    2. PatientsThe Chinese family study included five-generations comprising of 38 living family members, of whom nine were affected [Figure 1]. All living members were examined clinically. The clinical and radiographic images were published with the consent of the patient. The study was approved by the research Ethics Committees of Xi’an Jiaotong University School of Medicine. Written informed consents were obtained from all participants.Open in a separate windowFigure 1Haplotypes of the Chinese family affected by Von Hippel-Lindau (VHL). The open and filled blackened symbols indicated unaffected and affected individuals, respectively. The squares and circles symbolized the males and females, respectively. The proband, IV-3, was indicated by an arrow. The haplotyping was conducted using three polymorphic microsatellite markers. The marker order was determined from the Marshfield map and the UCSC Human Genome database (February 2009). The dark boxes symbolized haplotypes that cosegregated with the affected individuals and suggested linkage of VHL to disease in this family. The boxes with slash symbolized the dead family members.

    1. VHL Variants

This report studied the prevalence of germline variants in Japanese PPGL patients. Of the 370 PPGL original Japanese probands, this cohort consisted of 15 patients with pheochromocytoma and/or paragangliomas with germline VHL varaints. Blood samples were used to examine 7 genes: MAX, SDHB, SDHC, SDHD, TMEM127, VHL, and RET, using Sanger sequencing. Of these 15 VHL patients: 10 had bilateral PCC, 4 had PCC with abdominal/thoracic PGL, and one had metastatic PPGL. The variant R167W (c.499C>T) was one of the 15 found in a patient with PPGL. Their family history or further manifestations were not detailed.

    1. The VHL mutation pattern includes point mutations, small deletions and large deletions.

This paper analyzed the VHL gene of 21 patients who had clinical a clinical diagnosis of VHL, familial pheochromocytoma, or sporadic pheochromocytoma. 7 patients were from 3 families, while the remaining 14 were unrelated. Mutations were detected by realtime quantitative PCR (RQ-PCR) assays using peripheral venous blood. The mutation (c.499C>T) was found in 1 individual (33Y, M), without family history, who presented with pheochromocytoma, hypertension and diabetes mellitus. This case was also later presented in PMID: 24735130 (Pai et al, 2014). ACMG evidence codes: 'PP2' because they observe a missense variant in a gene where missense variants are a common mechanism of disease.

    1. In a study of 114 unrelated VHL families, 85 germline mutations were found. VHL mutations were detected in affected family members, but not in unaffected family members or 96 normal individuals. This missense mutation was found in 4 VHL type 2 families of 51 individuals altogether (family IDs 2338, 3490, 4477, 4405). 36 of 51 individuals had retinal hemangioblastomas, 17 had CNS hemangioblastomas, 16 with renal cell carcinoma, and 36 with pheochromocytomas. This mutation was also detected in an additional family with 1 single member, with retinal angiomas (family ID 3699). Supporting evidence of pathogenicity because cosegregation with disease in multiple affected family members in a gene definitively known to cause the disease (ACMG code: PP1).

    1. recognized in a large family that originated in Newfoundland but has some members who live in New Brunswick and Ontario. A collaborative investigation was begun in 1982 to document the number of affected members and the extent of their disease and to improve management of the disease. The condition has been documented in 38 members of the family, 28 living and 10 dead. The most common manifestations are retinal angioma (present in 60% of the gene carriers) and pheochromocytoma (present in 53%). Of the 28 living affected members 14 had been identified before the study began. Only 3 of the 14 patients in whom the disease was subsequently diagnosed presented with symptoms; in the remaining 11 the condition was detected by routine screening. Overall the mean age at the time of diagnosis was 23 years; in the 21 affected members of the fourth generation it was 18 years.

FamilyInfo: 38 family members had been affected at the date of 1986, and 10 had passed away. Pheochromocytoma, cerebellar, spinal and retinal hemangioblastomas, and various cysts were clinical manifestations of VHL seen throughout the family. This patient (IV-34) appears to have inherited this disease from her mother, along with many of her siblings. See Fig. 1 for family pedigree.

    2. recognized in a large family that originated in Newfoundland but has some members who live in New Brunswick and Ontario.

Screening of 3 patients affected with familial PC for mutations in the RET proto-oncogene (mutations related to MEN 2 syndrome) and the VHL tumor suppressor gene (mutations related to VHL disease) with the use of PCR, SSCP, heteroduplex analysis, and direct sequencing, revealed missense VHL gene mutations. Please note that there was no clinical diagnosis of VHL disease. This missense mutation c.499C>T was found in a Polish family with 3 siblings (two male siblings and their mother) with no evidence of RET; the older brother presented with unilateral pheochromocytoma at 15Y, the younger brother presented with bilateral pheochromocytoma at 11Y, and the mother presented with bilateral pheochromocytoma at 41Y. Additional phenotypic information was not provided. Please note that the paper states the mutation as c.712C>T because it uses a legacy reference sequence.

