    1. A 34-year-old man with a history of VHL and NF1 was referred to dermatology for evaluation and management of multiple bleeding papules. The papules were tender, and the bleeding caused his pillowcases to be soaked in blood. He reported many similar papules previously in various anatomical locations. Removal of prior lesions was successful and did not cause satellitosis. New papules would appear gradually over years, and at the time of the visit, 6 papules were bothersome because of intermittent bleeding. The patient was given the diagnosis of NF1 based on clinical findings that were first noted in childhood including multiple café au lait macules, axillary freckling, and Lisch nodules. VHL was suspected after magnetic resonance imaging showed hemangioblastoma of the left cerebellum during evaluation of sensorineural hearing loss. VHL was subsequently confirmed by mutation analysis (P81S mutation). Central nervous system hemangioblastoma is the most common tumor in VHL and was present in the patient. He did not have any additional tumors or cysts in the central nervous system or visceral organs that are common in VHL, which include retinal hemangioblastomas, renal cell carcinoma and cysts, pheochromocytomas, and pancreatic tumors and cysts.5 The patient did not have family history of NF1 or VHL. Medications included dextroamphetamine for attention deficit disorder. On physical examination, the lesions of concern were exophytic, red, vascular papules located on the right jawline, front of the right shoulder, left lower aspect of the back, and right lower aspect of the back, consistent with lobular capillary hemangiomas (Fig 1). These were painful with slight pressure. Several other similar, although smaller, papules were noted on the trunk. On the face, trunk, and extremities there were at least 20 scattered well-defined, ovoid, light-brown macules. Involving the chest, abdomen, and back were multiple discrete 4- to 8-mm subcutaneous rubbery-soft nodules (Fig 2). Axillary freckling was noted on both axilla and the groin. The patient returned to the dermatology clinic 4 months later with new growing and bleeding vascular papules in new locations; they were similar to those removed at the initial visit and consistent with lobular capillary hemangiomas.

    1. 454 C>T transition

Study of a German family of 16 family members with cosegregation of the pheochromocytoma-only phenotype with two concurrent germline mutations in the VHL protein alpha and beta domains. These mutations were c.562 C>T (stated as 775C>G in the paper which used the Latif sequence as reference) and c.241C>T (stated as 454C>T in the paper). Patient 1 was unaffected and had the wild-type allele. His spouse, patient 2, presented with pheochromocytoma and had both mutations. They had four children; one had a pheochromocytoma and died, two (patients 5 and 7) had both mutations and presented with pheochromocytomas, and one child (patient 6) was unaffected and carried neither mutation. Patient 7 had a child (patient 9) who also carried both mutations and presented with pheochromocytoma. In the other branch of the family, patient 3 who carried both mutations and presented with pheochromocytoma had a child with patient 4 who was unaffected. Their child (patient 8) carried both mutations and presented with pheochromocytoma. Patient 8 had two children; one who did not carry the mutation and was unaffected (patient 10) and another that carried both mutations but was disease-free (patient 11). DNA was isolated with whole blood and denaturing high-performance liquid chromatography (DHPLC) and dye terminator method on an ABI Prism 310 genetic analyzer (Applied Biosystems, Weiterstadt, Germany) were used for sequencing. Cosegregation with disease in multiple affected family members in a gene definitively known to cause the disease (ACMG code: PP1).

This paper analyzed VHL sequences in the renal cell carcinomas of patients exposed to high, cumulative doses of trichloroethylene. Whole blood and tumour tissues were isolated and mutations were identified, respectively, by direct sequencing of purified PCR products. For fast analysis of the presence of a frequent mutation at nt 454 (C>T), a restriction endonuclease-based assay (enzyme AciI based assay) was used.Four patients (1654, 1656, 1658, and 1695) were found to have the germline P81S (c. 241C>T) (denoted as nt454 C>T mutation in the paper) VHL mutation in DNA isolated from tumor-adjacent normal kidney parenchyma. All four patients presented with renal cell carcinoma and were diagnosed respectively at ages 59, 70, 66, 38 (1654, 1656, 1658, 1695). Three of the four patients (1654,1658, 1695) had multiple somatic VHL mutations and LOH. Patient 1656 only had the one germline mutation in the VHL gene.

