Limitations: Our study has a number of limitations. First, we used a convenience sample that is not representative of the general population in Greece. For instance, educational level of our parents is higher than that of the general population. Also, we collected information through social media and parents without social media accounts could not participate in our study. Thus, generalization of our results should be made cautiously. Second, we conducted a cross-sectional study but the situation regarding the COVID-19 vaccination programmes, the dynamic of the COVID-19 pandemic, and the associated policy measures is changing fast. Our study has several limitations. First, our definitions of contact may miss certain transmission routes, e.g., connections via healthcare workers35,36, indirect transmission over surfaces, undetected patient carriage37, or non-room/ward/building contact. Second, since our training and testing period occurred largely prior to the UK’s vaccination rollout, we were unable to include vaccination status as a patient variable. However, we must acknowledge some limitations. Firstly, the uncertainty ranges communicated in our messages were all drawn from existing reports. While this provides some ecological validity, it did mean we were not able to control for the magnitude of the range presented across some conditions. Previous research suggests people may be less trusting of uncertainty presented as a very large numeric range (Kreps & Kriner, 2020; van der Bles et al., 2020). More research is needed to identify the effect of range magnitude and how this varies across contexts. Secondly, Studies 2 and 3 were only conducted in UK samples. As shown in Study 1, there is between-country variation in how people respond to uncertainty. Therefore, we cannot confidently generalize our UK findings to other national contexts. Future research should seek to replicate these findings with other countries as well as investigating possible explanations for the variation seen in Study 1. Lastly, the types of uncertainty we examined in the current research were narrowly defined. Thus, our findings may not map onto to other forms or formats of uncertainty. We acknowledge some limitations in our study. First, the SIVEP-Gripe database contains only individual information of hospitalized severe acute respiratory illness patients, not informing the daily number of infection cases in each federative unit and the respective outcomes. Therefore, we limit ourselves to analyzing data from hospitalized patients. In common with other serological studies, our study highlights the limitations of estimating the prevalence of natural infection based on antibody titres against nucleocapsid protein. 25 Direct comparison of seroprevalence estimates between studies and countries is challenging due to variation in the timing of pandemic waves and waning immunity. Our study has several limitations. First, our data analysis includes only retrospective data. Despite our efforts to avoid potential bias by separating training, validation, and test datasets as well as external validation on a different data source, potential biases are inevitable. A prospective validation study is warranted, ideally, in hospitals that did not participate in data sharing with the database that we used to secure the validation of transferability. Second, our model focused only on the admission data to predict in-hospital clinical outcomes. It is possible to use multiple time points during the hospital stay to update models to achieve “real-time” predictions. Because minimal data preprocessing is required, our model can be easily modified to use different data points to predict future clinical outcomes. Third, we performed only a preliminary evaluation for the model predictions explanations, whereby we extracted data from 20 random sample patients and presented their predicted risk scores as well as the contribution score assigned for each medical event and asked infectious disease specialists to evaluate its relevance. Our study has several limitations, this was an observational, retrospective, single-centre study, and collection of data was not standardized in advance. The sample size and the absence of differences in the characteristics of the global cohort of patients admitted to our hospital during the duration of the study reinforce the present data. Multiple factors can condition changes in microbiota, including the use of antibiotics. Nonetheless, the design of the statistical analysis adjusted for the use of antibiotic therapy in the 3 months prior to the inclusion of the study, allowing us to limit this bias. The exclusion of these patients from the study would have greatly limited the external validity of our results. Finally, the 16S ribosomal RNA amplicon sequencing approach to study the microbiota could introduce bias in the obtained data because this method does not allow the study of the whole microbiome, but only the genera amplified by PCR. Nevertheless, it is the most common technique to study microbiota in clinical samples. Moreover, the microbiota bioinformatics analysis has not been standardized yet, which hampered comparison interpretations of our results. Given sample size limitations, limited variation in some policy adoption, and temporal variation in the progression of COVID-19 to death, we are limited in our ability to attribute deviations in daily death counts to specific policy actions. An additional consideration is that NPIs associated with decreasing case velocities but not associated with decreasing subsequent deaths may signal that case decreases occurred disproportionally among younger individuals with less risk for COVID-19 mortality. This may be particularly true for public mask mandates, which were significantly associated with decreased case but not mortality burden in adjusted models. Our modeling approach allowed us to evaluate the merits of various NPIs concomitantly in a time-dependent fashion. Limitations: There are a number of limitations in this study. First, this analysis was based on aggregate data reported by the surveillance system of COVID-19 in South Carolina. The data was arranged by date of report. Even though we shifted the