DiseaseAssertion: Restrictive cardiomyopathy (RCM), associated with a homozygous TPM1 p.Asn279His variant.

FamilyInfo: The patient’s parents are first cousins and confirmed heterozygotes for p.Asn279His (Fig 2).The patient’s 75-year-old father was diagnosed with HCM at 42 years of age. He reportedly has a history of heart failure but is currently asymptomatic. The patient’s 67-year-old mother had her first echocardiogram after the patient’s diagnosis.(Table I).

CaseHPOFreeText: She was diagnosed with RCM and is being evaluated for heart transplantation. was referred for chest pain, lightheadedness, shortness of breath, significant fatigue, dyspnea on exertion, three-pillow orthopnea and abdominal bloating. Eighteen months previously, she had been diagnosed with an unspecified form of cardiomyopathy after developing volume overload while hospitalized for a right oophorectomy and uterine fibroid removal. Her symptoms were consistent with NYHA III–IV heart failure. She has continued to have significant heart failure symptoms that have been difficult to control with medical therapy. Freedom from arrhythmias, however, has been notable. She is currently being considered for orthotopic heart transplantation but her evaluation has been complicated by psychiatric issues.

    1. The Ca2+handling properties of hiPSC-derived CMs were studied by Ca2+ imaging. HCMT-CMs had significantly more abnormalities than WT-CMs and HCMM-CMs, when all time points in each group were combined (Figure 2(d), n = 6 in statistical analysis, p < 0.05). Spontaneously beating CMs were divided into five different rhythm categories (normal, oscillation, low/middle peaks, double peaks, and plateau abnormality) based on the abnormalities observed in their Ca2+ signals (Figures 2(e) and 2(f)). HCMT-CMs had significantly higher number of double peaks than HCMM-CMs (p < 0.05, n = 6 in statistical analysis) when both cell lines and all time points were combined for each group

      ExperimentName: Ojala 2016 hiPSC-derived CMs

      PatientCell: CL_0002620 (skin fibroblasts)

GeneFunction: GO:0008016 (regulation of heart contraction)

GeneAlteration: hiPSC-derived cardiomyocytes (CMs) generated from skin fibroblasts collected from patients carrying a TPM1-Asp175Asn mutation

FunctionalEvidence: Figure 2 shows enlarged cardiomyocytes (CMs) (a-b), an increase in multinucleated CMs (c), and calcium handling abnormalities (d) for the HCM-TPM1 variant CMs compared to WT CMs.

    1. the effect of the mutations was a small decrease (5%) in the maximum sliding speed in the presence of saturating calcium (Table)

ExperimentName: Karibe 2001 In vitro motility

      NonPatientCell: mechanical actin-myosin functional assay

      GeneFunction: GO:0030049 (muscle filament sliding)

GeneAlteration: PCR-based mutagenesis of Ala to Ser was used at nucleotide position 340 (GenBank M19713) to produce cDNA encoding the V95A variant. This coding sequence was inserted into the pET3d expression vector and the wild-type or mutant tropomoysin protein was expressed in DE3 cells before being purified to homogeneity for use in the in vitro assays.

FunctionalEvidence: Mutant tropomyosin-containing actin-troponin-tropomyosin filaments combined with rhodamine-phalloidin had slightly decreased (5%) sliding speed on a heavy meromyosin-coated surface compared to wild-type tropomyosin-containing filaments under saturating calcium conditions (Table 1).
