(Developmental delay, Intellectual disability, Seizures, Scoliosis, Deep vein thrombosis, Osteoporosis of vertebrae, Genu valgum, Myopia, Long face, Narrow face, High palate, Crowded teeth, Respiratory failure, Cardiac arrest, Enlarged liver)
HPOsFreeText Diagnosed with homocystinuria after exam for developmental delay at 13mo. Patient had severe intellectual disability and seizures. At 14yo she had surgery for scoliosis, postsurgery she had a popliteal vein thrombosis. In addition to osteoporosis of the spine, she had genu valgum and myopia. The patient also had a long and narrow face, high-arched palate and crowded teeth. Respiratory failure and cardiac arrest at 22yo likely due to overuse of antiasthmatic medication.
(PMID:20329304): She was delivered normally at term, weighed 3kg, made uncomplicated neonatal progress, no history of convulsions, reached early milestones at expected times but did not sit up until 13mo. On report she was well fed and well developed, but with persistent shadow in right upper load on roentgenogram, enlarged liver of normal consistence, spleen was not palpable, negative neurological exam, mental age was 9mo. Aortogram was negative for vascular abnormalities in upper left chest. Skull and long bones were normal but osteoporosis of the spine was detected by roentgenogram.
CaseNotHPOs HP:0001083
(Lens dislocation)
NotHPOsFreeText no malar flush, lens displacement, or skeletal deformity on exam at 13mo.
CasePreviousTesting Plasma homocysteine was 45μM pretreatment. Pyroxidine treatment reduced those levels to trace amounts. In patient fibroblasts, no CBS protein was detect on Western blot and CBS assay detected no activity. (PMID: 20329304): "Full hemogram, routine urine analysis, liver function tests, serum ion and iron-binding capacity, plasma magnesium, serum calcium, inorganic phosphate, fasting blood sugars, blood urea nitrogen, and bone marrow examination were all normal". Strong positive on cyanide nitroprusside, heavy homocysteine spot on chromatogram of urinary amino acids.
CaseMethod1 PCR
CaseMethod2 Sanger
CaseGenotypingMethod Created a bacteria expression system in E.coli by substituting one of three fragments of (PCR amplified)patient cDNA into WT CBS sequence already subcloned in expression plasmids. Bacteria were screened based on which ones did not have CBS activity. Those that did not have activity were thought to have LOF variants in CBS. Plasmids containing the inactivating fragments, one cut with NcoI/XmaI and another cut with Bpu1102/BstX, were sequenced.

FamilyAddInfo Father unavailable for study. Mother was clinically/biochemically healthy with an unexplained macrocytosis. Mother had absent CBS activity, but no homocystinuria symptoms and was heterozygous for the Glu239Lys variant. The variant was found in inactive bacteria carrying the BB segment. Presence of the variant was reinforced by amplifying, subcloning, then sequencing her genomic DNA of the region corresponding to BB.

