Limitations of our study were based mostly on subjective interpretations about the feasibility. As our analysis compares representativeness with the population, it is impossible to directly compare the representativeness of samples recruited with mobile phone-based with different recruitment methods. Another perceived limitation was the population’s distrust in receiving non-official and unexpected phone messages. Moreover, we can not account for the real number of delivery messages due to limitations as internet failure, missing mobile phone numbers, or interrupted plan services, especially in the low-income population. We believe there are many potential applications of mobile phone-based advertising for researchers, governments, and community-based organizations who wish to learn about the populations they serve. Besides, we believe that these institutions should previously officially advertise the target population to promote a wider adhesion and confidence to respond to the survey.

However, our study is not without limitations. First, in the absence of a national testing strategy during the study period (September 2021), the capturing of COVID-19 infections may not be standardised nationally, although testing remained high. Second, the lack of adequate genomic surveillance in Malaysia rendered impossible to ascertain variant-specific vaccine effectiveness, and their variation, if any, over time. Our study period also preceded the detection of the Omicron variant of concern. Moving forward, we recommend that the monitoring of vaccine effectiveness over time remains prioritised to guide policy decisions on the rollout of booster dose(s). To build on our study findings, monitoring of effectiveness for other vaccine platforms used in Malaysia and among adolescents, which were first offered the COVID-19 vaccines in September 2021, should be initiated. Genomic surveillance efforts should also be strengthened to enable investigation into variant-specific vaccine effectiveness. Finally, as booster doses are rolled out, the performance and safety of the booster doses should be assessed to furnish data needs for policy calibration. There are several limitations to our review. First, since there was little to no information regarding the dominant SARS-CoV-2 strain among the studies, circulating SARS-CoV-2 strains were not accounted for in our review. mRNA vaccine VE against SARS-CoV-2 infection is decreasing with increasing report of breakthrough infections 4, 5, 14. This is likely due to both waning immunity occurring months after vaccinations, and an emergence of SARS-CoV-2 variants which become predominant during the study period. Several SARS-CoV-2 variants harboring mutations are associated with decreased mRNA VE against infection and/or severe COVID-19 22. Therefore, changes in circulating SARS-CoV-2 strains might have affected VE in the “middle” and “late” studies. Second, only U.S.-based studies were included. Our review focused on these studies for several reasons: 1) the initiation and progression of vaccination campaigns varied between countries; 2) some countries applied a modified vaccine regimen (e.g. prolonged interval between 1st and 2nd dose); and 3) the predominant circulating SARS-CoV-2 strain differs between countries. Future reviews should broaden scope to include studies conducted outside the U.S. Third, definition of VE (e.g. 7 vs 14 days after second dose, hospitalization vs death) and study design (e.g. test-negative, cohort registry) varied between the studies. Study limitations included single-arm, nonrandomized, open-label design with external historical controls, and between-group imbalance. 34 of 347 screened patients were enrolled (10%). Median time to discharge was shorter with telmisartan, 9 vs. 15 days (p=0.01) and 30-day mortality trend favored telmisartan (5.26 vs. 11.76%, p=0.41); there were no differences for ICU admission, mechanical ventilation, and a composite of ICU admission, mechanical ventilation, and death; fewer patients receiving telmisartan needed oxygen at day 15 (2/4 vs. 13/14, p<0.05). No telmisartan related AEs occurred and BP was similar. 82 of 185 screened patients were enrolled (56% [higher than our trial]). Some caveats need to be acknowledged. The most important limitation of this analysis is that the large trials may not necessarily be a perfect gold standard. No single clinical study can claim to possess the perfect truth, no matter how well it is conducted and how well it is protected from bias. The CIs of the large trials cannot exclude very small benefits on survival – or small harms. These trials have also shown no benefit also on other outcomes. However, small benefits (or harms) for these outcomes are also not possible to exclude with perfect certainty. Moreover, beneficial effects for some treatments may still exist in circumscribed, special circumstances, with different dosing regimens, and in specific patient subgroups that may have been outside the eligibility criteria of the large RCTs or may have been under-represented in these large RCTs. One limitation could be that our data was skewed with respect to the