date backward by 9 days to approximate the date of infection, this remained an estimation. Second, date of report is affected by holidays, on which days cases were not reported. Third, the effects of viral variants on transmission potential44 cannot be shown in this study. The first two cases of the Beta (B.1.351) variant in the U.S. were detected in South Carolina after the study period ended,46 so this may not be a severe limitation. Fourth, while re-opening of schools was staggered by grade in South Carolina, we lumped the re-openings together as we chose the first date of the re-opening as the date of policy change. However, for the county-level policy change analysis, we had specific school re-opening dates for all nine selected counties (Figures 2 and 3). And finally, we do not examine the impact of vaccinations on the transmission potential in South Carolina; however, our data was right-censored on January 10th, 2021, by which point there were minimal numbers of people fully vaccinated. There are several limitations of our study. First, our study includes substantially more female than male participants, reflecting the gender distribution among HCW at our institute. Second, the age distribution in the four groups is not identical. In particular, the AZD1222 group is, on average, considerably older as a consequence of restrictive use of the AZD1222 vaccine in individuals aged 60-64 years in the Netherlands. As immune responses tend to become weaker with higher age, this is a relevant factor when considering the weaker responses in the AZD1222 group. Finally, the samples we tested were taken at the expected peak of immunity. It will be relevant to study the durability of the neutralizing antibody responses after vaccination with each of these vaccines. Some studies suggest that immunity induced by adenovirus vaccines might be more durable than immunity from mRNA vaccines8,45. We have only analyzed known VOCs and VOIs and cannot predict how our results apply to future variants. One consideration is that current VOCs were probably selected based on increased fitness and/or transmissibility, while future variants may very well be selected based on escape from immunity when more and more people are vaccinated or have experienced COVD-19. Such escape variants may be more resistant to neutralizing antibodies induced by current vaccines than the VOCs and VOIs studied here and may render vaccines even less effective at preventing infection. Even though this study is to our knowledge the first attempt to quantify the societal impact of Covid-19 restrictions at both the national and global level, several limitations are worth highlighting. First, we were only able to collect survey data in five countries. Even though we found only relatively small differences in the stated utility weights across these somewhat diverse countries, it is possible that larger differences in the subjective valuation of measures would be found in a larger or more diverse sets of countries. Second, while we used census-based sampling weights to create nationally representative samples, it is also possible that respondents may not be fully representative of their respective age, gender, educational attainment stratum. Empirically, the differences across age, gender and educational attainment groups seem relatively small on average, which suggests that minor changes in sample composition will likely only have very small effects on the overall QALY losses estimated. The third limitation of the study is that there are currently no internationally validated questionnaires to estimate QALY utilities for states that are only indirectly health related such as Covid-19 restrictions. Our study has several notable limitations. Our HIT estimates are potentially biased downward because of general awareness within the US of the impacts of COVID-19 in other countries (e.g., China and Italy), which could have resulted in a fraction of the US population changing their behaviors to protect themselves from COVID-19 before the start of the local epidemic. In addition, our estimation of percent progress toward herd immunity crucially depends on seroprevalence estimates of the true disease burden. These estimates are associated with some uncertainty (49-51). As illustrated in Fig 6, percent progress toward herd immunity is underestimated if serological tests fail to detect all cases of infection. The reader must also be cautioned that our analysis depends on a number of assumptions. For example, we considered a compartmental model in which populations are taken to be well-mixed and to lack age structure. This is clearly a simplification. More refined estimates could be obtained by making the model more realistic, but this would have the drawback of increasing the complexity of inference, which at some point would make inference impracticable.

Strengths and limitations: We had complete capture of medicine dispensing for the whole Australian population, and person-level data on a 10% sub-sample. However, we do not have data on dispensing of private prescriptions, meaning we have likely underestimated the impact on use of some medicines. Further, disruptions to medicine use during COVID-19 likely have multiple causes, including lockdown measures, changed interaction with the healthcare system, reduced circulation of respiratory and gastrointestinal infections, and fear of not being able to access medicines, and these cannot reliably be disentangled. Conclusions: We demonstrated temporary changes in dispensing of commonly used medicines that were proposed for re-purposing for the treatment and prevention of COVID-19, including a large short-lived increase in hydroxychloroquine dispensing, most of which appeared to be due to anticipatory stockpiling, and a later smaller but longer-lasting increase in ivermectin dispensing. Balanced and informed communication of the changing evidence, including up-to-date and reliable access to evidence-informed advice is necessary to minimise any negative health impacts related to re-purposing of medicines. When similar situations arise, a quick response by regulators can help limit inappropriate re-purposing, to avoid supply shortages and potential harms.