We here add our observation of a similar bi-allelic OPA1 case with Behr syndrome. This proband is a 20-year-old Italian boy, born from unrelated parents (Fig. 1A, IV-4 in the pedigree), who presented a congenital nystagmus at birth and bilateral optic atrophy was recognized in the first year of life. He acquired autonomous walking at 14 months of age, but clumsy gait and frequent falls were reported. Motor difficulties and visual acuity worsened during childhood whereas cognitive development was normal. Brain MRI performed at the age of 4 and 7 years were normal. At 6 years of age, neurological examination showed ataxic-spastic gait with positive Romberg sign. Fundus exam confirmed bilateral optic atrophy (Fig. 1B). Tendon reflexes were brisk at the lower limbs and there was bilateral Babinski sign. The patient also had bilateral talipes equinovarus. Nerve conduction studies showed delayed sensory-motor velocities with reduced amplitudes of motor potentials and loss or severe reduction of sensory action potentials. Somatosensory evoked potentials could not be recorded and motor evoked potentials showed absent potentials at lower limbs and delayed latency at upper limbs. Brainstem evoked potentials were delayed bilaterally. Repeated urine organic acid gas chromatography/mass spectrometry analysis revealed mild elevation of 3-methylglutaconic acid (8–20 μmol/mmol creatinine). At muscle biopsy histology was normal and spectrophotometric assessment of the OXPHOS enzymatic activities was normal in muscle homogenate. Sural nerve biopsy demonstrated a marked reduction of the large-diameter myelinated axons (Fig. 1E–G). Teased fibre analysis disclosed regions of demyelination confined to the paranodal region, clustered in some axons, suggesting secondary demyelination. Ultrastructural examination confirmed the reduction of myelinated axons (not shown). A clinical revaluation at 15 years of age demonstrated elevated serum lactic acid after standardized exercise (3.9 mM, normal range 0.6–2.4mM) and optical coherence tomography revealed a generalized reduction of the retinal fibre layer thickness (Fig. 1D). At neurological examination, the patient was wheelchair bound due to severe sensory ataxia and spastic paraparesis and signs of cerebellar dysfunction, such as dysmetria, dysdiadochokinesia and positive Holmes rebound phenomenon, were evident. His vision was light perception with severe nistagmus in any direction of gaze. This clinical picture remained stable in the subsequent years. Figure 1. Open in a new tab (A) Reconstruction of the paternal and maternal genealogies of the proband with Behr syndrome (IV-4). The maternal side is remarkable for segregating isolated optic neuropathy in a dominant fashion associated with the heterozygous c.1705+1G>T splicing mutation. (B–D) Ophthalmological investigations. The fundus picture of the proband (IV-4) reveals severe optic atrophy with bilateral pale optic disc (B). The fundus picture of the mother (III-4) shows only a mild optic atrophy with temporal pallor of the optic discs bilaterally (C). Optical coherence tomography examination of retinal nerve fibre layer (RNFL) thickness in the proband shows a diffuse reduction of thickness compatible with the severe optic atrophy (D). (E–G) Semithin cross-section of the sural nerve biopsy. A marked depletion of large myelinated axons affects the sural nerve (E and F). At higher magnification some axons appear to have a thin myelin sheath related to the axonal diameter (G). (H–L) Brain MRI and 1HMR spectroscopy studies. On the left (H) the sagittal FSPGR (Fast SPoiled GRadient echo) T1 shows a mild cerebellar atrophy in the proband (IV-4), and on the right (I) a mild pathological accumulation of lactate (rectangle) was detected in the CSF of lateral ventriculi. On the axial FLAIR T2 sequence no signal changes were detected in the proband (IV-4) (J), whereas few focal hyperintensities, related to not specific areas of gliosis, in the cerebral white matter of the proband’s father (III-3) (K) and mother (III-4) (L) were present. TEMP = temporal; SUP = superior; NAS = nasal; INF = inferior; MRI = magnetic resonance imaging; HMR = proton magnetic resonance; CSF = cerebrospinal fluid; FLAIR = Fluid attenuated inversion recovery. The 43-year-old proband’s mother (Fig. 1A, III-4 in the pedigree) had isolated optic atrophy, most evident on the temporal side (Fig. 1C), which segregated as a dominant trait in her family (Fig. 1A). The remaining neurological examination was normal. Nerve conduction velocities and somatosensory evoked potentials were normal. The other affected individuals in this family all suffered a non-syndromic optic atrophy, which started in childhood and progressively worsened, but remained as an isolated symptom. The proband’s father and the elder sister (Fig. 1A, III-3 and IV-3, respectively) are healthy. In particular, neither signs of optic neuropathy nor abnormal nerve conduction velocities were recorded upon neuro-ophthalmological exam. Both parents and the proband have been investigated by brain MRI and proton magnetic resonance spectroscopy (1H-MRS) in a 1.5 T scanner. There were no MRI structural or metabolic abnormalities in the father (III-3) and the mother (III-4) of the proband, except for a few focal hyperintensities in the cerebral white matter (Fig. 1K–L), whereas in the proband mild cerebellar atrophy (Fig. 1H,J) and pathological accumulation of lactate in the CSF of lateral ventriculi (Fig. 1I) were evident. Direct sequencing of the entire coding regions of the OPA1 gene and the flanking intronic boundaries disclosed two heterozygous mutations. The first c.1705+1G>T mutation occurs in intron 17 and involves the highly conserved donor splice site consensus sequence. Analysis of cDNA revealed aberrant transcript splicing with the activation of a cryptic splice donor site in exon 17, which predicts a prematurely truncated gene product (data not shown). This haploinsufficiency mutation was also identified in the affected mother and in nine maternal relatives, three of which (III-10, III-14 and III-18) are clinically unaffected (Fig. 1A). The second mutation identified in the proband is missense, c.1146A>G (p.I382M), which lies in exon 12, being also present in the clinically unaffected father and sister. Interestingly this is the same missense mutation already reported in other bi-allelic cases (Schaaf et al., 2011; Bonifert et al., 2014). This mutation was found only in one other patient in Italy, out of 132 OPA1 complete sequences. This patient is currently under investigation, being another case of Behr syndrome with optic atrophy complicated by ataxia, but with only the heterozygous c.1146A>G (p.I382M) mutation at the moment (Leonardo Caporali and Valerio Carelli, unpublished data). In the Tübingen cohort of patients with bilateral optic atrophy (in most cases isolated optic atrophy) no further case with the c.1146A>G mutation was found among ∼500 patients that underwent sequencing of all coding exons of the OPA1 gene. This mutation has also been found at low frequency (about 1/1000) in large public data sets (1000 Genome Project, NHLBI GO Exome Sequencing Project). Sequence analysis of the OPA3 gene excluded the presence of pathogenic mutations. Fibroblasts were grown from skin biopsies obtained from the proband and both parents, after approval of the Internal Review Board and signed informed consent. In fibroblasts we first assessed mitochondrial network morphology, after loading mitochondria with Mitotracker Red and examination by fluorescence microscopy as previously reported (Zanna et al., 2008). The mitochondrial network of fibroblasts grown in standard Dulbecco’s modified Eagle’s medium-glucose medium bearing the heterozygous c.1705+1G>T (III:4) and c.1146A>G (III:3) mutations, respectively was similar to controls (Fig. 2A–C), whereas the fibroblasts from the proband were scored into three categories, those with a typical normal filamentous network (Fig. 2D), those with a hyperfragmented pattern (Fig. 2E) and those with filamentous mitochondria containing balloon-like structures (Fig. 2F), each class being present at approximately the same percentage. To investigate whether these OPA1 mutations could affect the bioenergetic efficiency, we measured the rate of mitochondrial ATP synthesis in digitonin-permeabilized fibroblasts. Compared to controls, mitochondrial ATP synthesis driven by complex I substrates (pyruvate and malate) was increased in the proband’s father fibroblasts (III:3) bearing the c.1146A>G mutation, and incrementally reduced in the mother’s fibroblasts bearing the c.1705+1G>T (III:4) mutation and in the proband’s fibroblasts (IV:4) carrying both mutations. The latter was significantly lower compared to his father’s fibroblasts. A similar but less severe situation was observed for the ATP synthesis driven by the complex II substrate (succinate) (Fig. 2G). Overall, these results indicate that the biallelic mutation in the proband led to abnormal network dynamics and impaired ATP synthesis through complex I. Both parent’s mutations were not severe enough to impair mitochondrial network dynamics under the experimental conditions used, but the mother’s mutation impaired partially complex I-dependent ATP synthesis. Figure 2. Open in a new tab (A–F) Mitochondrial network morphology. Control (A) and patient (B–F) fibroblasts were grown in Dulbecco’s modified Eagle’s medium-glucose and loaded with Mitotracker Red as described in Zanna et al. (2008). Representative out of eight similar images are shown for each individual. (G) ATP synthesis. Fibroblasts were treated with 50 µg/ml digitonin, the rate of ATP synthesis was measured as described in Zanna et al. (2008) and was normalized for citrate synthase (CS) activity. Data (mean ± SEM) were obtained from five controls and the three OPA1 mutated fibroblasts. The experiment was performed at least in triplicate. Asterisk denotes values significantly different (P < 0.05) by Kruskal-Wallis one-way ANOVA on Ranks. GPD = Glyceraldehyde 3-phosphate dehydrogenase.

