    1. To identify and characterize the anti-angiogenic domain of TIMP-3, structure function analyses and synthetic peptide studies were performed using VEGF-mediated receptor binding, signaling, migration and proliferation. In addition, the ability of TIMP-3 peptides to inhibit CNV in a mouse model was evaluated.

Selection B- Gene function is consistent with phenotype.

GOterm: GO:0016525 (negative regulation of angiogenesis)

FunctionalEvidence: The previously known ability of TIMP3 protein to block the binding of VEGF to the VEGFR-2 receptor (PMID:12652295, PMID:19478078) is mapped to the C-terminal region of TIMP3 consisting of amino acids 160-211 (Figures 1c, 2b). This region is relevant to disease in that nonsense variants (PMID:10854443) and splice site disrupting variants (PMID:9760202) of TIMP3 are predicted to remove most or all of this domain. This region is not only necessary and sufficient to block VEGF binding to VEGFR-2 in ELISA assays (Figure 2b), but also can block VEGF-stimulated endothelial migration (Figure 4) and proliferation (Figure 6). In an animal model of choroidal neovascularization (CNV) with lesions induced by laser, the injection of C-terminal TIMP3 peptides can limit CNV-based lesion size (Figure 7). This introduces the possibility that some phenotypes of Sorsby fundus dystrophy such as CNV may be mediated by loss of normal TIMP3 function rather than gain of an abnormal TIMP3 function.

CaseHPOFreeText: Proband was consanguineous - A multi-generational pedigree indicated her parents were first cousins of Palestinian descent.

A 42-year-old woman was referred for possible retinitis pigmentosa (RP). She had increasing nyctalopia since age 26. Recently, she had noticed worsened peripheral vision. She was taking ziprasidone for schizophrenia. She was otherwise well.

On examination, best corrected visual acuity was hand motion right eye and 20/50 left eye. Cycloplegic refraction was −4.00 + 1.00 × 90 OD, −3.00 + 1.00 × 90 OS. Pupillary responses and eye movements were normal. She did intermittently eccentrically fixate with the left eye. Slit lamp examination was normal with the exception of bilateral, dense, superior posterior subcapsular cataract, which did not involve the visual axis. Goldmann applanation tonometry was normal. Fundus examination showed moderate optic atrophy, moderate retinal vessel attenuation, almost 360 degrees of “bone spicule” pigmentary clumping, attenuation of the retinal vessels, and bilateral preserved central foveal islands of normal retina (Figure 4a). No optic nerve drusen were seen in the fundus examination. OCT showed an almost complete loss of photoreceptors, except for the subperifovea in both eyes (Figure 4b). FAF of the right eye was of poor quality. FAF of the left eye had marked peripapillary hypofluorescence (Figure 4c). She had maculopathy in the left eye with a central hyperautofluorescent ring inside of an area of mottled hypoautofluorescence. Visual fields were constricted to less than 10 degrees in each eye. Full field and multifocal ERG were isoelectric.

    1. The SFD eyes showed advanced pathology, including loss of virtually all retinal photoreceptors and gliosis of the remaining retinal layers. Bruch’s membrane was markedly thickened with extracellular PAS positive deposits that ranged in thickness from 15 to 60 μm (Fig 1D). The deposits, which were located between the RPE and elastin sublayer of Bruch’s membrane, were strongly TIMP-3 positive throughout their thickness where the RPE cells were retained (Fig 1E). No TIMP-3 reactivity was found in the cytoplasm of the melanin containing RPE cells, which were thinned but usually retained as a monolayer next to the deposits. In occasional areas where RPE cells had been lost, the adjacent Bruch’s membrane deposits remained PAS positive (Fig 2A) but had no TIMP-3 immunoreactivity (Fig 2B).

Selection B Expression altered in patients

ExperimentName: Immunohistochemical localization of TIMP3 expression in eye sections from SFD and unaffected human patients

TissueExpression: In both unaffected and SFD patient tissues, strong TIMP3 expression was observed in Bruch's membrane (UBERON:0003957) and associated drusen (Figure 1).

