The role of inflammation along all spectrum of severe illness associated with SARS-CoV-2 infection should be deeply studied, including its role in myocardial disfunction since it is a frequent complication both in adults with COVID-19 and children with PIMS. This study was subject to several limitations. First, we were not powered to assess vaccine effectiveness by age groups. We compared effectiveness in individuals above and below the age of 50 and did not observe major differences. Second, there were differences in the timing of introduction and eligibility for each of the vaccines. This should prompt some caution in the comparison of effectiveness between vaccines, as the calendar period and median duration from second dose differed somewhat between vaccines. For example, if effectiveness wanes over time, vaccines used earlier would have lower effectiveness than those introduced later. Additionally, changes in variant distribution during the study period could alter effectiveness by time since vaccination. We did not have individual level data on variants, which precluded assessment of variant-specific vaccine effectiveness. We used a matched, test-negative design with multivariable regression to reduce non-vaccine related differences between the cases and controls; however, there could be unmeasured differences that lead to confounding. Finally, an important question is when vaccines should be given to individuals with previous SARS-CoV-2 infection, and our study was unable to address this. We did not identify a high risk of admission in those with a history of immunosuppression (also in keeping with previous studies);30,32 although an important caveat to this finding is that there were relatively few of these admissions among our cohort of >17,000 children. Due to the large number of children managed by virtual care, another limitation of our study was that we were unable to characterise all the demographic and medical co-morbid and clinical features among cases of all the children under this model of care; but selected a random sample that was likely representative of the larger cohort of children. Our data revealed that obesity was associated with ICU admission, but this risk factor could not be evaluated in our multivariable model due to sample size. A history of asthma or viral induced wheeze was not associated with an increased risk of hospitalisation, yet other chronic respiratory conditions were - including chronic lung disease and obstructive sleep apnoea. This finding is consistent in the literature,3,4,6 and may be secondary to the TH2 predominant inflammation seen in asthma, with its associated IL-4, IL-5 and IL-13 response – the latter of which has been shown to downregulate the expression of SARS-CoV-2 host cell receptors.33 Our data revealed there are two dominant paediatric age cohorts at increased risk of medical admission due to SARS-CoV-2 infection. Our study had several limitations. These included the absence of characterization of cross-neutralizing antibodies in vaccinated mice against the B.1.1.529 Omicron variant, which emerged during the writing of this manuscript. However, we could expect that our vaccine would elicit T-cell responses against N epitopes because of the high homology (100%) between vaccine sequences and this VOC. Due to the limited availability of hCD40/K18-hACE2 mice, we did not evaluate various VOC challenges in mice. Finally we favored the analysis of T cell responses using samples from recovered individuals instead of in vivo preclinical models. Although these responses may be dependent on the “clinical history” of patients and their HLA haplotypes, they are less biased than those that would be observed in an animal model. Our results show that the in-vitro vaccine responses are directed against all vaccine proteins, which confirmed the broad HLA coverage of the vaccine sequences. In conclusion, it is becoming urgent to develop a “pan-sarbecovirus vaccine”. Several limitations of our study warrant mention. First, the cohort was too small to allow multivariate analyses. Second, as the disease status in our cohort was stable, safety and immunogenicity in patients with relapse or receiving high-dose steroids for acute or chronic GVHD are uncertain. Third, the precise titer border line providing clinical protection for COVID-19 remains unclear. However, we believe that many Japanese HSCT patients would have gained sufficient immunity, as did the healthy volunteers. To our knowledge, this study is the first prospective observational study on the safety and immunogenicity of BNT162b2 in Japanese patients who received allogeneic HSCT. Most HSCT patients obtained certain immunogenicity without severe adverse events, whereas patients taking steroids or with low lymphocyte counts did not yield sufficient antibody titers. Further investigation to confirm our findings are needed, and long-term monitoring of antibody levels is warranted.

