Of these, 22.5% (64/285) diagnosed with Pompe disease harboured c.1933G>A, c.1A>G, c.1927G>A and c.2783G>C as common and 10 novel pathogenic variants while 7.4% (21/285) patients diagnosed with Fabry disease carried c.851T>C, c.902G>A, c.905A>C and c.1212_1234del as frequent disease-causing variants along with 7 novel pathogenic variants. As many as 48.4% (138/285) patients were diagnosed with Gaucher disease and had c.1448T>C as the most common pathogenic variant followed by c.1342G>C and c.754T>C with 7 previously unreported disease-causing variants and in the 21.7% (62/285) diagnosed cases of Hurler disease, c.1469T>C, c.754delC c.568_581del and c.1898C>T were identified as the most common causative variants along with 21 novel pathogenic variants.

CaseNotHPOFreeText: This proband is believed to be an affected MPS-patient however, the proband did not have any obvious clinical symptoms to date (up to two years of age). The paper states that this attenuated MPS I case will need a long term follow up.

CaseEnzymeAssay: Proband identified by newborn screening. alpha-L-iduronidase activity in dried blood spot sample is 0.54 umole/L/Hr; this is below the study's DBS enzyme activity cutoff value of 3 umol/L/h

CaseUrineGAGs: Urine DMB GAG ratio, dermatan sulfate amount, heparan sulfate amount, and keratan sulfate amount was not calculated for this proband because their parents refused further analysis

    1. Since 2000 we have performed 54 HSCT procedures (on 44 children) for MPSIH at Manchester (Table 1). Of these, six (13.6 %) displayed significant cardiomyopathy at presentation. Interestingly three of these were from the same extended pedigree (from Kuwait), sharing the R628X IDUA gene mutation. All children were diagnosed within the first 4 months of life with dilated cardiomyopathy and significant LV functional impairment. Symptomatic cardiac failure was the predominant presenting feature in five children, with silent cardiomyopathy identified on screening echocardiography in the other. Median FS and EF at diagnosis were 12.5 % (range 3–20 %) and 35 % (15–40 %) respectively. Electrocardiography (ECG) patterns varied, but included strikingly tall QRS complexes consistent with LV enlargement/hypertrophy and evidence of myocardial ischaemia (ST segment depression; T wave inversion) in several cases. Further detailed cardiac investigation (including serum troponin) was rarely performed.

Case#: Patients 2 (male), 4 (male), and 5 (female). Kuwait. Onset between 1wk-3mo.

      DiseaseAssertion: MPS I Hurler

FamilyInfo: Patients are from the same extended pedigree (family in Kuwait)

CasePresentingHPOs: α-L-iduronidase activity= 3.73 umol/g/hr (normal range 10–50) after commencing ERT. ERT for 9 wks. Congestive cardiac failure. LV fractional shortening=10%. LV ejection fraction=29%. HSCT at 17mo. α-L-iduronidase activity= 11.24 umol/g/hr (normal range 10–50) at follow up. GAG= 31.3 after transplant. Enzyme replacement therapy results in a significant reduction in urine GAGs (either total or heparan sulfate)

    1. Other MPS I-H nonsense mutations (W180X, Y343X, Q400X and R628X), harbored in the compound heterozygous state, were also challenged, resulting in relevant rescue upon treatment with the exception of the Y343X variant. The highest degree of functional rescue was observed for the R628X/W402X heterozygote, suggesting additive effects of the R628X mutation, whose sequence context (UGAC) is predicted to be highly susceptible to suppression over the unresponsive W402X [57].

This paper evaluated factors shaping the outcome of readthrough, namely the nucleotide and protein context, with the latter being pivotal to produce functional full-length proteins. This variant was mentioned as a MPS I-H nonsense mutation, but no new case or experimental information.

    1. To assess the characteristics and distribution of causal variants for Mendelian disorders in the ChinaMAP, we filtered the pathogenic variants with the annotation of ClinVar database (20180603)22,23 and HGMD (Human Gene Mutation Database, 2016.2),24 and further analyzed the disease-causing variants following the guidelines from the ACMG (American College of Medical Genetics and Genomics).25 A total of 2026 variants or 1619 variants in the HGMD DM set were annotated as pathogenic or likely pathogenic by the ClinVar or ACMG, respectively (Fig. 3c and Supplementary information, Table S7).

Database information for this variant was compiled on supplementary table s7, but no new case or experimental data.

    1. Five patients (Patients 1, 4, 5, 6, and 7) had transient urticaria on the trunk, face, arms, and legs during an infusion given during week 4 or later, and in four patients (Patients 1, 5, 6, and 7) it recurred during subsequent infusions beginning at or after week 20. During these episodes, urticaria began midway through an infusion but resolved soon after the infusion was completed. In the four patients with recurrent urticaria, the episodes eventually became less frequent and less severe and finally stopped. In three patients (Patients 1, 5, and 6), the urticaria was accompanied by angioedema (thickening of the tongue and tightening of the throat) on a total of nine occasions and by moderate transient hypoxemia on three occasions.
