The disease prevalence at our clinic as measured by qRT-PCR on the two days of testing was approaching 50%, thus we are confident that the conclusions drawn from our sample size of 110 swabs are sound. Limitations of the study and future directions: There are several limitations to our study. The sample size of our cohort is relatively small and this reduces the confidence in our conclusions. However, this is mitigated by the longitudinal study design and the fact that we have focused on the systemic immune response in patients with the same asymptomatic/mild disease severity from the same households. Like any Phase 3 study done under pandemic conditions, the current trial had limitations, the most obvious of which are the limited number of older adults and smaller proportion of adults with comorbidities who were able to participate due to on-going vaccine roll-out programs and the relatively short period of follow-up due to the event-driven design. We acknowledge the lack of serial transcriptomic data from the same patients as a limitation of our work. However, our patients were enrolled during the early peak of the pandemic where in-person visits were minimized, making follow-up sample collection challenging. While we did not enroll a simultaneous non-KTR cohort, we used two different public non-KTR datasets to test the generalizability of our findings. We acknowledge that robust causal inferences cannot be made in our cross-sectional dataset. While overall signals of reduced T cell immunity during acute disease with subsequent recovery later were observed in our data, the signatures among individuals showed some heterogeneity. Our ability to molecularly detect signals consistent with reduced adaptive immunity could be exploited (and applied in external datasets) to develop and test biomarkers to potentially identify individual patients who would best tolerate IS reduction, and possibly guide timing of IS reinstitution. In summary, we demonstrate that blood transcriptomes of acute COVID-19 in KTRs suggest immune-insufficiency that is proportional to disease severity despite IS reduction. These signatures showed recovery after acute illness. These findings have implications for the pathogenesis and management, including IS reduction or antiviral approaches in COVID-19 infected KTRs.

Study limitations are that only European ancestry participants were studied as PRS(VTE) was developed within cohorts of European ancestry. Only patients that were COVID-19 tested were included in this study. The last patients that were tested in the study cohort were on June 1 2021 so vaccination status might play a role in the study. The serotype of viruses involved was not known. Treatment modalities had evolved during the study period. Diagnosis of clinical events in the immediate post index period might be affected by potential underutilization of imaging modalities required to make certain diagnoses. The latter was expected to dissipate however after a recommended short period of quarantine and with longer follow up. An important consideration in the issue of testing positive rates was exposure. It is unlikely that PRS(VTE) was associated with differential environmental exposure however. Strengths include a large sample size and geographically diverse population evaluated, the prospective nature of this study to examine testing (positive) rates and clinical sequelae of COVID-19 infection, and the use of genetic stratification to investigate post COVID-19 events. In summary, PRS(VTE) was associated with COVID-19 positive testing rates for EUR in this large prospective cohort from the MVP. The linkage of COVID-19 infection with thromboembolism processes is novel and may reveal mechanisms of early steps of viral infection and/or treatment and prevention strategies.

Strengths and weaknesses of the living systematic review methods: A strength of the methodology of this review is the transparent reporting, with openly available data and changes over different versions reported in the protocol. These preliminary findings should be considered with the known limitations of this analysis, including the small sample size that comprised primarily older participants and may not reflect the range of NAb titers observed across different age and demographic groups. Moreover, the assay used in this analysis was research-grade but had limited optimization against the B.1.1.529 virus prior to this analysis [21]. Additional studies assessing durability of NAb titers against B.1.1.529 after an mRNA-1273 booster dose and corroboration of these results with real-world effectiveness data are needed. These findings are consistent with a growing number of reports indicating that COVID-19 vaccines have reduced neutralizing activity against the B.1.1.529 variant [2, 4-8, 23], thus further emphasizing the need for a booster dose. Additional evaluation of vaccine-induced immunity among a representative population, as well as an understanding of the clinical characteristics of this newly-identified VOC, are required. Further insight should be gained by an upcoming analysis of a broad panel of sera collected from mRNA-1273 vaccine recipients 1 month after the primary series and 1 month after an mRNA-1273 booster dose (50 µg or 100 µg) using both live virus and pseudovirus neutralization assays. These data can allow for direct corroboration of neutralization results and provide more comprehensive information on mRNA-1273 vaccine–induced immunity against the SARS-CoV-2 B.1.1.529 variant.

