    1. The selected tissue blocks were cut in 3-μm sections and were immunolabeled with antibodies directed against chromogranine A (Dako, clone DAK-A3), HIF-1α (BD Biosciences, clone 54), HIF-2α (Abcam, clone ep190b), cyclin D1 (Dako, clone DCF-6), and CA9 (Novus Biological, polyclonal). Non-neoplastic pancreatic tissue was also immunolabeled with antibodies against insulin (Dako, polyclonal) and glucagon (Sigma, clone K79bB10) to differentiate endocrine microadenomas from normal islets. In addition, 5 different blocks with microadenomas obtained from 5 patients who presented at least 5 microadenomas were selected for an additional immunostaining with VEGF (Santa-Cruz Biotechnologies, polyclonal) and CD34 (Immunotech, QBEND10) for evaluation of microvessel surface.
    2. Eighteen VHL patients who underwent pancreatectomy for one or several endocrine tumors at Beaujon hospital from 1997 to 2008 were retrieved from the file of VHL patients (Table 1).

FamilyInfo: Family’s roots were traced to the German town of Leipzig in Saxony, from whence the original 11 siblings had immigrated to America around 1670. The following report is a 2011 updated pedigree on the previously reported Tisherman kindred. At the time of this study, 43/49 patients have undergone mutational analysis revealing the VHL familial mutation (Y112H) and 35/49 patients have clinical VHL manifestations. Phenotypic cumulative lifetime risk was 71% for Pheo/PGL, 15% for hemangioblastoma, 33% for retinal angioma, 3% for renal cell carcinoma, and 3% for pancreatic cysts. 6 members of the family (all deceased; 4 males, 2 females) had suspected VHL and 2 (males) of these were suspected to have had Pheo/PGL.

    1. MSY112H1 (5.6)1 (3.8)

26 VHL patients from 18 families, who had undergone a partial adrenalectomy for pheochromocytoma were retrospectively analyzed. 1 patient from a single family was confirmed to have the mutation (Y112H). Additional manifestations aside from pheochromocytoma are possible, but were not discussed. ACMG evidence codes: 'PP2' because they observe a missense variant in a gene where missense variants are a common mechanism of disease.

    1. An update on the genetic findings pertaining to a previously published (pmids: 8239848, 21713522) large familial pheochromocytoma Pennsylvania kindred. The family is comprised of: 18 living individuals with only pheochromocytoma; 7 living individuals with only retinal angioma; 6 living individuals with both; 51 other first-degree relatives; 5 additional relatives with pheochromocytomas, including a 69Y woman and her 42Y son described previously as non-penetrant individuals (pmid 8239848). Family classified as a VHL Type 2A (pheochromocytoma, retinal angioma, CNS hemangioblastoma, and little to no RCC). All 31 individuals (including 6 archival specimens) with a diagnosis of pheochromocytoma, retinal angioma or both had the identical T547C (legacy nomenclature) germline mutation determined through PCR, single-strand conformational polymorphism analysis and direct sequencing.

  2. Oct 2026
    1. Germline mutations were identified in 300/469 (63%) of the families tested; 137 distinct intragenic germline mutations were detected.

Germline mutational data on VHL was gathered from various countries to assess variation in germline mutations that produce disease within different populations. Families were grouped according to three possible outcomes of disease: renal cell carcinoma without pheochromocytoma, renal cell carcinoma with pheochromocytoma, or pheochromocytoma alone. SSCP and denaturing gradient gel electrophoresis identified mutations in 300/469 (63%) of families affected with VHL. This germline familial VHL mutation (Y112H (c.334T>C)) was taken from the National Cancer Institute database, with an unknown number of members belonging to two family IDs: Family ID 3738NCI and 4874NCI. It was reported in the paper as c.547T>C due to a difference in transcript reference. As reported in Table 4, this mutation is found in VHL families with pheochromocytoma. Other phenotypes are likely, including renal cell carcinoma, however not detailed here. Later publications reference this cohort as being reported in PMIDs: 7728151 and 8863170.

    1. In a study of 114 unrelated VHL families, 85 germline mutations were found. VHL mutations were detected in affected family members, but not in unaffected family members or 96 normal individuals. This missense mutation was found in 3 VHL type 1 family members (family ID 3738), using PCR and SSCP sequencing. The mutation of c.334T>C was referred to in the paper by c.547T>C due to a difference in transcript sequence. All had pheochromocytomas and one with retinal hemangioblastomas.

    1. 21 missense mutations, 5 nonsense mutations

Among patients prospectively followed by the members of the French VHL Study Group, 35 consecutive patients with well-documented endocrine pancreatic tumour (20 women and 15 men; median age,37 [21Y-57] years) were included. The data from 7 patients with EPT have been partially reported previously in Hammel et al., 1995;2000. All 35 patients had VHL germline mutations and were assessed for the presence of pheochromocytoma and renal carcinoma in addition to pancreatic tumours. The presence of other phenotypes was not described. A missense mutation (c.334T>C) was found in 2 members of the same family. Patient No. 10 and patient no. 17 from family 360 both presented with pancreatic endocrine tumour. Their familial relation and other family members were not described therefore there may not be sufficient evidence for PP1. However, patients' phenotypes are highly specific to VHL disease (PP4) and disease-causing missense variants are common in VHL disease (PP2).

Of 246 patients affected with von Hippel-Lindau disease, 64 patients (26%) from 38 families had pheochromocytoma, including 33 newly diagnosed during screening studies and 31 treated before screening at NIH. Germline mutations in the VHL gene was determined from peripheral blood. Family 6101 was found with p.Tyr112His mutation in the VHL gene (legacy notation c.547T>C converted to NM_000551.3): one patient was in this family with pheochromocytoma, and retinal angioma.