Our study has certain limitations. For example, social distancing measures meant that data collection had to be conducted remotely and using online technology. This may have affected participation from certain groups who may be less proficient in use of or have less access to digital technology. Relatedly, study promotion and invitations to join were mostly virtual, which meant that HCWs who do not routinely access virtual communication or use social media may have missed taking part. However, this study had a unique advantage of being able to collect data in real-time as the situation around COVID-19 vaccination has been unfolding in the UK, although the rapid change in scenario could also mean that some of the participants’ views may have changed from the time of data collection. Given this, we are making efforts to keep pace with the changes and bring out results rapidly, and some of our early results have already been considered by the government in their policy-making decisions (36). In conclusion, our study is one of the first to report in-depth qualitative data on attitudes of HCWs towards the COVID-19 vaccine and factors influencing vaccination decisions. While vaccine hesitancy among HCWs has been reported by other studies, our research delineates some of the reasons behind this hesitancy, such as trust and communication. One limitation of our study is that a sample obtained by the relative recall method might not be representative of the total number of existing studies [11]. Considering the high level of standardisation of systematic reviews it is possible they all cover much the same territory. An evaluation against a sample derived from a manual review of journals and other sources might provide a more reliable estimate [19]. However, this approach may not be suitable in the context of the deluge of scientific information about COVID-19. Another limitation of our evaluation is the scope of the assessment. We addressed only primary studies and not the other types of scientific articles that are contained in the COVID-19 L·OVE repository, which includes any type of scientific article. Considering the inclusive nature of the methods used to maintain the COVID-19 L·OVE repository, we can expect similar results for the other types of articles, but a formal evaluation would provide a definitive answer. Our study is not designed to assess the specificity of the repository nor any of the components of the COVID-19 classification platform. We believe the latter has enormous potential to increase the reliability of search processes and to reduce the amount of work involved. However, it is yet to be tested. This study has some limitations. Only a few children with severe pCOVID-19 were included in the study, and therefore the results obtained may not apply to rare cases of life-threatening disease in children. We did not have longitudinal samples available for all MIS-C patients. However, the number of patients included in the study was sufficient to detect early and late signatures of the disease. Finally, while an increasing number of cases of pCOVID-19 due to the delta variant have been recently reported [62], almost all samples were collected prior to the emergence of this variant. Therefore, the impact of the delta variant on innate and adaptive immune responses in children with pCOVID-19 and MIS-C remains to be studied. Despite these limitations, this study has helped identify novel age-, time- and treatment-related immunopathological signatures that characterize MIS-C and pCOVID-19.

Our study has several limitations. First, our predictions depend on level of neutralizing antibodies, without taking other immunologic mechanisms of humoral and cellular immunity into account. However, previous studies showed that neutralizing antibodies are highly predictive of immune protection. Second, to enhance comparability between different studies with different neutralization assays, we normalized the neutralization titers in each study by the mean titers in convalescent sera reported in Phase I/II trials against prototype strains that used the same type of neutralization assay. A limitation of our study is that we only considered NAATs and did not consider the diagnostic yield of antigen-based COVID-19 tests, as they were not widespread in Canada in 2020 (16). We were unable to examine the sensitivity and specificity of the SARS-CoV-2 NAATs as we were unable to define false positive tests, so it is possible that some of the positive test results we encountered are false positives, leading to an overestimation of diagnostic yield. While our study is based on a Canadian population without international sites, we believe our findings are generalizable given their wide geographic spread, and the cultural and racial diversity of our patient population. Finally, as data becomes available on the fourth wave of the pandemic, a future study should examine the impact of widespread vaccination on the yield of screening. As a larger proportion of the population is protected from severe disease and death through vaccination, decision makers should carefully consider the low diagnostic yield of a universal testing strategy going forward.

The major limitations of this study include the small sample size and the potential lack of generalizability of these results to all hospital centers. Our group has generated considerable experience in IVIG utilization in COVID-19 and as a result, has identified a ‘sweet spot’ where benefit was seen clinically and economically. However, it remains to be determined if IVIG will benefit patients with greater illness severity such as those in the ICU, on mechanical ventilation, or ECMO. Furthermore, while a significant morbidity and mortality burden of COVID-19 rests in extremely elderly patients and those with end-organ comorbidities such as renal failure and heart failure, it is uncertain if their COVID-19 adverse outcomes can be improved with IVIG or other therapies. We believe such patients may limit the pharmacoeconomic value of IVIG due to their generally poorer prognosis regardless of intervention. On the other hand, COVID-19 patients who are not that severely ill with minimal to no hypoxia generally will do well regardless of therapy and IVIG intervention may be potentially be an unnecessary treatment expense. Limitations: The participants in the present study are part of three, pre-existing, longitudinal cohorts in the Greater San Francisco Bay Area. There are several limitations to the analysis presented within. We here have assumed that individuals under the age of 12 are half as susceptible to infection by SARS-CoV-2 as those above 12 years. Our analysis is subject to important limitations: first, the relative rarity of severe outcomes in younger age groups means that our estimates for these groups are imprecise. Furthermore, our failure to identify heterogeneity in severity by age, when evaluating effects in children and teens, may simply be related to the imprecision of estimates in these age groups, rather than true lack of heterogeneity. A second limitation is the degree to which frequency of testing, particularly in younger age groups, has been dynamic in Ontario during the COVID-19 pandemic (21). Increasing frequency of testing in under-tested younger age groups over time, as VOC emerged in the population, could increase the fraction of mild cases identified, and may have served to mask increases in severity. There are some limitations in this study. First, the participants were mostly collected from East Asia (China) and some from Spain. Therefore, a larger multicenter study covering more patients with diverse races and more control groups is necessary before the model could be used globally. More data are being collected and will be used in the further study. Secondary, there might be potential confounding factors such as comorbidities influencing eye symptoms. We did not collect the comorbidities of our participants. However, the control and positive patients were randomly selected from the population, which could balance the baseline demographics between groups. Third, some of the demographic information (e.g., gender and ages) are not collected during image acquisition. Fourth, our model was based on the eye symptoms, however, it cannot determine COVID-19-related eye disease. The pathological significance of extracted features from COVID-19 patients should be carefully interpreted and re-verified by the ophthalmologist. Further clinical studies are needed to test the performance and provide a deeper understanding of our findings of the ocular surface feature-based classification network.