A 5-year-old south Asian female was incidentally found to have hepatomegaly on a trip to India, where she became acutely ill with fever, vomiting, and abdominal pain. Blood work done at that time demonstrated mildly elevated liver function tests (AST 48 U/L, ALT 80 U/L), and an abdominal ultrasound revealed hepatomegaly without biliary or splenic abnormalities. She was prescribed an antibiotic and her symptoms gradually resolved.A month after her return to the United States, her pediatrician noted persistent hepatomegaly and the new development of fever. She was referred to the emergency department because of suspicion of a tropical infectious disease. A repeat ultrasound demonstrated persistent hepatomegaly. Her transaminases (AST 133 U/L, ALT 112 U/L) remained elevated. The infectious workup was negative for cytomegalovirus, hepatitis A, Lyme disease, and malaria. She did have a positive Epstein-Barr virus IgM. Repeat blood work was obtained at follow-up infectious disease clinic visit where she was again found to have hepatomegaly (7 cm below the costal margin) and mildly abnormal transaminases. A computed tomography of the abdomen and pelvis (Figure 1) was performed that demonstrated hepatomegaly but had otherwise normal findings. Her fever resolved within 2 weeks. She was otherwise well and was not on any medications. There was no family history of liver disease. Her immunizations were up to date. Because of persistent hepatomegaly, she was referred to the liver clinic.Open in a separate windowFigure 1Computed tomography of abdomen and pelvis. The liver is enlarged, measuring up to 17 cm in width and craniocaudal dimension. The contour is smooth. The hepatic parenchyma is homogenous without a focal mass. There is no intrahepatic biliary dilatation. The portal and hepatic venous systems are patent and nondilated. In the liver clinic, she was noted to have an enlarged, firm liver, palpable 8 cm below the costal margin with no associated splenomegaly. Test results for autoimmune hepatitis, WD, and alpha-1-antitrypsin deficiency were negative. Nonfasting lipid levels were abnormal; repeat testing showed a fasting total cholesterol of 305 mg/dL and a triglyceride level of 142 mg/dL.On the basis of a clinical suspicion of a diagnosis of CESD, a liver biopsy was performed. It revealed diffuse, microvesicular steatosis in the hepatic parenchyma (Figure 2(a)). Portal tracts were expanded by foamy macrophages containing finely vacuolated material, which was periodic acid-Schiff positive and resistant to diastase digestion. Trichrome stain for collagen showed mild portal fibrosis with early delicate bridging fibrosis. Ultrastructurally, the cytoplasm had a moth-eaten appearance with the presence of lipid droplets and cholesterol crystals (Figure 2(b)). There was no evidence of hepatocellular necrosis, cholestasis, or bile duct proliferation.Open in a separate windowFigure 2(a) Liver biopsy, periodic acid-Schiff stain with diastase (magnification, ×40). Lipid demonstrated within the hepatocellular cytoplasm (arrows). (b) Electron micrograph (direct magnification, ×2500). The hepatocellular cytoplasm in places has a moth-eaten appearance with lipid droplets (arrows). Additionally, some foci show cholesterol crystals (∗) free in the hepatocellular cytoplasm.Liver tissue was sent to Dr. D. Wenger's Lysosomal Diseases Testing Laboratory at Thomas Jefferson University. She was found to have a low acid lipase activity of 7.2 (no units provided by Dr. D. Wenger's laboratory) in the liver. Follow-up testing through a commercial laboratory demonstrated low level of lysosomal acid lipase activity (0.008 nmol/punch/h) in blood. Lysosomal acid lipase gene sequence analysis was performed, which demonstrated that she was heterozygous for the following two mutations: c.57_60delTGAG and c.894G>A. Both parents underwent mutation analysis. The mother was confirmed to have the c.894G>A mutation while the father carried the c.57_60delTGAG mutation. The c.57_60delTGAG is a new mutation not previously described in the literature in patients with CESD.Go to:

gnomAD: This proband has a four-nucleotide deletion, c.57_60delTGAG, in exon 2 of the LIPA gene. This variant is predicted to result in a premature translation stop downstream of the deletion (p.Glu20GlyfsTer6). The variant is not present in gnomAD. This proband is scored default points.
This proband has a synonymous variant, Gln298= on exon 8/10 in a cononical splice site, which causes an abnormal splicing that results in the in-frame skipping of exon 8. The variant is reported in 167/128942 European (non-Finnish) alleles at a frequency of 0.001295, with no homozygotes in gnomAD v2.1.1. This is the most frequent variant in CESD patients (present in 40-60% of patients reported in the literature), but it is not confirmed if this is due to a hotspot or a common ancestor. This proband is scored default points.

Twelve human C1 inhibitor P1 variants were constructed by site-directed mutagenesis of the codon for arginine 444 and were expressed in COS-1 cells to analyze the functional properties.
Transiently transfected COS-1 cells were assayed for the presence of C1 inhibitor antigen with the mAb RII RIA (21). Expression levels of all P1 variants were comparable to wild type. . Lysates of transfected COS cells did contain recombinant C1 inhibitor P1 stop antigen, but in substantially lower amounts than the other P1 variants. This indicates that the premature stop codon results in an aberrantly folded protein, which is degraded intracellularly.
P1 His retained the ability to complex with target proteases, but considerably higher amounts of this variant was required compared to recombinant wild-type C1 inhibitor.
P1 Cys was shown to be nonfunctional in kinetic analyses.
Conclusion: P1-Cys variant is nonfunctional toward Cls, beta-factor XIIa, kallikrein, and plasmin. The low activity of P1-His toward Cls is probably not physiologically relevant.

FamilyAddInfo: 12 individuals carried MYH7-A719H, and 16 carried MYOZ2-L169G. Among the 12 subjects who were clinically diagnosed with HCM, 7 carried both MYH7-A719H and MYOZ2-L169G, 4 had only MYH7-A719H, and 1 did not possess either mutation. Those who carried only the MYH7 mutation had mild LV hypertrophy or normal heart function and morphology, and all experienced only chest discomfort or no symptoms at all after activities. Ten of the subjects who carried only the MYOZ2 mutation showed no LV hypertrophy, abnormal ECG or echocardiographic findings, or discomfort. The 7 patients with both MYH7-A719H and MYOZ2-L169G had more serious symptoms, commonly complaining of palpitation and dyspnea. During the follow-up period, 1 of these subjects experienced SCD, and another suffered from myocardial infarction without obvious hyperlipidemia. Compared with subjects with only MYH7-A719H or MYOZ2-L169G mutation, patients with both the MYH7-A719H and MYOZ2-L169G mutations had the largest median left atrial diameter (Kruskal–Wallis test, P = 0.015) and the highest frequency of cardiovascular events (Fisher exact test, P = 0.007). The MYOZ2-L169G mutation alone was not found to be pathogenic, but it did appear to play a modifying role in HCM.

