VariantEvidence: Quantitative Southern blot/real-time quantitative PCR analyses was normal in this patient. Known polymorphic variant on the 3’UTR of the VHL gene, as well as the 5’UTR. PCR–RFLP assay approach showed that both alleles were expressed in apparently equimolar amounts (Fig. 2d) suggesting that the 11 bp duplication has no effect on expression of the affected allele.

216 patients with clinically expected VHL disease due to family history or presence of VHL typical tumours were routinely examined in an eye centre in Germany between January 2019 and January 2020, majority of which were of European descent (~95%). Since data on the genotype–phenotype correlation of the onset of Retinal Hemangioblastoma (RH) is scarce in relation to VHL disease, this study wanted to inversitage the relationship between VHL gene variants (truncating vs single amino acid substitutions) and the ocular phenotype of the disease. 42 different rare VHL gene variants in 148 patients were detected - some of whom had RH and some who did not. Although date of onset of RH, as well as data on central nervous system haemangioblastomas (CNSH), pheochromocytoma/paraganglioma (PPGL), clear cell renal cell carcinoma (ccRCC) and pancreatic neuroendocrine neoplasm (PNEN) were collected, it was not reported on a per patient basis and therefore could not be discussed per variant. There were 3 patients reported with the p.F76del (c.227_229del) mutation. All 3 members presented with retinal hemangiomas, while no other clinical manifestations were discussed. Additionally, no family information was provided.

    1. Clinical caseA 35-year-old African-American male with VHL syndrome presented with increasing nasal congestion and epistaxis. He had a past history of multiple benign CNS and visceral tumors from VHL syndrome and was known to harbor a pathogenic germline mutation in exon 3 of VHL, R161P (c.482G > C). Computed tomography (CT) imaging showed a right-sided nasal mass.Magnetic resonance imaging (MRI) further characterized a large enhancing mass extending through the right medial orbital wall with extraconal

    2. In a study of 114 unrelated VHL families, 85 germline mutations were found. VHL mutations were detected in affected family members, but not in unaffected family members or 96 normal individuals. This mutation was also found in a VHL type 1 family of 27 individuals (family ID 3003), using PCR and SSCP sequencing. The mutation of c.502_503insTTGTTCGT was referred to in the paper by c.715insTTGTTCGT due to a difference in transcript sequence. Five of 11 individuals were identified with renal cell carcinoma, 11 with retinal hemangioblastomas, and 9 with CNS hemangioblastomas. Previously published in PMID 8956040 (type 1 3003NCI); and PMID 9829911. ACMG codes as follows: PVS1 a frameshift variant in a gene where loss of function is a known mechanism of disease.

Germline mutational data on VHL was gathered from various countries to assess variation in germline mutations that produce disease within different populations. Families were grouped according to three possible outcomes of disease: renal cell carcinoma without pheochromocytoma, renal cell carcinoma with pheochromocytoma, or pheochromocytoma alone. SSCP and denaturing gradient gel electrophoresis identified mutations in 300/469 (63%) of families affected with VHL. This germline familial VHL mutation (L153fs (c.457delC)) was taken from the Cambridge, Department of Pathology database. It had an unknown number of members belonging to Family ID 41CA, reported in Table 3, as being found in VHL families without pheochromocytoma, and with renal cell carcinoma. Additionally, it was reported in table 5, specified as families without detailed clinical information, under family ID 99CA. It was reported in the paper as c.670delC due to a difference in transcript reference.

VariantEvidence: Tumor analysis was performed since there was a paraffin embedded sample. Patient 10 did not have loss of heterozygosity, but was found to have a somatic variant in the exon 3 of VHL (c.482G>A; p.Arg161Gln). This was the only patient observed to have a cluster of tumor cells with a positive membranous CA9 immunostaining, this can be seen in VHL-related PPGL. CA9 is a protein reported to influence regulation cell proliferation and tumor progression and is known to be expressed at the membrane of tumour cells in the case of VHL inactivation can be seen in VHL-related PPGL. Subsequently as expected, the PPGL of patient 10 had a significantly low expression of VHL E1-E1' mRNA VHL and PNMT mRNA compared to VHL-tumours used as controls. This suggests that the two VHL variants change the VHL mRNA splicing.