protected features. As we are using neural networks, different distributions of the protected label can give significantly different results for the adversary model. This has previously been discussed [16], as using balanced data was found to have a much stronger effect on adversarial training. Thus, future experiments would greatly benefit from balanced training data. With respect to debiasing against ethnicity, one limitation is the ambiguity of certain categories, namely, “Unknown,” “Mixed,” and “Other.” In our experiments, we kept these categories in order to maximize the number of cases (especially COVID-19 positive cases) used in training. Limitations included absence of placebo control and published mitigation strategies to reduce QTc prolongation (e.g., excluding baseline QTc prolongation and co-administration of additional QTc prolonging medications [100% received azithromycin]), single-center design, small sample size, and baseline imbalance. Prolonged QTc was also reported in two additional retrospective case series of hospitalized COVID-19 patients treated with HC with or without azithromycin. In the small French series of 40 patients, baseline QTc>460 msec was an exclusion. 93% developed some QTc prolongation; 36% developed more severe QTc prolongation (more commonly with concomitant azithromycin [33% vs. 5%, p=0.03]). No ventricular arrhythmias including torsades de pointes occurred. Seven patients (17.5%) stopped medication due to adverse events, ECG changes, or acute renal failure [15]. In the Boston series in 90 patients, combined therapy was associated with a larger median increase in the QT interval than HC monotherapy (23 vs. 5.5 msec, p=0.03), resulting in 13% vs. 3% of patients, respectively, having QTc change >/= 60 msec. The risk of QTc prolongation to >/=500 msec was similar (21% vs. 19%). The authors implied a baseline QTc prolongation exclusion. Ten patients (11%) discontinued medication because of adverse events (nausea, hypoglycemia, and one case of delayed torsades de pointes) [16]. There are also some limitations in this study. First of all, for cohort 1 (the training set), the patient population we studied was mainly hospitalized patients, and they generally exhibited more severe symptoms and therefore had a higher mortality rate than the general population, which may have caused some bias in our predictive model in the general population. Second, the characteristics of the cohort may vary performance of models and its ability to be validated. For example, the model’s performance was slightly lower in cohort 2 than cohort 1, because the structure of the two cohorts, such as age distribution, sex ratio, mortality rate, etc. is different. In addition, although we adopt functionally similar features, the differences between these features may also be responsible for the difference in model performance between cohorts. Moreover, since most of the clinical features adopted in this study were missing to varying degrees, the imputed data were affected by other data, which may affect the accuracy of the predictive model. Our study is subject to limitations. First, like all observational SARS-CoV-2 studies, we could not implement routine universal SARS-CoV-2 screening to identify all patients with SARS-CoV-2 infection. This limitation is problematic especially for prognostic analyses, because of uncertainty regarding the proportion of included patients being asymptomatic at diagnosis and their COVID-related symptom burden at the time of diagnosis. The observation that median days to hospitalization after estimated date of infection did not differ significantly by HIV status provides some support that trajectories of disease progression were similar for both groups. Second, to reduce bias in our analyses due to missing records, varying standards of care, and referral indication, individuals were excluded if part of the COVID-19 care cascade occurred outside of UCSD Health. According to custody health officials, incarcerated individuals could ask custody staff or medical personnel to exchange their soiled or torn mask for a new one without limitation. However, our findings suggest that in practice, access to new masks was extremely limited and highly variable across individuals. Relatedly, and perhaps consequently, most incarcerated participants felt unable to protect themselves from COVID-19. Our findings emphasize the need for systematic implementation and oversight of mask availability and distribution in these settings. Both counties surveyed here recognized the importance of jail de-densification to COVID-19 control and subsequently extended California’s emergency bail schedule for lesser charges, leading to population reductions of up to 45% [30, 31]. However, many who remained in custody experienced severe hindrances to their cases due to COVID-19, citing substantial delays and cancellations of court dates. These findings correspond to those of a recent investigation into the backlog of cases in California that has been exacerbated by COVID-19, which found that over a quarter of unsentenced jail residents in the two counties we studied had been in custody