    1. 8702C->T substitution was detected in patient II/3 (Figure 2) resulting in the R167W codon change

The proband (II/3), of italian descent, was found to have the c.499C>T change (referred to in the paper by 8702C->T due to a difference in transcript sequence) causing R167W of the VHL gene. This variant was detected using direct sequencing of PCR products obtained from genomic DNA extracted from the patients peripheral blood leukocytes using the DNA extractor BIO ROBOT EZ 1 (Qiagen). The c.499C>T change was detected in the exon 3 of VHL gene. He had retina angioma with renal carcinoma. His daughter (III/3) also had the mutation and was symptomatic but specific phenotypes were not specified. His son (III/4) was asymptomatic, but it is unclear if he was tested or not for the genetic variant. His father (I/1), had the mutation and presented with a haemangioblastoma. His brother (II/2), also had the mutation and presented with renal carcinoma, central nervous system haemangioblastoma, pheochromacytoma and pancreatic cysts. ACMG evidence codes: 'PP1' since we have cosegregation with the disease in multiple affected family members, 'PP4' since the patient presents phenotypes highly specific for a disease with a single genetic etiology.

A case report of a woman presenting with bilateral adrenal pheochromocytoma and  multiple pancreatic massess (paraganglioma) at 18 years of age is presented. Molecular analysis on peripheral leukocytes confirmed the presence of the above mutation (c.499C>T). Additional clinical manifestations in this patient include retinal and cerebellar hemangioblastomas (at 19Y and 22Y), and a neuroendocrine tumour (at 24Y). Family history shows 3 additional family members who likely have VHL, but have not been genetically tested. Clinical manifestations in these individuals included: pheochromocytoma, pancreatic cysts, and paraganglioma. ACMG evidence codes: 'PP2' because they observe a missense variant in a gene where missense variants are a common mechanism of disease, 'PP4' because the patients' phenotype or family history are highly specific for a disease with single genetic etiology.

Through PCR purification and Sanger sequencing of peripheral blood (using an alcohol precipitation method), 121 consecutive, unrelated, index PCC/PGL patients underwent genetic testing for five PCC/PGL susceptibility genes (RET, VHL, SDHB, SDHD and SDHC) and were evaluated for clinical diagnosis of neurofibromatosis type1. Three unrelated probands were genetically confirmed with a familial mutation at c.499C>T of the VHL gene. The first was an 11 year old male who presented with bilateral pheochromocytoma and retinal capillary hemangioma. The second was an 11 year old male who presented with bilateral pheochromocytoma and PNETs. The third was an 18 year old female who presented with bilateral pheochromocytoma and sympathetic paraganglioma, CNS hemangioblastoma and PNETs. Family histories are unknown but positive.

216 patients with clinically expected VHL disease due to family history or presence of VHL typical tumours were routinely examined in an eye centre in Germany between January 2019 and January 2020, majority of which were of European descent (~95%). Since data on the genotype–phenotype correlation of the onset of Retinal Hemangioblastoma (RH) is scarce in relation to VHL disease, this study wanted to inversitage the relationship between VHL gene variants (truncating vs single amino acid substitutions) and the ocular phenotype of the disease. 42 different rare VHL gene variants in 148 patients were detected - some of whom had RH and some who did not. Although date of onset of RH, as well as data on central nervous system haemangioblastomas (CNSH), pheochromocytoma/paraganglioma (PPGL), clear cell renalcell carcinoma (ccRCC) and pancreatic neuroendocrine neoplasm (PNEN) were collected, it was not reported on a per patient basis and therefore could not be discussed per variant. There was 1 patient reported with c.499C>T (p.R167W). This member presented with retinal hemangiomas. No family information was provided.

    2. Molecular and clinical analysis of 126 French VHL families tested for renal involvement revealed 92 different mutations. 90 of these families had renal involvement. The p.R167W (c.499C>T) germline variant of the VHL gene was detected in multiple families. Family 1: Members 296, 419, 295 and 414 had no renal involvement. Members 427, 318, 403 and 406 had unknown renal involvement. Members 402, 322, 403 and 1 had mild renal disease (RCC). Members 401 and 404 had moderate renal disease (RCC). Family 23: Member 1037 had no renal involvement. Members 1035 and 1036 had moderate renal disease (RCC). Family 50: Member 6338 had no renal involvement. Members 6340 and 6339 had mild renal disease. The paper states that these families are previously reported in PMID 12202531 and/or 9829912.