Some limitations apply to this study. The study period runs over Christmas 2021 and New Year’s Eve 2021/22, which are public holidays in Denmark. Despite government advice to limit social activity, it is likely that there has been considerable social mixing with family and friends outside the households during this period. Social mixing over the holidays in conjunction with the high incidence levels in Denmark during this period likely means that some secondary cases in this study are actually misclassified co-primary cases, i.e. infections picked up outside the household and testing positive after each other. 4.1 Limitations: There are several limitations in the preliminary evaluation study performed on the mEP respirator as the generation of SARS-CoV-2 bioaerosols necessitated the use high containment (biosafety level 3) laboratory environment and unique aerobiology facilities configured for studies with high consequence pathogens and not necessarily respirator efficacy testing. The configuration conventionally used to test efficacy (NIOSH standard 42 CFR 84 also referred to as “ Part 84”) requires the use of high flow input (≈85 LPM) as inlet flow across the face of the filter substrate being tested for removal efficiency. The high flow simulates pressure drop associated with the velocity of inspiration experienced by the user when breathing through a filter-based respirator. The mEP respirator is configured whereas there is near inperceptable pressure drop upon inspiration, thus there is no utility for the use of high flows for the purposes of evaluation. Rather, the inlet flows used in this evaluation approached but did not exceed 6 LPM during the studies. The disparity between the flowrates used in the NIOSH testing and this evaluation can be considered a weakness of the study. Similarly, although the mEP and the N95 respirators were tested under the inspiration configuration (6 LPM either across the filter substrate of the N95 or through the inlet of the mEP), neither respirator was tested for efficacy for particle removal efficacy upon expiration. Our study had several limitations. As children were recruited weeks or months after infection, no PCR based confirmation from the acute phase of infection was available. As previous infection was defined through seropositivity alone, a combined positivity in three independent serological tests was required and this approach should minimize the possibility of false positives. A further limitation based on the retrospective study design is that a detailed history from the time of infection as well as the exact time of infection could only be collected for a fraction of participants. Because of a reluctance in small children and their caregivers, as well as the technical difficulty regarding blood draws, younger age groups were relatively underrepresented. Also, CD8+ T cell responses were not investigated - these could provide important insights into cellular anti-SARS-CoV-2 immunity and should be the focus of further studies.

Some limitations of our study are the small numbers of study subjects in some of the groups and grouping together of drugs by class. T cell responses, including IL-4, IL-2 and IFN-γ production, showed a significant correlation with RBD and spike - specific antibody responses. There was substantial induction of T cell cytokines and release of cytotoxic molecules following spike peptide pool stimulation of PBMCs collected following one dose of vaccine. Some subgroups and parameters showed increases in T cell cytokines with two doses. Multivariate analysis of the data showed that several groups had decreased IFN-γ after dose 1 of vaccine, but these deficits were largely corrected following the second vaccine dose. When data were pooled for all subjects, it was apparent that the IL-4 response was particularly dependent on 2 doses of vaccine and showed a waning trend 3 months later. As IL-4 is an important mediator of B cell proliferation, which in turn impacts antibody levels and B cell memory,28 the lower IL-4 response after 1 dose as compared to 2 doses of vaccine highlights the need for second doses to maximize B cell responses. Taken together, our study shows generally robust T cell responses in most patient groups treated with immunosuppressants or biologics after 1 or 2 doses of mRNA vaccine, improving somewhat with a second dose but with some attenuation by 3 months after the second dose. We first describe the limitations of our study. The analysis and hospitalisation forecast assumes that the Omicron variant of SARS-CoV-2 is the dominant variant. However, if the Delta variant is still prevalent, hospitalisation and the severity estimations may differ from those for the Omicron variant. Regular and timely sequencing data will tell us whether the Delta variant is still in circulation or not. Further, our study focused only on cases and associated estimates of required hospital beds. In particular, we have not examined several other important factors, e.g., workforce shortages, overcrowding events in enclosed spaces, etc. Moreover, our model has a homogenised contact rate for the entire week, and the impact of the curfew is modelled as a reduction in the weekly contact rate. Finally, as already highlighted, our assessment of the curfew is based on Google’s published community mobility reports which may suffer from an inherent sampling bias. With these caveats highlighted, we now discuss the outcomes of our simulation case study for Bengaluru Urban. Reduced testing levels or increased home testing may bring the case counts down, resulting in an overestimation of the predicted cases. But estimates of hospitalisation and ICU beds with ventilators may yet be robust since individuals needing such care may eventually seek healthcare support.