Anti-lipoate antibody detected strongly decreased levels of the expected lipoylated E2 subunits of α-oxoacid dehydrogenases in fibroblasts of individuals P1 and P2, whereas normal bands were seen in the control (Figure 3A). In fibroblasts of an individual with LIPT1 deficiency,7 these bands were undetectable (Figure 3A). Moreover, immunofluorescence staining examined by a confocal microscopy showed absence of protein lipoylation in P1 and P2 fibroblasts (Figure 3B).
ExperimentName: Reduced E2-lipoylation in LIPT2 patient cells
PatientCell: Fibroblast (not specified where it was derived)
GeneFunction: GO:0009249
GeneAlteration: Patient fibroblasts from P1 harboring the LIPT2 c.89T>C(p.Leu30Pro) and c.377T>G(Leu126Arg) and fibrobalsts from P2 harboring the c.377T>G(Leu126Arg) and c.314T>G(p.Leu105Pro) were used in this assay.
FunctionalEvidence: Patient fibroblasts both from P1 and P2, both with compound heterozygous mutation in LIPT2 that represent 3 different variants (one overlaps), were assayed for E2 lipoylation in PDHc and alpha-KGDHc (Figure 3), compared to control cells. The assay indicates reduced lipoylation of both PDHc and alpha KGSHc compared to controls both by western blot (Figure 3A) and immunocytochemistry (figure 3B). the reduction in lipoylation was consistent with that observed in patients with LIPT1 mutations, indicating a role for LIPT2 in lipoylation.

A 46-year-old Spanish male (Patient 1) born to healthy and non-consanguineous parents was referred because of a long history of muscle cramps and fasciculations affecting upper and lower limbs with onset during adolescence and associated to stutter speech. He had developed muscle weakness a few years prior to the visit affecting his ability to climb stairs or stand up from sitting and had recently started with falls when going downstairs. Neurological examination revealed diffuse fasciculations in lower limbs and mild weakness affecting both psoas and quadriceps that were atrophic (Fig. 1A). Tendon reflexes were hypoactive in lower limbs and normal in upper limbs. Sensory examination was normal. There was no spasticity and plantar reflexes were flexor. There were no signs of cerebellar involvement. Examination of the cranial nerves was normal without involvement of the bulbar muscles. Speech was described by speech therapists as spastic with a dystonic component leading to evident stuttering (Supplemental video 1).

RESULTS Gene and mutation identification Analysis of the single nucleotide polymorphism (SNP) microarray genotypes from six patients with PHID identified a single common 1.4 Mb region of shared homozygosity within cytogenetic band 10q22.1, between SNPs rs10509321 and rs1900515 (Fig. 1A). Inspection of the Ensembl and UCSC Genome Browsers demonstrated that the region included 14 known and predicted genes: ADAMTS14, C10orf27, CDH23, EIF4EBP2, LRRC2, KIAA1274, NODAL, NPFFR1, PCBD1, PPA1, PRF1, SLC29A3, SGPL1, UNC5B. Positional candidate genes were selected for sequencing in order to identify pathogenic mutations (Fig. 1B). Five different homozygous mutations were identified in the SLC29A3 gene encoding the equilibrative nucleoside transporter 3 (Fig. 1C, Table 1). Human ENT3 has been shown recently to be the cause of H syndrome, an autosomal recessive disorder with overlapping skin features to PHID (6), indicating that these are allelic disorders. hENT3 belongs to a structurally unique protein family of nucleoside and/or nucleobase transporters (13,14). The five homozygous mutations identified segregated with the disease within each family and were not present in over 100 Lebanese and Pakistani control chromosomes. Figure 1.Open in new tabDownload slideHomozygosity mapping and mutation results in five PHID families. (A) Homozygosity mapping of chromosome 10. The deletion causes a frame shift mutation p.Y314Tfs in transmembrane domain 7 and is predicted to result in the substitution of transmembrane domains 8–11 with an irrelevant protein sequence of 91 amino acids derived from an alternate reading frame. The nonsense mutation p.E444X is predicted to lead to premature protein truncation with loss of part of the final cytoplasmic domain and transmembrane domain 11 of hENT3. It would appear unlikely that the either residual protein would be capable of nucleoside transport function.Three homozygous missense mutations of SLC29A3 were identified to involve conserved amino acids (Table 1). The first missense mutation involves Methionine116 in transmembrane domain 2, which is a hydrophobic amino acid in species as distantly related as humans and nematodes, with a large basic amino acid, Arginine (p.M116R; Fig. 1C and E). The Grantham score for this substitution is 91, which is classified as a moderate change (15). Analysis using the SIFT prediction software also classifies this as a likely deleterious change (16). The introduction of a large basic amino acid into a transmembrane domain would likely result in significant disruption of the hydrophobic nature of the domain and result in retention of the transporter protein within the endoplasmic reticulum, as has been demonstrated for other membrane proteins (17).The second missense change is a recently reported Glycine to Arginine substitution at hENT3 amino acid 437 (p.G437R Fig. 1C), which is located in the putative cytoplasmic domain between transmembrane domains 10 and 11. This is the only mutation in the PHID cohort that is shared with the known mutations in H syndrome. Of particular interest is the observation that this is the sole individual in the PHID cohort who has not developed insulin-dependent DM. The p.G437R change involves the substitution of a large basic amino acid at a position where 41 members of the ENT family as distantly related as humans and Arabidopsis thaliana encode a small hydrophobic amino acid (Fig. 1E) (18). The Grantham difference for this change is 125 and is classified as a moderately radical change. The third missense change occurs at hENT3 amino acid residue 449, which is also located in the final putative intracellular domain between transmembrane domains 10 and 11, and involves the substitution of a Threonine by an Arginine (p.T449R). The amino acids encoded at this position in hENT3 orthologs are small polar/uncharged or small hydrophobic residues (Fig. 1E). Substitution by arginine is associated with a Grantham score of 71 and is predicted by SIFT as not to be tolerated.