    1. Medical records of 26 patients with germline mutations of the VHL gene who had been diagnosed with VHL disease in Severance Hospital (Seoul, Republic of Korea) and Samsung Medical Center (Seoul, Republic of Korea) between 2003 and 2012 were respectively reviewed. Three unrelated probands were genetically confirmed with a germline missense mutation at c.499C>T of the VHL gene. Patients 42 and 44, 10Y and 30Y males respectively, both presented with multiple and/or bilateral pheochromocytoma and CNS hemangioblastomas. Patient 43, 30Y male, presented with multiple and/or bilateral pheochromocytoma and retinal hemangioblastoma. Patients 42 and 43 had a family history negative for VHL disease while patient 44 had a positive family history of VHL disease. Patient 43s phenotypes were revised from a previous report (PMID 19949673). Patient 44 was previously reported in PMID 16142346 as well as Kim et al., 2005 (reference 15 in the current paper). All of these patients were diagnosed with VHL Type 2A.

    1. c.499C>T p.Arg167Trp

During the period from 2009 to 2012, 39 families with VHL disease were diagnosed with hereditary VHL disease at Peking University First Hospital Department of Urology were described as part of a broader study examining telomere length and anticipation. In these families, 19 families had two or more than two patients in two or more than two generations, including one family with obscure onset age. Therefore, a total of 57 patients with VHL disease from 18 families were enrolled in this study. Using peripheral blood and southern blotting, DNA sequencing was completed on the following patients. Family 5 involved four members with a c.499C>T change leading to a p.Arg167Trp protein change. The first member, the father, presented with renal cell carcinoma at 63. His 3 daughters also presented. The first daughter presented with hemangioblastoma, renal cell carcinoma, ovarian cyst, and pancreatic cysts at 37, as well as pheochromocytoma at 39. The second daughter presented with pancreatic cysts at 36 and hemangioblastoma at 38. The third presented with retinal capillary hemangioma at 34. The first born son did not present with any symptoms nor the mutation. ACMG Evidence Code: 'PP4' because the patients' phenotypes and family histories are highly specific for VHL disease.

    2. Relative telomere length assessment Genomic DNA was isolated from peripheral blood by using a blood DNA extraction kit (Tiangene). We followed the method described by Cawthon to quantify relative telomere length by measuring copy number ratio of telomere repeats (T) to the single copy gene 36B4 (S) using qRT-PCR (24). The 10-μL PCR mixture contained 2X SYBR master mix (Takara) 5 μL, genomic DNA 30 ng, 300 nmol/L telomere primer Tel1 (5′-GGTTTTTGAGGGTGAGGGTGAGGGTGAGGGTGAGGGT) and 900 nmol/L Tel2 (5′-TCCCGACTATCCCTATCCCTATCCCTATCCCTATCCCTA), or 200 nmol/L single copy gene primer 36B4u (5′-CAGCAAGTGGGAAGGTGTAATCC), and 500 nmol/L 36B4d (5′-CCCATTCTATCATCAACGGGTACAA; ref. 19). qRT-PCR was run in an ABI 7500 PCR instrument using the profile of 95°C for 30 seconds and 40 cycles of 95°C for 15 seconds, 54°C for 2 minutes, and 72°C for 15 seconds. A standard curve from a control DNA sample (male, 45 years old) by serial 1/4 dilutions from 50 ng to 0.19 ng was constructed to evaluate the amplification efficiency (E), and this sample was measured in every batch of PCRs as the inter-run calibration. Threshold cycle (Ct) values were automatically determined by the 7500 software v2.0.5. The measurements of telomere length and single copy gene 36B4 were triplicate in one batch for each sample, and the mean of the three Ct values (Cm) was used for the calculation. PCR efficiency and the calibration of copy numbers were also enrolled in the telomere length calculation (19, 25). The formula to calculate the copy number ratio of telomere repeats (T) to single copy gene 36B4 (S) is as follows, and T/S represents the value of relative telomere length. <img class="highwire-embed" alt="Embedded Image" src="https://cancerres-aacrjournals-org.uhn.idm.oclc.org/sites/default/files/highwire/canres/74/14/3802/embed/graphic-1.gif"/> The relationship between age and relative telomere length in 325 normal controls can be expressed by the linear regression equation of Y = 1.4503-0.0117*X (see Fig. 2). Normal relative telomere length at the DNA sample-obtained age can then be predicted by this equation. The difference between predicted normal relative telomere length at the DNA-obtained age and the relative telomere length actually measured was the age-adjusted relative telomere length (19), which was used for the comparison among patients with VHL in generations. Telomere length measurement by Southern blot analysis The oligonucleotide (TTAGGG)4 was tailed with DIG-dUTP as the probe by using terminal transferase. Two micrograms of genomic DNA were digested with HinfI/RsaI and separated in 0.8% agarose gel. The DNA fragments in the gel were then transferred onto a Hybond-N membrane by Southern blotting. After UV cross-link and prehybridization, the membrane was hybridized in a solution containing 2 pmol/mL probe, 50% formamide, 2× SSC, 0.1% lauroyl sarcosine, 0.02% SDS, and 1% milk powder at 42°C for 6 hours, washed at room temperature in 2× SSC, 0.1% SDS for two times, and in 0.1× SSC, 0.1% SDS, 15 minutes for two times. Hybridized probe on the membrane was recognized by alkaline phosphatase conjugated anti-DIG antibody and chemiluminescent method. The luminescent image was developed on a phosphoimager. The telomere length was calculated by a telomeric software (version 1.2; ref. 26).
    1. Case studies of three families affected by VHL and their children were described. The legacy mutation C712T in VHL was found in three members of family 1. The father was 48Y when he was found to have renal cell carcinoma and bilateral pheochromocytoma after initially presenting with loin pain, pyrexia and haematuria. He died one year after diagnosis despite chemotherapy treatments. His son was diagnosed with and treated for bilateral pheochromocytoma at 13Y and endolymphatic sac tumors at 19Y after noting tinnitus, increasing hearing difficulties in his left ear since the age of 16. The son had a sister who tested positive for the mutation and appears to be asymptomatic. It is unclear whether the mother and the second son were negative for the mutation or were not tested.