ExpressionEvidence: As previously published (PMID:9006347), strong TIMP3 expression was observed in Bruch's membrane and drusen in both normal and SFD samples (Figure 1). In SFD patient tissues, thickened Bruch's membrane stained positively for TIMP3 as well, but only in areas where the retinal pigment epithelial cells were still intact (not in areas exhibiting RPE loss). Numerous extracellular deposits that stained positively with Periodic acid-Schiff (presumably drusen) were also TIMP3-positive. In some areas, retinal blood vessels were occluded by TIMP3-positive deposits as well. These findings clearly localize TIMP3 to the sites closely associated with pathogenesis within the SFD retina. Interestingly, SFD-like deposits in retinitis pigmentosa tissues were similarly TIMP3-positive (Figure 2).

    1. A family with RP3 type of X-linked retinitis pigmentosa: an association with ciliary abnormalities

    1. To identify the genetic mutations responsible for MSS, we subjected the genomic DNA of an affected individual born to unrelated parents (FI1; Figure 1 and Table S1) to ciliome resequencing, using a 5.3 Mb customized Agilent SureSelect Target Enrichment library to capture 32,146 exons of 1,666 genes. We first focused our analysis on consensus splice-site changes, nonsynonymous variants, and insertion and/or deletion in coding regions. Considering that MSS-causing mutations are rare, we assumed that the affected individual was likely compound heterozygous for variants absent in the dbSNP132, 1000Genome, and in-house databases. This pointed to one candidate gene only, IFT140 (intraflagellar transport protein 140 homolog (chalydomonas)

Enzymatic tests ruled out defects in the CLN1, CLN2, and CLN10 loci, while CLN3, CLN6, and CLN8 defects were ruled out by Sanger sequencing of the coding regions. SNP array-based homozygosity mapping was performed, revealing 7 homozygous candidate regions in the affected members from the (consanguineous) family. These regions were analyzed for candidate NCL genes, finding CLN5 as the most likely locus to harbor the causative variant.

CaseMethod1: SNP array-based homozygosity mapping was performed, revealing 7 homozygous candidate regions in the affected members from the (consanguineous) family. These regions were analyzed for candidate NCL genes, finding CLN5 as the most likely candidate for further genotyping.

CaseMethod1: Sanger sequencing of PCR products from all exons and flanking regions of the GUCY2D locus

CaseMethod2: HhaI restriction digest of the PCR-amplified exon 13

CaseGenotypingMethod: The proband was genotyped by Sanger sequencing of PCR products from all exons and flanking regions of the GUCY2D locus. Parents were implied to have been genotyped as well. The patient and family members were subjected to additional genotyping through restriction fragment length polymorphism analysis, then confirmation by HhaI restriction endonuclease digestion of PCR products from the relevant exon 13.

Variant: The proband harbors the NM_000180.4(GUCY2D):c.2513G>C (p.Arg838Pro) variant in the heterozygous state.

      ClinVar: 811743

gnomAD: The variant was not found in gnomAD.

VariantEvidence: NM_000180.4(GUCY2D):c.2513G>C (p.Arg838Pro) is a missense variant located within the coiled-coil domain that mediates dimerization. Codon 838 has proven to be a hot spot disrupted in other families similarly affected with autosomal dominant cone-rod dystrophy (PMID: 11115851, PMID: 12552567, PMID: 26298565). The variant has been reported in an apparently unrelated family from this same publication.

CaseAddInfo: The proband's parents are unaffected and negative for the variant, indicating that this may be a de novo case. It is uncertain whether genotyping methods were sufficient to confirm paternity and maternity.

    1. Identification of Compound Heterozygous Mutations in the BBS7 Gene in a Korean Family with Bardet-Biedl Syndrome

      PMID: 25553308

      Gene: BBS7

      HGNC: 18758

      Disease: Bardet-Biedl syndrome 7

      MONDO: 0014435

      InheritancePattern: autosomal recessive

Prevalence: The prevalence of BBS7 variants among Bardet-Biedl syndrome cohorts has been reported at 4.2% (PMID: 19402160), while the prevalence of Bardet-Biedl syndrome overall has been estimated between 1 in 13,500 and 1 in 160,000 live births, depending on the population (PMID: 22713813).

Penetrance: The penetrance of the phenotype among homozygous cases appears to be complete.

