One of the things we look for on a site is the HTML element rel="canonical". If this is present, we respect the canonical URL the site gives us as the site where the annotation should be. When I open a Youtube video by clicking on it from this page, rel="canonical" points back to youtube.com, and not the actual address of the video. I've noticed that, at least for now, when I open a video in a new tab the rel="canonical" link for the video will actually be the video's address. So you might try opening a video in a new window if you want to annotate on the video's actual page and not have it end up on the home page.