Patients with ELSTs were identified and included in the International Endolymphatic Sac Tumor Registry. The registry contained 93 patients as of July 2014, and of those, 25 were sporadic, and 68 associated with VHL. Patients with ELSTs and clinically confirmed diagnoses of VHL were included in this study. Statistics of this VHL subgroup are as follows: mean age 30, 62% female, 91% symptomatic, 54% with retinal angioma, 72% with CNS hemangioblastoma, 46% with renal cell carcinoma, 19% with pheochromocytoma, and 12% with pancreatic neuroendocrine tumour. In the data from this study, only endolymphatic sac tumours can be confirmed as a phenotype for this mutation (c.499C>T), which was found in 3 individuals each from France, UK, or Germany.

    1. R167W

A 25 year-old female patient presented with a 2-year history of progressive intermittent headaches, hot flushes, diaphoresis and hypertension. The patient is a non-smoker, non-drinker, with no comorbidities, no recreational drug use or any family history of endocrinopathies, pheochromocytomas or familial syndromes. She denied current or past symptoms of episodic or sustained tachycardia, palpations, dyspnoea, abdominal pain, fatigue or weight loss. A renal Doppler ultrasound ruled out renal artery stenosis, but located a unilateral left adrenal massAn MRI with gadolinium contrast revealed left adrenal mass and detected a smaller right-side adrenal mass and neuroendochrine tumors (confirmed by testing for chromogranin A). The largest pancreatic mass was medial to the uncinate process (3.7×3.0×5.6 cm) and was solid with areas of necrosis while the other pancreatic masses were significantly smaller, cystic, well defined and uniform in character and were in the body and tail of the pancreas. An MIBG whole-body scan confirmed bilateral functional phaeochromocytomas. The patient was suspected for VHL disease which was confirmed by genetic testing that found the R167W (missense) mutation of the VHL gene which had been previously reported in a kindred from Newfoundland. Genes CDC73, CDKN1B, MAX, MEN1, PRKAR1A, PTEN, RET, SDHA, SDHAF2, SDHB, SDHC, SDHD, TMEM127 and TP53 were found not to be pathogenic. Further imaging found multiple lumbar and thoracic hemangioblastomas.

74 patients (38 males, 36 females; 24 with prior sequencing data) with pheochromoctyoma were first analyzed by denaturing high performance liquid chromatography (dHPLC). PCR of all exons of VHL was performed on leukocyte DNA extracted from stored blood samples, and those showing variance were sequenced. In addition, 35 positive controls were used to confirm this method. This patient was a positive control who has been previously sequenced (not referenced). One patient was found with the R167W (c.499C>T) VHL mutation. No family history was mentioned or clearly stated of their phenotypes.

    1. p.R167Q/W

Pathogenic variants from 10,389 individuals across 33 cancer types were examined from The Cancer Genome Atlas (TCGA) cohort. 12 of these individuals had a germline mutation in the VHL gene. The R167W (c.499C>T) missense VHL mutation was found in two individuals, a white 67Y0M female diagnosed with clear cell renal cell carcinoma, as well as a white 19Y0M female diagnosed with pheochromocytoma. No family information was provided.