    1. To determine if postsynaptic expression of nyctalopin is necessary and sufficient to rescue the nob phenotype, we constructed transgenic mice that expressed an EYFP-nyctalopin fusion protein on the dendritic tips of the DBCs. Immunohistochemical and ultrastructural studies verified that fusion protein expression was limited to the DBC dendritic tips. Fusion gene expression in nob mice restored normal outer and inner visual function as determined by the electroretinogram and RGC spontaneous and evoked responses. Together, our data show that nyctalopin expression on DBC dendrites is required for normal function of the murine retina.

ExperimentName: Rescue of Nyx loss-of-function mouse by transgene expression in depolarizing bipolar cells of the retina

      RescueModel: Mus musculus

GeneAlteration: This naturally occurring model, previously known to exhibit the absence of b-wave in ERG recordings, harbors an 85-bp deletion in the final exon of the Nyx gene that shifts the frame and replaces the C-terminal 61% of the protein product with 170 novel amino acids. The predicted effect is loss-of-function.

RescueVariant: Rescue was performed with a wild-type Nyx transgene, rather than a variant.

RescuePhenotype: HP:0007984 (Electronegative electroretinogram), HP:0000649 (Abnormality of visual evoked potentials), HP:0007663 (Reduced visual acuity)

RescueMethod: An EYFP-tagged mouse Nyx gene was placed under the control of the mouse GABAcp1 promoter for tissue-specific expression in bipolar cells of the retina, including rod and cone depolarizing and hyperpolarizing bipolar cells. The transgene was introduced by pronuclear injection to generate transgenic lines, then crossed to homozygous Nyx mice to yield transgenic male offspring with Nyx disruption.

RescueEvidence: The EYFP-Nyx transgene restores both the ERG b-wave (Figure 5B) and the spontaneous and light-evoked responses of retinal ganglion cells (Figure 6) in Nyx-deficient mice.

ModelVariant: The model is naturally occurring, and so the affected animals harbor a variant that does not have an exact human patient equivalent.

    1. Novel ADAM9 homozygous mutation in a consanguineous Egyptian family with severe cone-rod dystrophy and cataract

      PMID: 25091951

      Gene: ADAM9

      HGNC: 216

      Disease: cone-rod dystrophy 9

      MONDO: MONDO:0013002

      InheritancePattern: autosomal recessive

Prevalence: The prevalence of cone-rod dystrophy in general is estimated to be 1 in 40,000 (PMID: 17270046), however, the specific prevalence of cone-rod dystrophy 9 is unknown.

Penetrance: Penetrance appears to be complete in reported families, but it is possible that incomplete penetrance may be associated with some variants.

    1. By directly measuring the turnover of TIMP-3 and SFD-TIMP-3 protein in the matrix of human RPE cells, we show that in all cases mutation induces multimerization, rendering a proportion of the SFD proteins resistant to degradation and providing a mechanism for TIMP-3 accumulation in the disease.

NonPatientCell*: ARPE-19 (human retinal pigment epithelial) cell line (CLO:0001749, CL:0009004)

GeneFunction: GO:0008191 (metalloendopeptidase inhibitor activity), GO:0046872 (metal ion binding), GO:0002020 (protease binding), GO:0005615 (active in extracellular space), GO:0031012 (active in extracellular matrix)

GeneAlteration: TIMP3 expression constructs encoding the "Ser181Cys", "Ser156Cys", and "Glu139X" variants (officially referred to as the Ser204Cys, Ser179Cys, and Glu162X variants) were generated by PCR-based mutagenesis of the wild-type TIMP-3-pCI.neo construct (previously described in PMID:10854443).

FunctionalEvidence: TIMP3 variants were exogenously expressed in ARPE-19 cells alongside wild-type and empty vector controls. The percentage of exogenously expressed wild-type TIMP3 protein remaining associated with the extracellular matrix after separation of transfected cells from ECM was 20% (after 1 day) or <1% (after 4 days). All three variants were turned over at a slower rate, with 35-50% remaining after 1 day, and 20-25% remaining after 4 days (Figure 2). On the other hand, while the Ser179Cys and Glu162X variants showed reduced ability to inhibit cell surface activation of Pro-MMP2, Ser204Cys did not (Figure 3). As a result, the key, consistent function of TIMP3 variants in SFD may be (toxic) aggregation rather than loss of inhibitory activity.