2.2.1. Patient #F061The patient was an Italian male, born from a consanguineous marriage, who was admitted to the hospital at 2 months of age for diarrhea and steatorrhea associated with abdominal distension, anemia (RBC 3.02 × 106/μL; Hb 7.7 g/dL), hepatosplenomegaly, increased liver enzymes (AST 86 IU/L—n.v. 10–40; ALT 38 IU/L—n.v. 5–35), hypoproteinemia (4.8 g/dL—n.v. 6.1–7.9) and failure to thrive. Peripheral blood smear showed the presence of vacuolated leukocytes; bone marrow biopsy showed vacuolization in histiocytes suggesting intralysosomal and cytoplasmic accumulation of lipids (foam cells). The adrenal glands were markedly enlarged and extensively seeded with calcific deposits. LAL activity in skin fibroblasts was 1.4 nmol/mg protein/min (n.v. 55.8 ± 15.6 nmol/mg protein/min). The clinical features and the severe LAL defect were consistent with the diagnosis of WD. The patient died of hepatic and adrenal failure at 6 months of age.

Details on readthrough studies from fibroblast extracts from patients carrying the mutations. References a previous publication PMID: 15081804
a 60-year-old man and his 64-year-old sister (p.[S256R];[Q285R]), who both show ataxia, myopathy, peripheral neuropathy, and spasticity in addition to optic atrophy [42],
This variant is mentioned as a reference to a previous publication (PMID: 20157015) which has already been included in the VCI.
While the Q785R mutation presented normal mtDNA and partially affected mitochondrial morphology, similar to the D603H mutation, the S545R mutation had an intermediate behavior, borderline across phenotypes. Indeed, the complete fragmentation of the mitochondrial network was associated with only a slight, non-significant reduction of mtDNA amount, supporting a borderline nature of this variant, causing either DOA or DOA “plus” possibly depending on modulation of other genetic and/or environmental modifiers. MEFs analysis suggests that a complete fragmentation of the mitochondrial network, in association with mtDNA depletion, hallmarks the DOA “plus” phenotype, whereas if these two parameters are mildly affected the final outcome is expected to be pure DOA. This hypothesis is represented in the scheme of Fig. 10.
Expression of the Ser600Arg variant in OPA1-null mouse embryonic fibroblasts showed complete fragmentation of the mitochondrial network and a slight, non-significant reduction of mtDNA amount indicating that this variant impacts protein function (PMID: 30293569)(PS3_Supporting).
In another study, Atçeken et al10 analyzed 15 patients with MPSI from Turkey. They used direct sequencing to determine the disease‐caused mutations in IDUA gene and identify nine new different pathologic variants (c.494‐1G > A, c.793‐6C > G, c.793‐5C > A, p.M1L, p.Y64X, p.A327P, p.W402X, p.P533L, and p.R628X).
This variant is mentioned in reference to a previously published report (PMID: 27511503)
Instead, in P3 we found a homozygous p.Ala394Thr change; this is the second case of homozygous missense mutation in OPA1 reported to date, after the description of a patient with a severe fatal cardiomyopathy associated with the p.Leu589Arg mutation [8].
This variant is mentioned only in the context of a previous publication
Table 1. Rare nsSNVs identified in 36/44 HCM patients. A comment inferred from review of literature, in silico predictions and evolution conservation analysis, is given for each DNA change in the last column. Based on those considerations and on the type of variant, comments with a gray background highlight DNA changes predicted to be possibly pathogenetic. Comments in bold stand for DNA variants that have been previously reported as phenotype modifying factors.

