    1. VHL DNA sequence analysis depicting part of exon 3 sequence of the VHL gene with the 491A>G mutation (arrow) in the 2.75-year-old girl (A) and her father (B). [Color figure can be viewed in the online issue, which is available at www.interscience.wiley.com.]

A 2.75 year old patient (32 month-old girl) presented with a pheochromocytoma. Genetic sequencing (not specified) revealed a c.491A>G (p.Gln164Arg) heterozygous germline mutation in exon 3 of the VHL gene. Sequencing of her mother, father, a sibling, and her paternal grandparents revealed that the mutation was present only in her father, and was de-novo in her father. Upon screening, her father (a 31 year old male) presented with a retinal angioma, an adenoma of right adrenal gland, a right-sided renal cyst, an epidydymal cyst, a contrast-enhancing lesion in the pancreas, and bilateral pheochromocytomas.

    1. A 6 year-old boy with von Hippel-Lindau (VHL) disease presented with hypertension due to bilateral pheochromocytomas. At age 13 he developed a renal carcinoma and bilateral paragangliomas, as well as clear cell RCC. His mother's age of first presentation was 39Y old and her last known age was 41Y old. She was found with retinal angiomas, bilateral pheochromocytomas and a cerebellar hemangioblastoma. Both had the same germline VHL missense mutation. This mutation was reported as c.704A>G and converted to NM_000551.3:c.491A>G. ACMG: PP4

      DiseaseAssertion: adrenal pheochromocytoma

FamilyInfo: Her one sister had died just after delivery, following a severe hypertensive crisis. Her 26-year-old niece presented with a 3 cm left adrenal tumor, an 8 cm pancreatic tumor and disseminated hepatic metastasis, evolving to death within a few months. Her liver biopsy was compatible with a moderately differentiated neuroendocrine carcinoma of uncertain origin. Molecular diagnosis was not performed in the affected sister and niece. Her asymptomatic 5Y old daughter tested positive for the mutation: Q164R (c.491A<G).

CaseHPOFreeText: She first presented with paroxisms of severe hypertension associated with headaches, diaphoresis and, ocasionally, fainting, started one year before. She had had acute pulmonary edema and a cardiorespiratory arrest during her first caesarean section. A CT scan revealed a right adrenal tumor (6.0 cm), showing increased 123I-MIBG uptake. After conventional adrenalectomy, a PHEO was immunohistochemically confirmed. Close follow-up has shown no signs of recurrence or evidence of new tumors.