      FAVariants: Variants used were equivalent to NM_000362.5(TIMP3):c.610A>T (p.Ser204Cys) (ClinVarID:12676), NM_000362.5(TIMP3):c.536C>G, (p.Ser179Cys) (ClinVarID:12678), and NM_000362.5(TIMP3):c.484G>T (p.Glu162X) (ClinVarID:12680).

CasePreviousTesting: Previous testing for mutations in the CLN1, CLN2, CLN3, CLN6, and CLN8 had already occurred, using PCR amplification and Sanger sequencing to exclude the possibility of variants in the exons and surrounding noncoding regions. Genome-wide analysis methods were not used, as NCL had already been diagnosed based on clinical phenotypes.

CaseMethod1: PCR amplification of the coding and surrounding non-coding regions of each gene, followed by bidirectional Sanger sequencing

      CaseMethod2: as mentioned

CaseGenotypingMethod: CLN5 genotyping was performed by PCR amplification of the four exons and surrounding non-coding regions was performed, followed by bi-directional Sanger sequencing. Synthetic multiplex ligation-dependent probe amplification (MLPA) was then used to calculate average gene dosage ratio for each ratio, in order to detect large deletions.

Variant: The patient harbors the NM_006493.4(CLN5):c.524G>A (p.Trp175Ter) and NM_006493.4(CLN5):c.956_959del (p.Lys319fs) variants.

      ClinVar: 56544 and 56529

gnomAD: Maximum allele frequency for NM_006493.4(CLN5):c.524G>A (p.Trp175Ter) is 0.00003517 (in the European non-Finnish population, no homozygotes). https://gnomad.broadinstitute.org/variant/13-77570221-G-A Maximum allele frequency for NM_006493.4(CLN5):c.956_959del (p.Lys319fs) is 0.0001148 (in the African / African-American population, no homozygotes). https://gnomad.broadinstitute.org/variant/13-77574979-CACAA-C

VariantEvidence: NM_006493.4(CLN5):c.524G>A (p.Trp175Ter) is a nonsense variant that results in truncation of approximately 51% of the protein product. The NM_006493.4(CLN5):c.956_959del (p.Lys319fs) variant triggers a frameshift that affects the final 40 amino acids (approximately 11%) of the protein product. While both variants occur in the final exon (exon 4) and neither is predicted to trigger NMD, the C-terminal region disrupted in both cases has been shown to be glycosylated and is important for proper localization of the protein (PMID:19309691).

CaseAddInfo: Parents do not appear to have been genotyped, so it is not clear whether the two variants are confirmed to be in trans.

    1. In 30 families, either one or both of the mutations were not identified, which accounted for 25.9% of the total alleles. Therefore, all 14 exons of the alpha-L-iduronidase gene were screened in these patients and 23 different sequence changes were found, 17 of which were previously unknown. The novel sequence changes include 4 deletions (153delC, 628del5, 740delC, 747delG), 5 nonsense mutations (Q60X, Y167X, Q400X, R619X, R628X), 6 missense mutations (C205Y, G208V, H240R, A319V, P496R, S633L), a splice site mutation (IVS12+5g>a), and a rare polymorphism (A591T).

    1. (2) utilization of a cryptic splice site for Gln310-->Ter.

PVS1 evidence: RT-PCR showed a doublet "with the upper band slightly larger than the normal band (505bp). Sequence analysis demonstrated an admixture of species with 28nt of intronic sequence, indicating use of a cryptic splice site in intron 5." "This alternative splicing creates a frameshift and an open reading frame until exon 9"

CaseERT: Yes - the patient had been treated with intravenous rhIDU for 13 years before IT rhIDU and had not undergone HSCT. The patient was also enrolled concurrently in an IRB approved non‐interventional longitudinal study of brain structure and function in MPS at the University of Minnesota. He showed stable or improved neurocognitive outcomes after 24 months of IT rhIDU. The patient showed the greatest improvement in his ability to encode verbal stimuli, moving from significantly impaired to the average level. This was accompanied by a significant improvement in adaptive behavior and an improvement to an above average level, in his ability to remember verbal material in a context. Nonverbal memory, both encoding and recall increased although those scores remained below the average range. A baseline of average IQ and attention performance also somewhat improved.