for more than a year awaiting arraignment, trial, or sentencing [32]. There are a number of limitations to these data and analyses. First, Ct values report viral RNA copies and do not necessarily capture infectiousness. While we chose a Ct value cutoff of 30 as a proxy for infectiousness13–17, this threshold should not be considered a perfect predictor of infectiousness. Our study had limitations. The transmission rate of SARS-CoV-2 may vary according to different durations of exposure. In this study, we selected 6 hours of non-contact transmission to simulate the scenarios of staying with an infected index patient within the same facility for a routine business day and on medium-haul flights. It would be worthwhile to further compare the transmissibility of the Omicron and other variants after different durations of exposure in future studies. It would also be important to investigate the pathogenicity and transmissibility of the Omicron variant in additional animal models such as the hACE2-transgenic mouse and non-human primate models, as each of these animal models have their advantages and disadvantages in recapitulating human disease. In summary, the present study shows that despite comparatively lower pathogenicity than the Delta variant, the Omicron variant undoubtedly still causes obvious clinical effects, increased viral burdens and pro-inflammatory cytokines/chemokines, as well as histopathological damages in infected hosts. Taking into consideration the Omicron variant’s higher transmissibility than the already-highly transmissible Delta variant, our findings highlight the urgent need to find next-generation COVID-19 vaccines and broad-spectrum therapeutics, as well as to tighten non-pharmaceutical measures to reduce acute and chronic disease burden (long COVID) on the public and healthcare facilities.

We drew on the experience of other, clinical research teams operating in this environment, and adopted a pragmatic approach to study design, opting for an agile, scalable tool which allowed the capture of data which has clear limitations but nevertheless provides unique insight into mental health impacts on staff during a unique period of operational stress in the NHS. There are several limitations to this study. First, a selection bias may limit the generalisability of our findings. This study was conducted at a single centre and a single country. ISRR can be affected by environmental factors of the vaccination centre and can occur in clusters or group settings.26 Therefore, the incidence rate of ISRR cannot be specified in a mass vaccination centre. As is the case for all PCR-based genotyping methods, there are limitations to the utility of this RT-ddPCR assay. The example of ACG->ACA mutation in K417T illustrates that assay accuracy is subject to change with additional mutations. There is also no guarantee that the particular set of mutations of concern detected by the assay will continue to be mutations of concern in the future. However, direct targeting of those mutations of concern, rather than relying on larger-but-less-relevant genomic changes, increases the likelihood that the assay will continue to be useful to researchers and clinicians alike. We have previously used RT-ddPCR to identify SARS-CoV-2 Alpha variant in clinical specimens, but here we expand both the number of mutational lineages that can be tracked with the assay and our understanding of how robust the assay is. It can accommodate a wide range of sample concentrations, coinfections, and other confounds, and still yields an accurate determination of SARS-CoV-2 mutations quickly enough to enable clinical decision-making.

Our study has several limitations: First, the observational, retrospective nature of this study of patient EHR data could introduce case selection biases, over representation of symptomatic testing, reporting and follow up issues. During the COVID-19 pandemic, autopsy-driven research, using multimodal approaches, attempted at defining organ tropism of the virus and at unraveling organ-specific pathomechanisms 8,9,25,41-43, yet a critical and systematic study investigating the limitations of in situ detection of SARS-CoV-2 in autopsy tissues has not been performed. Looking into the discrepancy of results from studies using autopsy material to determine organ tropism and especially identification of organ-specific target cell types of SARS-CoV-2 for most organ systems led to conflicting results hampering research progress 3,8-10,44,45. For instance, published data include studies revealing direct infection of neurons by SARS-CoV-2 with substantial neuroinvasion 46, single infected cells in a subset of patients 25, but also the absence of virus and COVID-19-specific alterations 47. Validations are complicated by the use of various, often not well-evaluated antibodies in different studies and the lack of sufficient positive and negative controls. Here we determined the limitations of SARS-CoV-2 detection by IHC and thin section EM using a defined set of control tissues including FFPE highly susceptible human cell lines and autopsy tissues with expectable high SARS-CoV-2 viral load.