As already pointed out by DiPiazza et al, drawing conclusions from animal models and extending these observations to the immunological situation in humans may be a difficult task (DiPiazza et al., 2021), considering the limitations of the system, especially in the absence of experience with vaccines targeting SARS-CoV and MERS-CoV and evidence for VAERD of all vaccine concepts including whole-inactivated viruses in humans. However, we were able to replicate VAERD induced by a protein based COVID-19 vaccine candidate with a very similar phenotype in a second animal model, Syrian hamsters, using an unmodified low-passage virus isolate. Therefore, our data strongly support the idea to monitor vaccinated human patients that experience a break-through infection closely. In any case, while our experimental vaccines mimic, but are not the same as the authorized vaccines, these and previously published data by diPiazza et al. point at considerably few concerns for vaccines developed to trigger TH1-biased responses such as viral vector platform-based vaccines and mRNA vaccines. Moreover, even if VAERD as observed in our model should occur in human patients, this immunopathology would be treatable by dexamethasone, which revealed to be an effective medication for severe courses of COVID-19, anyway (Tomazini et al., 2020; Horby et al., 2021). While this study, to our knowledge, is the first to use an advanced dimensionality reduction approach to understand relationships between biomarker patterns and clinical phenotypes during the inflammatory phase of COVID-19, there are limitations worth noting. Samples were collected from April 2020 to January 2021 and treatment practices and epidemiologic changes over time may have affected inflammation patterns. Hence, we incorporated a sensitivity analysis excluding those that received systemic steroids in Cluster 1 to aid in interpreting the findings. In addition, the sample size may limit our ability to identify uncommon biomarker patterns and external validation is needed of patterns identified. Additionally, regression was used to adjust for peak severity to identify biomarker and comorbid condition associations with TDA clusters distinct from severity trajectory differences. While this is a novel feature of this biomarker study, residual confounding related to peak severity remains possible. Despite limitations, results presented here are hypothesis generating and should be evaluated further in additional cohorts. This study has some limitations. First, incidence data of breakthrough infections in the general population, as well as those of non-breakthrough infections in unvaccinated people, were not available for comparing these rates with those of our pwMS cohort. In solid organ transplant recipients, the diminished antibody responses to SARS-CoV-2 resulted in 41-to-82-fold (depending on the statistical approach) higher risk of breakthrough infection vs general population17. Second, we have no data on SARS-CoV-2 molecular characterization, as the Delta variant reduces vaccine effectiveness17, but high enough SARS-CoV-2 antibody titers can protect against infection and severe outcomes18. In conclusion, the results of this study suggest that the rates of breakthrough infections in pwMS on DMTs at least partially depends on the level of humoral immunity to SARS-CoV-2, which, is reduced in patients under specific DMTs and that declines over time. Larger cohorts are needed to understand the protective role of vaccination for severe Covid-19 in pwMS under different DMTs. Limitations: A major limitation of this study was the absence of a commonly used psychophysical test to assess the extent of chemosensory dysfunction. Therefore, our results obtained with the CPT were more difficult to compare directly with other studies using psychophysical testing. Unfortunately, in clinical practice, psychophysical evaluations are seldom used for the diagnosis and follow-up of chemosensory impairments, which are mostly treated based on subjective assessments. While we encourage the use of validated psychophysical tests in primary care, neurology and ENT clinics, findings in this study confirmed a reliability of questioning olfactory functions subjectively especially in the context of follow-up of COVID-19 patients. An additional limitation of this study was the high prevalence of chemosensory disorders during the acute phase of COVID-19. Indeed, we observed higher participation rates among individuals with chemosensory disorders than those who have either recovered or simply never noticed any OD, GD or TD. Our study sample had a 20% higher rate of OD and GD, thus probably over estimating prevalence of chemosensory disorders at 11 months.

Despite limitations associated with this review (e.g. low number of studies) we believe that our findings are highly relevant as they provide a rather consistent picture on antiviral and preventive benefits of Echinacea in coronavirus infections overall. Also, they provide important implications for the preventive use of Echinacea during Covid-19 epidemic. The reduction of coronavirus loads is medicinally highly relevant in whereas virus concentrations appear to correlate with community transmission, influence illness severity and progression in adults and children [41–44]. Two clinical studies have shown over 98.5% reduction of coronavirus concentration in nasal secretions obtained from children and adults treated with Echinacea. Evidence is still missing that vaccines significantly reduce viral loads, especially of SARS-CoV-2 delta variant and this asset would be a decisive argument for use of Echinacea during Covid-19 pandemic. Notably, all cited studies applied the same Echinacea purpurea extract (Echinaforce®) either as diluted tincture or as tablets, both of which were kept in mouth for a while prior to swallowing. As already hypothesized by Signer et al., pharyngeal administration of the extract may be key to inactivate respiratory viruses at the main entry site prior to infection [19]. The 4C Score was chiefly used to inform treatment escalation, but was also used to inform decisions on patient destination (i.e. discharge or admission location) as well as treatment limitation. Although half of participating sites incorporated the 4C Score within guidance, documentation of the score varied substantially between sites. Audits of documentation of established mortality risk scores such as CURB-65 (for patients with community-acquired pneumonia) have also demonstrated variable recording (8,9). Nevertheless, our findings suggest that documentation of the 4C Score may be encouraged by incorporation into clinicians’ workflow through admissions proformas, detailing 4C Score variables or online calculator within guidance, in addition to linking the score to management recommendations. This project demonstrates the capacity of national trainee networks to facilitate multi-centre studies. Collaboration with NITCAR and other trainee networks enabled the rapid recruitment of 41 UK sites and completion of >4,000 case note reviews within a short timeframe. Strengths and limitations: In Sweden, geriatric hospitalizations are indicated on criteria of biological (and not chronological aging). Non-frail and non-comorbid older patients may have been hospitalized in other clinics (eg. infectious diseases) and not included in our cohort. Nine out of eleven existing geriatric clinics in Stockholm participated in the study. Living situation, comorbidities and medications were obtained from electronic health records with imperfect ascertainment. Medications were considered present if they were present or prescribed within 24h of hospitalization and would have included both newly prescribed medications and those removed after admission. We did not have information on vaccination, which would have been useful for assessing the decline of mortality in the third wave of the pandemic. Strengths of this study are the large geriatric cohort including nine out of eleven geriatric clinics in Stockholm treating patients with COVID-19. Also novel is the analysis of geriatric hospitalizations for other causes and the long study period including three pandemic waves over a period of 17 months.