Our study has several limitations. First, this was an online survey organized during COVID-19 measures, introducing risk for selection bias. Although there is no guideline for conducting online surveys, we used several strategies to limit bias, including the use of online panels, partnerships with organizations for sample recruitment, review of analytics, and prespecified analysis plans.13 Second, although we were able to capture data from different times during the COVID-19 epidemic, this was a series of retrospective cross-sectional studies, and we did not capture how sexual behaviors and access evolved over the course of the pandemic. A follow-up survey in selected countries is now underway. Third, our sample included more women, people with higher education, and people living in high-income countries compared to populations in respective countries. At the same time, data from one of the convenience samples included in this analysis suggested that the convenience sample included similar proportions of adults within subnational geographic areas compared to census data.30 Fourth, our study had fewer studies from low-income countries which may have been due to later COVID-19 initial waves and less capacity for research alongside the pandemic. Our findings are also in line with the approach outlined by NHS England/National Institute of Clinical Excellence (NICE) suggesting that remote consultations are more appropriate for consultations of chronic, stable patients, where a physical examination may not be required.[8] It also provides the beginnings of an evidence base for which hospitals can start adapting to digital health clinics as part of the pre-pandemic NHS Long Term Plan to move up to a third of visits non face-to-face.[20] A number of limitations are worth addressing. This analysis has several limitations to consider. First, BICS uses a quota sample from an online panel instead of a probability sample. However, respondent-level calibration weights were estimated to adjust for sample composition to produce representative surveys for each DMA. Moreover, information bias is a concern for contact surveys due to difficulties with recall and social desirability bias, which may be expected to be especially pronounced in the middle of a pandemic. Respondents may therefore have under-reported their number of contacts in the survey. At the same time, the BICS survey captures only two-way conversational and physical contacts. Other types of contacts that might be relevant to SARS-CoV-2 transmission, including, for example, contacts that happen from being in close proximity to others, were not captured in this analysis. When interpreting the results of our study, the reader should be mindful of several limitations. First, HCPOST had a relatively small sample size, particularly compared to the HCPRE cohort (a limitation largely imposed by the COVID-19 pandemic-related disruption on clinical research)29. However, multiple factors provide high confidence in the solidity of our observations, including the consistency of our observations across individuals, scanners, age groups, genotypes, and sexes (see Appendix, Suppl. Figure 3), the relative stability of the pre-lockdown markers, the observed elevation of two separate imaging markers measured using independent imaging modalities and the finding of significant enrichment in genes highly expressed in glial cells. Second, our analysis was largely based on unpaired comparisons of pre- and post-lockdown groups. While our single subject scanned both pre- and post-lockdown demonstrated PET signal elevations in the same regions observed in the group comparison, longitudinal assessments of more individuals scanned in the same manner will be needed to better understand the impact of the COVID-19 pandemic on brain health. Limitations: Our study had certain limitations. Our study population was large, but not representative since we used a convenience sample. For instance, the proportion of women, people with a high level of education, and workers in healthcare facilities was considerably higher in our study compared to the general population in Greece. Moreover, online surveys during the COVID-19 pandemic are a rational approach but diminish the representativeness of the study population since individuals with limited internet access are less likely to participate. Additionally, the response rate and the profile of non-correspondents cannot be estimated in online surveys. Thus, it would be wise not to generalize our conclusions but to carry out studies with more representative samples. Information bias was possible in our study since the study questionnaire was self-administered and we cannot objectively verify self-reported vaccination. Anonymity in our study may have reduced this information bias. Finally, we have investigated several factors that may affect individuals’ decision to uptake a COVID-19 vaccine, but there may be other factors influencing this decision.