Of 31 Brazilian patients, one was found to harbour the c.540delC (p.I180Mfs*22) mutation of the VHL gene. Genomic DNA was extracted and all VHL coding regions were analyzed by Sanger sequencing. MLPA was performed to investigate large deletions using SALSA® MLPA® P016 VHL probe mix. Data analysis was performed with Genescan 3.7. This patient, age 14, presented with CNS and retinal hemangioblastoma. It is not clear if this mutation is familial.

VariantEvidence: The modified amino acids are located in a domain common to all pVHL isoforms, the highly conserved cctbinding domain of the protein, involved in the formation of the pVHL elongin complex that is crucial for tumor suppression. Furthermore, the mutant protein is prematurely truncated and lacks the C-terminal half of exon 2 and exon 3 completely, which includes the nuclear export signal and the elongin C binding site of pVHL. Thus, this truncated protein would affect the two functional isoforms of pVHL: the major isoform pVHL30 and pVHL19, but not isoform 2, which lacks exon 2 but is not functional

    1. Thirty-five unrelated patients suspected of having von Hippel-Lindau disease were analyzed and 32 VHL gene variants were found in 35 patients using PCR and SSCP by peripheral blood leukocytes. An association of clear cell renal carcinoma development with a relatively high loss of structural stability in VHL protein missense mutations was discovered. This missense variant (p.L128R) was found in a 22 year old patient with retinal and cerebellar hemangioblastomas, renal cysts, and pancreatic cysts (patient no. F41). Familial antecedents were found but unknown of any additional features than the index case.

    1. Molecular and clinical analysis of 126 French VHL families tested for renal involvement revealed 92 different mutations. 90 of these families had renal involvement. The p.K159Efs*15 (c.473dup) germline variant of the VHL gene was detected in an individual (ID 5299, Family 235). They presented with mild renal disease (simple renal cyst, multicystic kidney[s], small lesion [less than 10 mm in size] of unknown significance, or small solid tumor without indication for surgical removal [less than 30 mm in size]). Individual RCC phenotype is noted as negative. The paper states that this individual is previously reported in PMID 12202531 and/or 9829912.

CaseHPOFreeText: The patient P1-IT is a 55 year-old female sporadic case referred for genetic testing with a clinical diagnosis of Usher syndrome (Table 1). She is the second of two sisters born to reportedly non-consanguineous parents from a small local community. Her medical history is insignificant with no cases of hearing loss and/or RP in the family (Fig. 1a).

The patient presented night blindness since the age of 14 years, and received a diagnosis of RP at the age of 19 years. At the last ophthalmological examination (55 years), she had a hand motion visual acuity in both eyes, an RP fundus with bone-spicule pigmentation (BSP) in mid-periphery, and macular atrophy (Fig. 2a). FAF imaging revealed reduced autofluorescence at the posterior pole, with focal areas of absent autofluorescence surrounding the optic disc and in the macular area (Fig. 2a). SD-OCT imaging revealed a diffuse retinal pigment epithelium (RPE) dystrophy with epiretinal membranes but without cystoid macular edemas (CME) (Fig. 2a). Scotopic and photopic ERG responses were unrecordable in both eyes (Table 2), and she had bilateral pseudophakia. She also suffers from post-verbal hearing loss with a reported onset at the age of 18 years and she is currently using hearing aids (auditory prostheses). The most recent audiological examination confirmed the diagnosis of moderate, bilateral and symmetric sensorineural hearing loss (SNHL) with a Pure Tone Average (PTA) of 59 dB in both sides and a sloping audiogram at intermediate-high frequencies (500 Hz–8000 Hz) (Fig. 3). The hearing loss appeared non-progressive over the years.