    1. (R167W)

A 64 year old female was found to have a germline VHL missense mutation (Arg167Trp). The paper does not provide any information about nucleotide change. The patient presented with a large suprasellar hemangioblastoma, and had a documented past history of severe VHL disease, with bilateral ccRCC, bilateral pheochromocytomas, pancreatic cysts, retinal hemangioblastomas, and multiple CNS hemangioblastomas. No information about patient family history was provided. Neurological exam was significant for Romberg sign and mild dysmetria of bilateral upper extremities. A visual field test of the left eye revealed new onset inferior temporal quadrantanopsia. MRI confirmed a suprasellar hemangioblastoma within the circle of Willis with increased optic tract edema and compression of the optic chiasm. Left eye extraocular movements and pupillary reaction intact. The patient was treated with octoreotide for 9 months, leading to resultant clinical stability and decreased tumor volume.

    1. c.C712T, p.R167W

Three patients with mutations in the VHL gene were diagnosed with pancreatic neuroendocrine tumours before the age of 18. Case 2 was a young female who developed bilateral phaeochromocytomas at 12 and underwent staged bilateral adrenalectomies at 14. At the age of 16, she was discovered to have a 2.5 cm tail of pancreas pancreatic neuroendocrine tumour. This enlarged during follow-up and additional smaller pancreatic neuroendocrine tumours in the pancreatic body became visible. A second 1 mm grade 1 pNET was also present and was completely excised. In the 7 years of follow-up since this operation, new pNETs developed within the pancreatic head, body and tail and have continued to enlarge. During a period of follow-up, she was found to have a small asymptomatic spinal haemangioblastoma. Genetic testing was done when the patient was 11-years-old and confirmed an inherited mutation in the VHL gene. She was found to have inherited a mutation in exon 3 of the VHL gene (c.499C>T, p.R167W). This missense mutation is noted in the paper as c.712C>T due to a difference in the VHL reference sequence notation used. The proband's mother died at the age of 45 from a progressive metastatic pancreatic neuroendocrine tumour diagnosed at the age of 35. The mother also had a pheochromocytoma, hemangioblastoma, and renal cell carcinoma. The proband's uncle is also described as having VHL disease and presented with pheochromocytoma, hemangioblastoma, renal cell carcinoma. The proband's maternal cousin, 'Case 3' in the paper, was a 3-year-old male who was diagnosed with VHL disease after cascade genetic screening and was found to have the same mutation as 'Case 2' in the VHL gene (c.499C>T, p.R167W). At the age of 11, he underwent resection of an abdominal paraganglioma, followed by staged bilateral adrenalectomies for phaeochromocytomas at the age of 14. Post-operative surveillance imaging detected a 1 cm body of pancreas pancreatic neuroendocrine tumour at the age of 12. Case 3's brother was a 26 year old male with a pheochromocytoma and hemangioblastoma.

    1. nt 712 C to T, Arg to Trp

Fourty-four of 389 patients with VHL (between 1988-1999) screened positive for pancreatic neuroendocrine tumours (PNETs) either by pathologic analysis of tissue specimens or by characteristic radiographic appearance on CT and MRI. VHL gene germline mutations were determined by quantitative Southern blotting to detect deletions of the gene and gene rearrangements as well as complete gene sequencing. Eight patients presented with the c.499C>T VHL gene mutation (stated as 712 C to T in the paper which used the Latif sequence as reference) causing a change of Arg to Trp at protein position 167. From family 3407, patients 30 (50-year-old male) and 31 (33-year-old male) both presented with a single radiologic PNET. From family 2301, patient 32 (34-year-old female) presented with two pathologic PNETs with metastases, and patient 33 (20-year-old female) presented with a single pathologic PNET with metastases. Unrelated patients 34 (43-year-old female), patient 35 (20-year-old female), and patient 36 (27-year-old female), all presented with three pathologic PNETs. Unrelated patient 37 (46-year-old female) presented with a single pathologic PNET. ACMG evidence codes: 'PP1' since we have co-segregation with the disease in multiple affected family members (family 3407, 5069, and 2301), 'PP4' since the patients' present phenotypes highly specific for a disease with a single genetic etiology.