All variants were confirmed by Sanger sequencing, and family segregation confirmed that the affected individuals, as well as the unaffected siblings, are heterozygous for the variants (Figure 3A). Variants in PIGG and COQ5 are not present in publically available databases, whereas the variant in RGS12 has a cumulative frequency of 4.95e−5 in the Exome Aggregation Consortium (ExAC) Browser.43 The variant in PIGG is nonsense, whereas the other two are missense.Open in a separate windowFigure 3Identified Variants(A) Sequencing chromatograms of the identified variant in family EG. The parents and unaffected siblings are heterozygous, whereas the affected individuals are homozygous for the c.928C>T mutation (noted by the black arrow).(B) Sequencing chromatograms of the identified variant in family JP. The father has the heterozygous c.2005C>T variant, but the mother does not (noted by the black arrow).(C) XHMM analysis using WES data of individual JP01. A microdeletion involving PIGG (chr4: 60,226–2,452,836) is indicated.(D) qPCR analysis of exons 7 and 12 of the PIGG genome from individual JP01 and her parents shows a heterozygous PIGG deletion in the affected individual, but not in her mother, suggesting that the deletion occurred de novo on the maternal chromosome.(E) Sequencing chromatograms of the identified variant in family PK are noted by the blue transparent boxes. The parents are heterozygous, and the affected individuals are homozygous for the c.2261+1G>C mutation.PIGG was considered the most prominent candidate because the variant identified causes a clear loss of function. A recent inquiry of the ExAC Browser verified that no homozygous loss-of-function variants have been reported and that the most common loss-of-function variant, c.1515G>A (p.Trp505∗) (GenBank: {"type":"entrez-nucleotide","attrs":{"text":"NM_001127178.2","term_id":"573459749","term_text":"NM_001127178.2"}}NM_001127178.2), has a minor allele frequency (MAF) of 0.0012 in the European population. This translates to a frequency of less than 1 in 600,000 individuals, compatible with a rare autosomal-recessive disorder.
This family's affected individuals have some phenotypes consistent with COQ10 deficiency (hypotonia, seizures, uniform developmental delay or intellectual disability), but the COQ5 variant was heterozygous which is inconsistent with the MOI for COQ5-AR COQ10 deficiency. Additionally, variants in other genes were found that could explain the phenotype.

However, a glycosyltransferase assay using the wildtype EXTL3 and th eP318L variant showed reduced enzyme activity in the P318L variant compared to wildtype, and simiar levels of activity as untransfected/ mock controls. This indicates the P318L mutation acts as a loss of protein function of the enzyme activity.

we describe a third patient with a homozygous frameshift variant in GDAP2
MonDO: MONDO:0032706
Case: 58-year-old male of Greek origin presenting with a progressive cerebellar syndrome starting at the age of 33 years with mild gait imbalance, ataxia and dysarthria. Apart from dyslipidaemia, type 2 diabetes, and chronic diarrhoea presenting at 37 years of age and attributed to irritable bowel syndrome, the rest of the patient’s medical history is unremarkable. Proband has NM_017686.4; c.134delC with mother as carrier.
DiseaseAssertion: Cerebellar ataxia
FamilyInfo: His relatives have reported personality changes with emotional lability, aggressive behaviour and depressive episodes. Family history is negative for neurological disorders. Both parents originated from the same small village but there was no known consanguinity in the family
CasePresentingHPOs: HP:0002141, HP:0002070, HP:0002066, HP:0001260, HP:0003119, HP:0005978, HP:0002028, HP:0000726 (Gait imbalance, Limb Ataxia, Gait Ataxia, Dysarthria, Abnormal circulating lipid concentration, Type II diabetes mellitus, Chronic diarrhea, Dementia)
CasePreviousTesting: Neurological examination at the age of 50, when the proband was first seen, showed severe gait ataxia with mild pyramidal features consisting of brisk reflexes, dysmetria, dysdiadochokinesia, cerebellar dysarthria, jerky pursuit and nystagmus, mild behavioural changes, and disturbances of short-term memory. Follow-up neurological examination at the age of 58, revealed severe gait disturbance, dysmetria, dysdiadochokinesia and brisk deep tendon reflexes with flexor plantar responses, as well as gaze-evoked nystagmus, jerky pursuit, hypermetric saccades, cerebellar dysarthria, and mild sensory loss in the lower limbs (Supplementary Video 1). Scale for the assessment and rating of ataxia (SARA) score at that time was 31/40 and Mini-Mental State Examination (MMSE) score was 24/30. A detailed neuropsychological assessment was performed, revealing widespread deficits in encoding, learning, and consolidating new verbal information. Overall, the patient exhibited prominent deficits in episodic memory, verbal working memory, and executive functions. A brain MRI scan, including susceptibility-weighted imaging (SWI), was performed and revealed marked cerebellar atrophy affecting the hemispheres and more significantly the vermis, as well as mild cortical atrophy, midbrain and pons atrophy. Nerve conduction studies (NCS), electromyography (EMG) and ophthalmological evaluation were all normal. Laboratory testing of alpha-fetoprotein levels, vitamin E, copper, and folic acid were also normal. To exclude the possibility of an underlying autoimmune disease, the patient’s serum was tested for binding to various markers, including a panel of paraneoplastic markers of autoimmunity, GAD, NMDA, AMPA, GABAB, DPPX and VGKC, as well as monkey cerebellum frozen tissue. No binding to any of these markers was detected.
GenotypingMethod: Blood samples from the proband and his mother were collected for whole exome sequencing ans Sanger confirmation. Initial screening for Friedreich’s ataxia, as well as spinocerebellar ataxia (SCA) types 1, 2, 3, 6 and 7 in the proband was negative.