CaseHPOFreeText: This proband started experiencing a decrease in visual acuity since her teens, followed by night blindness, and was referred for an ophthalmological assessment at the age of 16 years. She was from a non-consanguineous family, however both parents came from the same village (Fig. 1f).

At the first visit, her BCVA was 0.3 D in both eyes and the scotopic ERG responses were severely reduced, while the 30-Hz Flicker had reduced amplitude. At her last visit, aged 38 years, her BCVA was 0.1 D in both eyes, and the GVF was reduced to 30° bilaterally. FAF imaging showed several areas of patchy hypoautofluroescence at the posterior pole, which increased over time. The OCT scans at the last visit showed reduced retinal thickness with a severely altered layer of RPE and an absent ellipsoid layer at the posterior pole (Fig. 2f, Table 2). She never complained of hearing problems, however, the audiogram perfomed when she was 36 years old showed a reduced hearing threshold at higher frequencies (between 2000 and 4000 Hz) to 35 dB in the right ear and to 20 dB in the left side (Fig. 3, Table 2).

CaseAddInfo: No other family member had vision problems, however her younger sister (II.3) had severe hearing impairment (Fig. 1f). The genetic testing of the proband revealed a homozygous missense variant c.908G>A, p.(Arg303His) in the AGBL5 gene (Table 1, Fig. 1f). The sister who presented isolated severe hearing loss was heterozygous for the AGBL5 variant but was found to be compound heterozygous for two variants (c.35del and c.2229T>C) in the GJB2 gene, explaining her hearing impairment (Fig. 1f). The proband did not carry any of the GJB2 variants.

gnomAD: Variant present at a frequency of 0.006316 in the Ashkenazi Jewish (9658 alleles tested) and 0.001173 in the Latino populations.

AddCaseInfo: To investigate whether the stop variant represents a founder alteration, we performed haplotype analysis for each of the index subjects (CIC00173 II.2 from family A) and CIC01611 II.1 from family B) by selecting 11 microsat- ellite DNA markers flanking the KIZ locus. 25 These markers were distributed over a physical distance of z7.38 Mb, cor- responding to z10.5 cM (genetic distance according to Marshfield genetic maps). Both subjects were found to share a common haplotype of five polymorphic microsa- tellites (DS20S54, DS20S63, DS20S190, DS20S868, and DS20S180) flanking KIZ and spanning z1.75 cM (1.18 Mb) (Figure 1). This result suggests that c.226C>T (p.Arg76*) is most likely a founder mutation causing autosomal-recessive RCD in the southern European population

CasePresentingHPOsFreeText: All probands underwent a complete ophthalmological evaluation, including best corrected visual acuity, biomicroscopy, color fundus photography, fundus autofluorescence, electrophysiological testing, and swept‐source optical coherence tomography (DRI SS OCT Triton, (TOPCON corporation)). The clinical diagnosis of a RD was made by retina specialists based on the patient's history of visual symptoms, clinical examination, retinal imaging, and electrophysiology. Clinical and family history details were collected during genetic consultation.

      CasePreviousTesting:

CaseGenotypingMethod: Gene enrichment during library preparation was performed using the commercial ClearSeq Inherited Disease Panel (Agilent Technologies). This panel has a 10.5 Mb capture capacity and targets the coding exons and splicing regions of 2,742 genes involved in inherited disorders. The panel includes 199 RD‐related genes, in accordance with the Retinal Information Network (RETNET; https://sph.uth.edu/retnet/home.htm) and the Online Mendelian Inheritance in Man (OMIM) catalog (https://www.omim.org/ ) at the time of the study (accessed on September 2018). One hundred and four of these genes have been implicated exclusively in non‐syndromic RDs, 23 are associated with both syndromic and non‐syndromic forms of RDs, and 72 are involved only in syndromic RDs (Table S1).

CaseHPOFreeText: Proband was born a from consanguineous union between first degree cousins of Portuguese origin. She was diagnosed with a classical form of retinitis pigmentosa at the age of 36 years.