    1. A 64-year-old female with a germline VHL missense mutation (R167W), a known large suprasellar hemangioblastoma, and history of right eye enucleation presented with worsening vision of her remaining left eye. The patient had a past history of severe VHL disease with bilateral clear cell renal cell carcinomas, bilateral pheochromocytomas, pancreatic cysts, retinal HBs, and multiple CNS HBs. Neurological exam was significant for Romberg sign and mild dysmetria of bilateral upper extremities. Left eye extraocular movements and pupillary reaction intact. A visual field test of the left eye revealed new onset inferior temporal quadrantanopsia. Magnetic resonance imaging confirmed a slowly growing (3.45 cm3) suprasellar hemangioblastoma within the circle of Willis with increased optic tract edema and compression of the optic chiasm (Fig. 3A).Open in a separate windowFigure 3Symptomatic, surgically unresectable VHL-HB treated with octreotide.(A) T1 magnetic resonance image with contrast of 64 year-old female patient with germline VHL missense mutation presents with a 3.45 cm3 suprasellar HB. (B) 68GA-DOTATATE PET/CT imaging of suprasellar HB showing avid 68GA-DOTATATE uptake. (C) 68Ga-DOTATATE-PET imaging at presentation and 6 months after octreotide treatment (D). The pituitary stalk HB exhibits diminished uptake of 12.0 SUV compared to 22.6 SUV in initial study. Central photopenia also noted, indicative of necrotic process. (E) Tumor volume was measured using Osirix® software on FLAIR images. Seven images were analyzed, with the first image acquired just prior to initiation of Octreotide treatment. Measurements in cm3. (F) Visual field analysis of patient’s left visual field prior to starting octreotide treatment (left) and 6 months after beginning treatment. The newly symptomatic suprasellar mass was deemed inoperable due to the location (suprasellar, within the circle of Willis), her extensive co-morbidities, and the risk of loss of vision in her remaining eye with an attempted surgical resection. Whole body 68Ga-DOTATATE-PET revealed that the suprasellar mass was avid for DOTATATE (maximum standard uptake value (SUVMax) = 22.6) (Fig. 3B and C). As such, octreotide was initiated off label with a trial dosing of 20 μg daily subcutaneously for two weeks. Following this, she was prescribed long-acting Octreotide-LAR 30 mg by intramuscular injection, once a month, for 9 months. On Octreotide-LAR, she reported diarrhea managed with increased pancrealipase dosing and an increased insulin requirement. Follow up 68Ga-DOTATATE-PET imaging was done 6 months after primary scans and more than one month after her most recent octreotide dose. These images revealed maximum SUVMax of 12.0, compared to the initial SUVMax of 22.6 prior to starting treatment. Moreover, the 6 month follow up scan revealed central tumor photopenia, indicative of necrosis (Fig. 3D). As a reference, the pituitary uptake initially measured 24.9 SUV and 6 months after treatment measured 50.3 SUV. Serial FLAIR MRI images were acquired periodically throughout octreotide treatment and revealed a steady decline in the volume of the suprasellar mass from 3.45 cm3 to 2.54 cm3 with 9 months of Octreotide-LAR therapy (Fig. 3E). Furthermore, visual field analysis remained stable with continuous therapy (Fig. 3F).

    1. missense mutation at codon 167 (CGG to TGG, heterozygous)

A 40-year-old woman was found with clear cell renal cell carcinoma, pancreatic endocrine tumours, and pancreatic-islet cell hyperplasia. Genetic analysis using peripheral blood revealed a germline mutation c.499C>T (p.Arg167Trp) in the VHL gene. The patient had negative family history for VHL disease. 46 months later the patient had no evidence of recurrence.

    2. A 40-year-old woman was admitted to our hospital for further evaluation of a left RCC that had been found during a routine checkup. Computed tomography of the abdomen showed multiple, small tumors (≤10 mm in diameter) in the head of the pancreas as well as a left renal mass, measuring 8×7×6 cm in size (Fig. 1a). Selective abdominal angiography (via the celiac and supramesenteric arteries) disclosed numerous small tumor stains in the pancreatic head (Fig. 1b). Multiple pancreatic metastases from the left RCC were first suspected. Pancreatic microadenomatosis of MEN1 syndrome was also suggested, but no other lesions related to this syndrome (such as pituitary adenoma, parathyroid adenoma, or adrenocortical tumor) were observed. This patient had no clinical symptoms of inappropriate pancreatic hormone secretions such as insulin, glucagon, and gastrin.

    1. recognized in a large family that originated in Newfoundland but has some members who live in New Brunswick and Ontario.

    2. R167W germline heterozygous missense mutation was identified in exon 3

A retrospective study of 14 patients (6 female and 8 male; mean age, 43.5 yr) with no family history and no other clinical manifestations of VHL disease and were operated for symptomatic CNS hemangioblastoma between 1993-2002 at the Casa Sollievo della Sofferenza Hospital in Italy was conducted. Peripheral blood was used through PCR and dHPLC for molecular genetic analysis. Germline mutations of the VHL gene were identified in 2 of the 14 patients. Patient #9 (45Y female) had a R167W germline mutation in the VHL gene and cerebellar hemangioblastoma. At VHL gene analysis of close family members revealed a R167W mutation in her sister, who did not have any clinical VHL symptoms at the time of the study.