    1. A 50-yr-old male (II.1) presenting with hemangioblastomas of the central nervous system was found to have a hemizygous VHL c.241 C→T (p.Pro81Ser) variant and a concomitant large deletion encompassing all exons of the VHL gene. The screening of his relatives confirmed that the p.Pro81Ser has been inherited from his healthy father (I.1), whereas the large deletion occurred de novo. The proband’s son (III.1) inherited his father’s germline VHL large deletion, and not the VHL p. Pro81Ser variant. In addition, the son (III.1) was found to have a hemizygous VHL c.340 + 5 G→C (IVS1 + 5) polymorphic variant, present on the allele inherited from his healthy mother (II.2). The son (III.1) was, therefore, clinically screened and found to have retinal and cerebellar hemangioblastomas as well as pancreatic cysts, typical lesions of VHL demonstrating that VHL cosegregated with, and is therefore caused by, the VHL large deletion and not the p.Pro81Ser variant. These case reports were, therefore, instrumental in raising two issues. First, could the long-presumed pathogenic RET and VHL mutations actually be harmless polymorphisms? Second, if the RET and VHL variants are pathogenic, then how commonly do patients with heritable PGL harbor two or more pathogenic mutations in the same or different genes?

    1. , VHL, RET, and MAX germline and somatic mutations have been reported in 3–25%, 13–9%, 5–5%, and 1–3% of tumours, respectively [33]. Recently activating mutations in HIF2A and HRAS were reported in a subset of these tumours [6, 7, 19, 20]. Whilst HIF2A mutations may be found in multiple tumours from patients without a detectable germline mutation (suggesting low level constitutional mosaicism) or occasionally as a germline mutation [5], to date HRAS mutations have only been detected as somatic changes (germline HRAS mutations are associated with Costello syndrome but PCC/PGL/HNPGL are apparently not a feature of this disorder) [34].
    1. c.241C > T

All patients included in the study were VHL mutation carriers of 18 years or older examined between 1972 and 2012 in the VHL expertise centers University Medical Center Groningen (UMCG) and University Medical Center Utrecht (UMCU). Clinical data of 75 VHL mutation carriers were retrospectively retrieved from electronic and written patient record. Of these patients, one was found to harbour the c.241C>T germline mutation of the VHL gene. They presented with a retinal hemangioblastoma, cerebellar hemangioblastoma, spinal hemangioblastoma, renal cell carcinoma, pancreatic neuroendocrine tumor, renal cyst and pheochromocytoma. Their age at the last follow-up was 59. Previously published in: PMID 24132471.

3 unrelated patients with unexpected or non-syndromic phenotypes, accompanied by a VHL component tumor received germline gene panel testing as part of the PROACTIVE study at the Dana-Farber Cancer Institute, each detected with a low penetrant variant. At age 32, patient 3 (female) presented with posterior headaches, which a CT scan and MRI revealed to be a cerebellar hemangioblastoma. Treatment involved a gross total resection of the lesion. Somatic testing of a left cerebellar mass revealed a somatic VHL variant: p.Leu63fs*4 (c.188_188T>TG); updated to p.R64fs (c.189dupG). Single gene testing of VHL revealed a heterozygous germline VHL p.Pro81Ser (c.241C>T) variant. To this day at age 40, she continues to be monitored. Her mother (age 72) and two of her three siblings (ages 38 and 36) carry the same VHL p.Pro81Ser variant, but have been negative for hemangioblastoma or any other VHL component tumor. ACMG: PP4

Their phenotypes were grouped together: retinal hemangioblastoma, cerebellar hemangioblastoma, spinal cord hemangioblastoma, renal cell carcinoma, pancreatic neuroendocrine tumour, pheochromocytoma, and renal cysts. Family history was not described. The group was classified as a 2a/b which was described by the author to included de novo VHL mutation carriers or VHL mutation carriers having a family history with diagnosis of pheochromocytoma and other VHL related manifestations.