Initial visual complaints started with photophobia at 11 years of age and progressed to night blindness and photopsia by the age of 25. Constriction of the peripheral visual field was noted at the age of 28. Disease progression led to a sharp reduction in visual acuity at age 38 and to loss of color vision by the age of 40. The patient underwent cataract surgery at age 40 and 48. At 50 she developed metamorphopsia in the OS.

At the last ophthalmologic examination, patient P2 showed a BCVA of 0.3 in the OD, 0.16 in the OS, and severe constriction of the peripheral visual field, with bilateral tunnel-like vision, restricted to the 5 degrees central. Dilated fundus examination revealed attenuated retinal vessels, pale optic discs (Fig 2B) and a marked generalized chorioretinal atrophy in the posterior pole and in the periphery, with areas of complete atrophy in the inferior peripheral region of the right eye. Mild to moderate mottled pigment deposition in the retinal periphery, along with some irregular white patches in the posterior pole near the superior vessels, was also observed. Fundus autofluorescence was notable for generalized hypo-autofluorescence with a wide hyper-autofluorescent ring in the posterior pole (Fig 2D). SD-OCT revealed pronounced atrophy of the outer retinal layers and the presence of a foveal cyst (Fig 2F).

P2 gave birth to two children.

P2 displays a pale optic disc, vascular thinning and marked retinal changes, with opalescent areas of the retinal tissue in the periphery and central atrophic lesion. Scarse pigment deposition can also be found adjacent to the optic nerve head and periphery.

P2 suffered from frequent migraines and recently underwent a cerebral CT imaging examination, which revealed normal ventricular volumes and absence of other morphological anomalies.

CaseAddInfo: Patient P2’s paternal aunt (P3) and her uncle (P4) were also diagnosed with RP, showing similar progression and manifestations. Specifically, P3 began to be visually impaired in her 20s with the onset of night blindness. Her visual field progressively shrank for both eyes and considerably worsened after the birth of her two daughters in her 30s, evolving to a state of complete blindness by the age of 50.

There is no clinical history of other ciliopathic disorders in this family, with the exception of P4, who suffered from chronic bronchitis. This latter patient died of multiple myeloma prior to this study, thus preventing further clinical analysis. Interestingly, P4 never conceived any offspring in spite of multiple attempts (as reported by his sister, patient P3), whereas both female patients P2 and P3 had two healthy children.

CasePresentingHPOsFreeText: All probands and their affected and unaffected relatives were enrolled at the Retinal Dystrophy Unit of ASST Santi Paolo e Carlo Hospital, University of Milan (Italy), from 2011 to early 2019. They underwent detailed clinical examination, including best-corrected visual acuity, slit-lamp examination, visual field analysis, spectral-domain optical coherence tomography (SD-OCT), fundus autofluorescence (FAF), and dark- and light-adapted electroretinogram (ERG). Diagnosis of RP was based on typical clinical signs (optic disc pallor, bone spicule pigmentation, retinal vessel attenuation), characteristic full-field electoretinographic patterns, varying from reduced A- and B-wave amplitudes in early stages to nondetectable ERG in advanced stages (as established by the International Society for Clinical Electrophysiology of Vision), visual field constriction, and particular SD-OCT and FAF patterns. Patients suspected to have US (retinitis pigmentosa and sensorineural bilateral hearing loss) also underwent complete audiometric evaluation.

Detailed demographic data and family history were recorded by interview. For each proband, the disease (RP or US) and the inheritance model (AD-RP, AR-RP, or XL-RP) were suspected on the basis of clinical data and pedigree, when available.