    1. Peripheral blood from unrelated patients with pheochromocytoma was tested for mutations of proto-oncogene RET, tumor suppressor gene VHL, succinate dehydrogenase subunit D (SDHD) gene, and the succinate dehydrogenase subunit B (SDHB) gene. Of 271 patients with nonsydromic pheochromocytoma and no family history of the disease, 30 mutations were found in VHL. Multiple pheochromocytomas were statistically more frequent among patients with mutations than among patients without mutations (21 of 66 vs. 5 of 205, P<0.001). The mutation R167W (c.499C>T) was detected in the patient, referred to in the paper by c.712C>T due to a difference in transcript sequence. This missense mutation was found in the VHL gene and six patients with pheochromocytoma. Any relation is unstated. This mutation was said to be spontaneous.

    1. c.499C>Tp.R167W

Medical records of patients with PPGLs between 2005 and 2016 were reviewed. Diagnosis was confirmed histologically and stratified into sporadic or familial/syndromic. Patients underwent next‐generation whole-exome sequencing. A 17-year-old patient presented with unifocal (adrenal) pheochromocytoma. Genetic testing confirmed a familial missense mutation at c.499C>T of the VHL gene.

    1. Germline mutations were identified in 300/469 (63%) of the families tested; 137 distinct intragenic germline mutations were detected.

    1. missense mutations

In a study of 138 unrelated VHL families, 109 germline mutations were found. Missense mutations were more frequently found in families with pheochromocytoma (PC). Deletions, nonsense, and frameshift mutations were more frequently found in families without PC. Large deletions and mutations predicted to cause truncated protein were associated with lower risk of PC. The mutation R177* (c.499C>T) was referred to in the paper by c.712C>T due to a difference in transcript sequence. This missense mutation was found in one of the VHL families (family ID 153).

    1. Arg167Trp (712C-T)

Genomic DNA was extracted from peripheral blood and PCR amplifications of the three exon regions were performed. 4 of 5 families presenting with non-syndromic familial pheochromocytomas were found to have VHL germline mutations. The mutation R167W (referred to in the paper by c.712C>T due to a difference in transcript sequence) of the VHL gene was found in one proband and was confirmed to occur in other members of their family. The patient and their familial members had no clinical manifestation of hereditary syndromes, including MEN 2A, MEN 2B, VHL disease, and NF1. The laboratory and image examinations also excluded the presence of the above-mentioned hereditary syndromes. ACMG evidence codes: 'PP4' since the patient presents phenotypes highly specific for a disease with a single genetic etiology.

    1. Patients' records in a tertiary care university hospital were reviewed. Fifty-four Black patients presenting with phaeochromocytoma between 1980 and 2009 were included. Two patients had VHL syndrome (submitted for publication). This diagnosis of VHL syndrome was made in the pregnant woman (Case 9) who had bilateral adrenal phaeochromocytomas. There were no clinical features of VHL syndrome; however, MRI showed a haemangioblastoma in the cervical spine. Genetic testing revealed a missense mutation (c.499C>T) in the VHL gene, located on chromosome 3p26-p25. Her 6-year-old daughter screened positive for the mutation; her 8-year-old son was negative. Both mother and daughter will be investigated further and monitored long-term. ACMG PP4

Data was collected from 82 VHL mutation carriers in the Dutch VHL surveillance program. One patient was found with this missense varaint in the VHL gene. The patient was 43Y at the last follow-up and presented with: retinal hemangioblastoma, spinal cord hemangioblastoma, renal cell carcinoma, pheochromocytoma, and renal cysts. Family history was not described. This cohort was published later in PMID 31087189.

    1. c.499 C[T

A study describing phenotypic and genotypic characteristics of VHL in an Indian population of 31 subjects, from 15 families diagnosed with VHL. Multi-cystic pancreas and CNS hemangioblastomas were the most common manifestations in this population. This missense mutation was found in a male VHL patient with retinal angiomas and pheochromocytoma (family ID D).

    1. 499C>T Arg167Trp Germline mutation

Tissues embedded in paraffin blocks were collected from 203 cHAB patients operated in different Medical Centres in Poland. Eight of these harboured germline mutations in unrelated patients from distinct undiagnosed VHL families. Various variants were unverified as somatic or germline as they were detected in tumour but blood samples were not tested. Other phenotypes and demographic information is not known. Patient 53 was found with a germline p.Arg167Trp mutation in the VHL gene.

    1. Results: A germline missense point mutation at codon 238 of the vhl gene (CGG-->TGG; Arg-->Trp) was detected in all patients with phaeochromocytoma and in only one of the asymptomatic family members.