VariantEvidence: PMID:8162015 previously reported this variant to be associated with the infantile form of SD. A fibroblast line from this patient with the infantile form of the disease was investigated for mutations by single strand conformation polymorphism analysis and direct sequencing of PCR products. A nonsense mutation was found which generated a stop codon at codon 284. The study investigated the steady-state level of mRNA produced by cells containing stop codons using a competitive polymerase chain reaction technique and found that at codon 284, the mRNA levels were 0.8%, as percent of normal per single gene dose.

In family L, the novel variant p.Lys22Asn was predicted to be pathogenic by both CADD and MutationTaster, and its frequency in the population is unknown. This variant may be related to the presentation of Stargardt disease in two individual cases. In individual I1, this variant was associated with the complex allele p.[Ser1642Arg; Val1682_Val1686del], whereas in individual II2, this variant was associated with the p.Gly1901Glu variant. This repetition may support the hypothesis that the variant is indeed pathogenic.

The Ca2+handling properties of hiPSC-derived CMs were studied by Ca2+ imaging. HCMT-CMs had significantly more abnormalities than WT-CMs and HCMM-CMs, when all time points in each group were combined (Figure 2(d), n = 6 in statistical analysis, p < 0.05). Spontaneously beating CMs were divided into five different rhythm categories (normal, oscillation, low/middle peaks, double peaks, and plateau abnormality) based on the abnormalities observed in their Ca2+ signals (Figures 2(e) and 2(f)). HCMT-CMs had significantly higher number of double peaks than HCMM-CMs (p < 0.05, n = 6 in statistical analysis) when both cell lines and all time points were combined for each group

Recently, Lenglet et al., 2018 identified a new VHL cryptic exon termed E1' deep in intron 1 that is naturally expressed in many tissues, and found E1' mutations in eight families with either erythrocytosis or VHL disease. The goal of this study was to determine the prevalence of E1' germline mutations from two cohorts, 221 patients with a single or multiple tumours, suggestive of VHL disease, and 946 patients with a Pheochromocytoma/paraganglioma (PPGL) but without an identified mutation in the three VHL exons or in the main PPGL susceptibility genes. They identified a rare germline genetic variant (maf <1%) in the E1’ VHL cryptic exon in 1% of patients (12/1167). Patient 11 was diagnosed at the age of 12 due to multiple head and neck paragangliomas, and was found to have a rare germline c.340+725A>T E1' variant.
However, genetic testing of the daughter's (IV:6) PBLs by direct sequencing revealed a 3-bp deletion (TTC) at nucleotides 439–441 (fig. 4B).
Two families where index patients had mosaic parents from the NIH-VHL Family Registry were reviewed. Family 2 had an index patient (IV-6), 23Y female, with brainstem and spinal hemangioblastoma (15Y old), solid renal lesion, retinal angioma, and pancreatic cysts (20Y old). Direct DNA sequencing found a NM_000551.3:c.226_228del germline mutation in the VHL gene. Note that the paper describes it as c.439_441del since it uses an older reference sequence (Latif et al.). She had 5 siblings, 3 of which tested negative for the VHL mutation and the other 2 were not evaluated. The patient's father was tested and confirmed to be mosaic for the mutation with the mutation present in approximately one of seven peripheral blood lymphocytes. The presently 48Y father (III-4) was 39Y old and found with renal cell carcinoma, brainstem and cerebellar hemangioblastoma, pancreatic cysts, and renal cysts. ACMG PP4

Genotype-phenotype correlations of 573 VHL patients from 200 kindreds were analyzed and confirmed that higher risk of pheochromocytoma is associated with missense mutations that result in substitution of a surface amino acid or that disrupt the structural integrity of the VHL gene product (pVHL). Age-related risks of retinal hemangoblastomas and renal cell carcinoma were higher in patients with nonsense or frameshift mutations. Southern blot and/or MLPA analysis by whole blood detected this mutation: c.272T>A and 273C>A mutations (HGVS nomenclature as c.272_273delinsAA) which leads to this nonsense mutation. It was found in a VHL patient with retinal hemangioblastomas diagnosed at 21Y and cerebellar hemangioblastomas at 23Y. No other family info mentioned.