      CasePreviousTesting:

CaseGenotypingMethod: DNA was extracted using a commercial kit (E.Z.N.A. Blood DNA kit; Omega Bio-Tek, Norcross, GA, USA) and analyzed by next-generation sequencing (NGS) on an Illumina MiSeq personal sequencer (Illumina, San Diego, CA, USA) using an in-house bioinformatics pipeline as previously described.9,10

The gene panels were periodically updated with new disease-associated genes reported in the literature, so that four different custom NGS panel releases were used in the 8-year period of observation. The list of genes in the latest multigene panel (used to test 267 samples between 2017 and 2019) is shown in Supplementary Table S1; a comparison of genes included in the other three panels can be found in Supplementary Table S2. All genetic variants were validated by Sanger sequencing using a Beckman Coulter CEQ 8000 sequencer (Beckman Coulter, Milan, Italy). Sanger sequencing was also used for family segregation studies when additional family members were available

CasePresentingHPOsFreeText: The prospectus was a 52-year-old Hispanic male with only hand motion vision in both eyes. He had two brothers and three children diagnosed with RP (Figure 1A). Extensive bone-spicule-like pigment deposits and severely attenuated blood vessels were seen bilaterally, consistent with end stage RP. Optic disc pallor and macular retinal pigment epithelium atrophy were also present. Goldman visual fields were not possible due to the patient’s poor vision.

CasePreviousTesting: Each proband has been previously tested for mutations in the complete coding region of CA4, CRX, FSCN2, IMPDH1, NRL, PRPF31, RDS, RHO, ROM1, and RP9. Samples were also screened for mutations in mutational “hot spots” of RP1, PRPF3, PRPF8, and NR2E3.

CaseGenotypingMethod: PCR products were treated with ExoSapIt (USB, Cleveland, OH) and sequenced unidirectionally with BigDye v1.1 (Applied Biosystems) and the primers described in Table 1. PCR product sequencing was employed to screen patient DNA for mutations in the entire coding region and flanking intron/exon junction of TOPORS.

Variant: A 1 bp deletion at nucleotide 2,569 was detected (c.2569delA; p.Arg857GlyfsX9). This deletion caused a frame-shift at amino acid residue 857, and was predicted to result in the addition of eight incorrect amino acids followed by premature termination.

Case#: Patient M (male) is an Italian boy who was diagnosed with MPS I by enzyme assay at 3 days of age. His weight was 7.5 kg (75th percentile), length was 67.0 cm (90th percentile), and head circumference was 43.0 cm (75th percentile).

      DiseaseAssertion: MPS1-H/S

FamilyInfo: The patient is a younger asymptomatic sibling who was diagnosed with the disease because of his older affected sister. Two months before patient M birth, his 4.5-year-old sister, patient F, had been diagnosed with the Hurler-Scheie form of MPS I on the basis of clinical findings, an elevated urinary glycosaminoglycan level, and low α-l-iduronidase activity in leukocytes.

He has shown no signs of dysostosis multiplex on radiographs, and his heart valves functioned normally and were not thickened or dysplastic.

      CaseEnzymeAssay: N/A

CaseUrineGAGs: 651.7 μg/mg creatinine (normal value for age: 30–200 μg/mg); predominance of heparan sulfate and dermatan sulfate. Urinary glycosaminoglycan excretion decreased to normal levels for age within 4 months of therapy, with complete disappearance of dermatan and heparan sulfate bands on electrophoresis.

CaseERT: Yes - started laronidase at the age of 5 months. Weekly intravenous infusions of laronidase, at the standard dose of 100 U/kg of body weight. Over the following 5 years, treatment was administered regularly (only 3 infusions were missed), and it was well tolerated without any significant adverse effects.

CaseNotHPOFreeText: This proband is believed to be an affected MPS-patient however, the proband did not have any obvious clinical symptoms to date (up to two years of age). The paper states that this attenuated MPS I case will need a long term follow up.

CaseEnzymeAssay: Proband identified by newborn screening. alpha-L-iduronidase activity in dried blood spot sample is 0.54 umole/L/Hr; this is below the study's DBS enzyme activity cutoff value of 3 umol/L/h

CaseUrineGAGs: Urine DMB GAG ratio, dermatan sulfate amount, heparan sulfate amount, and keratan sulfate amount was not calculated for this proband because their parents refused further analysis
