CaseHPOFreeText: Inclusion criteria were patients with a history of mucocutaneous bleeding (Figure 1 shows a bar diagram of all the clinical manifestations in the patient cohort in this article)

      CaseNotHPOs: N/A

ProteinExpression: Platelet receptor alphaIIbbeta3 protein was analyzed by flow cytometry followed by western blotting

Platelet Aggregation: Inclusion criteria for probands was absent or reduced platelet aggregation with agonists such as ADP, adrenaline, arachidonic acid, and collagen (no mention of ristocetin here-- this article does not tell us the specific aggregation for each proband in the paper)

    1. The patient was born in 1944; at the age of 7 he underwent a tonsillectomy, bled excessively and required a blood transfusion. He had dental extractions when he was 14 and bled for a week afterwards. At 25 he had a birthmark removed from his left shoulder and again bled excessively. At that time two-stage FVIII:C assays were performed and he was diagnosed as having mild haemophilia A with a FVIII:C level of 19%. 3 years later (in 1972) he presented with a large haematoma of the buttock after minor trauma and a two-stage assay reported a FVIII:C level of 11%. Interestingly, his APTT was always normal. In 1977 he had sebaceous cysts removed under the cover of FVIII concentrate. He received further concentrate in 1982 following an injury to his left thigh and 6 weeks later developed hepatitis (in retrospect hepatitis C). He has not had any further bleeding problems. At a recent routine visit his baseline FVIII:C level was checked with our (now routine) one-stage assay and found to be normal. Subsequently on three blood samples taken on different days we obtained the assay results shown in Table I. On the second of these samples the factor VIII antigen was measured by Immunozym ELISA (Techniclone, Dorking) and found to be 82 iu/dl.

GenotypingMethod: Sequence variations in the VWF were identified through direct sequencing, while the detection of large rearrangements utilized the multiplex ligation-dependent probe amplification (MLPA) technique or assessed copy number variations (CNVs). Sanger sequencing methodology was applied for VWF analysis in patients investigated prior to 2016 (approximately 14% of IPs), whereas next-generation sequencing (NGS) was employed for the analysis of VWF in patients inspected from 2017 onwards (about 86% of IPs). In both strategies, the entire VWF, encompassing all 52 exons, intron/exon boundaries, and promoter regions, was analyzed.

      "Myocardial bridging, Pulmonary edema"

CaseHPOFreeText: In the last weeks, patient had frequently been visited by a pediatrician to treat seasonal allergies. Full-body CT-scan was performed to exclude abuse or trauma and did not show any anomaly. The heart (weight: 240 g, longitudinal diameter: 11 cm, transverse diameter: 10 cm, thickness of the anterior wall of the right ventricle: 0·3 cm, thickness of the left ventricle: 1·0 cm) did not reveal morphologic or structural anomalies of ventricular walls and valves. The coronary circulation was right-dominant and presented myocardial bridging (length: 1·1 cm, thickness: 0·5 cm) involving the middle segment of the left anterior descending coronary artery. No other significant elements were found upon macroscopic observation, with the exception of pulmonary edema. Histopathological examination showed patchy interstitial fibrosis, myocyte disarray and waving of the myofibers in the tissue surrounding the tunneled artery. Several areas of interstitial and subendocardic fibrosis in the other samples of the left ventricular myocardium were also observed.

      CaseNotHPOs:

      CaseNotHPOFreeText:

CasePreviousTesting: The low value of serum tryptase (6·12 µg/L) excluded a fatal anaphylaxis. Toxicology tests were negative. NGS analysis used a custom resequencing panel which included 147 genes associated with cardiomyopathies and channelopathies. Authors identified five rare genetic variants in heterozygous state (Table 4).

GenotypingMethod: Genomic DNA was extracted from postmortem whole blood of the proband. Whole blood samples of the proband mother, father and sister were obtained to extract genomic DNA and thus to perform Sanger sequencing to analyze the segregation of each genetic variant identified by NGS. Authors performed Sanger sequencing on the DNA extracted from the mother, the father and the sister of the proband. The father carried both variants identified in the DPP6 gene, whereas the mother carried the rare variants identified in MYH7, SCN2B and NOTCH1. The sister carried all the genetic variants.

CaseNotHPOsFreeText: On admission to the hospital, proband had no sign or symptoms of bleeding.

ProteinExpression: In comparison to normal platelets, very low αIIbβ3 (1,053 ± 544 copies/platelet, n=3) and αvβ3 (755 ± 180 copies/platelet, n=3) expression were measured on the cell surface expression of our patient’s platelets.

PlateletAggregation: Light transmission aggregometry showed his platelets were unable to respond to ADP, collagen, arachidonic acid, and epinephrine; however, platelet agglutination in response to ristocetin was normal.

CasePreviousTesting: To identify the molecular genetic defect in this patient, we isolated genomic DNA from the patient’s leukocytes and amplified all exons of ITGA2B and ITGB3 gene by PCR.