Limitations: This study has several limitations. First, our sample is not representative of all essential workers in the Republic of Macedonia. However, a representative sample of essential workers would be very difficult to obtain given the multitude of job categories included in the list of essential workers and the lack of data on how many people work within each category in the nation. There are strengths and limitations to our analysis. Strengths include that we obtained results from each of the trials to inform both the meta-analysis and the decision-analysis. As with all trials, the demographic and disease characteristics of the population enrolled may differ from those seen in other health systems, particularly in vaccination status. Usefulness and limitation of the positive count method: Although SARS-CoV-2 RNA concentrations in wastewater were lower than the LOQ, the number of positives in repeated RT-qPCR reactions could trace the prevalence of COVID-19 in the community. In conventional qPCR testing, the limit of quantification is usually 5 to 10 copies per reaction. In contrast, the positive count method could distinguish lower copy numbers, i.e., 0.1 to 4 copies per reaction, which means that it increases the sensitivity of qPCR by 50 to 100 times to reveal the trend of viral RNA concentrations in wastewater. For the purpose of tracking the virus infection dynamics in the community, exact values of virus concentrations are not always necessary; it is enough to see the change of viral RNA levels in wastewater, for which the positive count method is useful. We used the positive count method with RT-qPCR of the supernatant fraction of wastewater. This method can also be used with the solid fraction of wastewater. Recent studies have improved the sensitivity of viral RNA detection in wastewater by RT-qPCR by concentrating virus from the solid fraction of raw influent or primary sludge rather than the water fraction.6,8-11 However, in regions where the virus concentration in wastewater is not high enough for quantification from the solid fraction, the positive count methods might be useful to reveal changes in viral RNA levels in wastewater. Our modeled estimates rely on multiple publicly available data sources, each of which has its own limitations. For vaccination coverage, we used data from Merritt et al27,28, which endeavors to address known biases in the CDC vaccination data. We further adjusted these vaccination data to assure that no greater than 100% of the over 12 population could have been vaccinated by the end of our study period. While most indicators suggest lower cumulative infections among children, there is evidence that contradicts this and suggests a higher seroprevalence for children compared to adults40-43. For our infection rate estimates, we used a model which leverages case notification and COVID-19 mortality data from Johns Hopkins University and have incorporated modeled uncertainty in these estimates into our model of immunity. The model for infections assumes individuals can only be infected once, so possible reinfections and breakthrough infections amongst vaccinated individuals are not accounted for in our estimates of immunity. One limitation of our study is that for some VOCs, e.g. Beta, almost all the sera dropped below the limit of detection of our assay (ND80 of 10), providing less granularity, complicating comparisons with other studies, and obfuscating correlations with more sensitive Ab-detection assays such as the RBD-ELISA. Elsewhere, our data on Omicron neutralisation following 2 doses of BNT162b2 fits with recently reported findings. Dejnirattisai et al., Cameroni et al., and Garcia-Beltran et al. all showed drops in neutralisation of >30-fold [18-20, 30]. However, it is clear from our data, and that of Garcia-Beltran et al., [20] that boosting with a 3rd dose of BNT162b2 generates a much higher overall titre of neutralising antibodies to D614G (median ND80 ≥3070.9 in the 70-79 age group, Figure 4B) and that these high titres enhance cross-protection against Omicron and mitigate against the significant drops in neutralisation seen after two doses, highlighting the important role of boosters in providing robust long-term immunity to SARS-CoV-2. A smaller pool of antigenicity data is available on the wide range of other variants (VUIs/VUMs) that have emerged since the beginning of the pandemic. Limitations: We have not considered compliance in this model. This is explicit as compliance is a complex and multifaceted behavioural science problem which is far beyond the scope of this simple physical-system model. Secondly, while the sensitivity of rapid antigen tests is well known within laboratory conditions, quantifying this in real-world situations is not trivial. To overcome this, we have incorporated some uncertainty into the model by using a random variable based on real-world conditions to describe the rapid antigen test sensitivity assigned to each simulated individual. Lastly, we have considered time within this system from a purely mathematical viewpoint. That is, days are 24-hour periods and the testing/releasing of simulated individuals occurs exactly at unit periods. In conversion to the real-world we understand that should someone begin what they consider day 0 at 23.59 and conducts their tests at 07:00 this will shorten the time window compared to the model. Figure 1 shows the results are still reasonably robust in this scenario however care is required in translation of time periods.

Although our study shows surprising results that are possibly impactful for COVID-19 patients’ outcomes, there are also some limitations that should be mentioned. Our cohort comprises patients and biological samples that were collected in Germany earlier in the pandemic. Therefore, it is important to initiate similar studies with additional subjects to confirm whether our findings can be generalized to patients from other countries.
