Asn (c.334T>A) mutation of the VHL gene cosegregated with the disease in all five tested affected individuals from the family (V-5, VI-1, VI-2, VI-3, and VII-1). 7/13 have had renal cell carcinoma. Other manifestations of the disease in this family include renal cysts, retinal angioma, and CNS hemangioblastoma. There was one case of pheochromocytoma (VII-1). No other pheochromocytomas or related neuroendocrine tumors of the pancreas or adrenal gland have been diagnosed or recognized retrospectively by review of medical records and recall of relatives. The mutation was also found in three asymptomatic relatives (VII-2, VII-3, and VII-4).

A previous study of 26 Japanese, VHL families was extended to 41 additional families. Germline mutations were detected in 55 of 77 Japanese families using SSCP, direct sequencing, and Southern Blot analysis of peripheral blood samples. The Y112N (c.334T>A) germline missense mutation of the VHL gene was found in 2 VHL family members with hemangioblastomas of the central nervous system, retinal angiomas, pancreatic cysts or tumors (unspecified) (family ID 74).
