There is a typo that persists, in the caption of figure 1, which reads "abnormal white blood cell count (<12 000 or <400 cells/µL)"; clearly, the first "less than" sign is reversed, and it should read, "abnormal white blood cell count (>12 000 or <400 cells/µL)". While many readers will be sophisticated enough to understand this, the online version should be corrected.