To study the presence of germline mutations in the von Hippel-Lindau gene (VHL) in a kindred with a predominance of familial phaeochromocytoma in order to confirm the diagnosis of von Hippel-Lindau disease (VHLD) as well as to identify asymptomatic members. DNA extracted from peripheral blood was amplified by the polymerase chain reaction using oligonucleotide primers corresponding to exon 3 of the VHL gene. Specific mutations were screened by restriction endonuclease digestion of PCR products with Msp I. The results were confirmed by DNA sequence analysis. Two generations of a family consisting of 15 individuals were studied. A germline missense point mutation stated in the paper as codon 238 of the VHL gene (CGG-->TGG; Arg-->Trp) due to a difference in transcript sequence, was detected in all patients with phaeochromocytoma and in only one of the asymptomatic family members. The mutation (R167W (c.499C>T)) was found in 6 members total, and presumed to be in at least another 2. Patient II3 and patient III6 had cerebellar hemangioblastomas. Patient III6 also had retinal angiomas. Cosegregation with disease in multiple affected family members in a gene definitively known to cause the disease (ACMG code: PP1).

    1. germline mutations

Mutational analysis for 16 patients with clinically diagnosed VHL disease, revealed 12 germline mutations. This missense mutation was found in 4 VHL family members (proband no. 6). The 65-year-old father had renal cell carcinoma; The 37-year old sister had pancreatic cysts and the 33-year old sister had retinal angiomas. The 38-year-old proband had hemangioblastomas of the central nervous system, renal cell carcinoma, pancreatic cysts, and epididymal cystadenoma. 1. c.499C>T

Of 31 Brazilian patients, five were found to harbour the c.499C>T (p.Arg167Trp) mutation of the VHL gene. Genomic DNA was extracted and all VHL coding regions were analyzed by Sanger sequencing. MLPA was performed to investigate large deletions using SALSA® MLPA® P016 VHL probe mix. Data analysis was performed with Genescan 3.7. Two of these patients were familials. Individual 12 of family 6, age 36, presented with paraganglioma, pheochromocytoma, pancreatic endocrine tumor (9.6cm), renal cyst, pancreatic cyst, retinal hemangioblastoma as well as a local duodenal invasion. Individual 13 of family 6, a 16 year old male, presented with paraganglioma and a right-side pheochromocytoma (1.7cm). Individual 5 of family 3, age 30, presented with paraganglioma, pheochromocytoma, pancreatic endocrine tumor, renal cell carcinoma pancreatic cyst, CNS hemangioblastoma and retinal hemangioblastoma. Individual 11 of family 5, age 26, presented with paraganglioma, pheochromocytoma, and pancreatic endocrine tumor. Finally, individual 25 of family 18, age 35Y, presented with paraganglioma, pheochromocytoma at age 26Y, pancreatic endocrine tumor at 35Y, and CNS hemangioblastoma.

    1. Screening of 92 unrelated patients with VHL disease revealed 61 DNA variants. No variants were found in 96 control cases. The R167W (c.499C>T) germline missense mutation was found in a VHL type 2A patient, a VHL type 2B patient and a family with 5 VHL type 2B patients (patient no. V30, V23, V1). All of these families presented with pheochromocytoma. The first two individuals had no family screened but 14 unaffected family members showed no evidence of this mutation in the family with 5 mutation-positive affected members. All of these families are stated as being previously reported in PMID 15300849.

    1. VHL c.499C>T.

This paper presented a pheochromocytoma detected by autopsy. The index case was a 26-year-old male who presented to the emergency department complaining of obstipation, severe headache and abdominal pain. The family background of the parents and siblings history is unavailable since the patient was an orphan. An autopsy revealed bilateral pheochromocytoma and acute myocardial infarction. Molecular genetic analyses for germline mutations of the VHL gene were performed after DNA extraction from normal liver tissue using standard methods. A mutation in VHL gene R167W (c.499C>T) was detected.

    1. We have described the development and validation of a new gene-expression diagnostic test called Pheo-type for classification of PPGL tumors based on the underlying genetic driver of the disease. Our assay confirms that the predominant gene-expression subtypes of PPGL are closely linked to tumor genotype and for the first time show that classification can be achieved using a relatively small gene set. Pheo-type is similar to other clinical assays developed on the Nanostring platform for subtyping of solid tumors (50–52). As with these other studies, we have shown that robust gene-expression analysis can be achieved from small amounts of FFPE tissue material generating data that are comparable to other gene-expression platforms. The assay is rapid, largely automated, and cost effective, enabling the generation of results in 2 days from RNA extraction to data analysis, facilitating its implementation in a routine diagnostic laboratory.
