"Multiple joint contractures, Toe walking, Neck flexor weakness, Hip flexor weakness, Foot dorsiflexor weakness, flexion contractures"

CaseHPOFreeText: Generalized weakness that is most notable in the distal musculature, presented with toe walking age 2 years, required multiple Achilles tendon lengthening surgeries between ages 11–14 years. From age 19 years patient developed progressive weakness and contractures. At age 32 years, patient could walk only short distances and had prominent contractures of the finger flexors, neck flexors, and tendo-Achilles. There was marked weakness of neck flexion, shoulder abduction, hip flexion and ankle dorsiflexion and moderate weakness of wrist/hand flexors and mild weakness of knee flexion/extension. Facial and ocular muscle function was normal.

      CaseNotHPOFreeText:

CasePreviousTesting: Muscle MRI of the thigh reveals involvement of many muscles including sartorius, quadriceps, and adductors, with relative sparing of biceps femoris. Muscle MRI of the distal lower extremity shows fatty infiltration in the soleus and lateral gastrocnemius with relative sparing of the medial gastrocnemius. Electromyography (EMG) showed myopathic features and serum CK was mildly elevated. Cardiac evaluation (via EKG and echocardiogram) was unremarkable. At age 32 years, based on formal pulmonary function testing, patient exhibited moderate restrictive lung disease (forced vital capacity 63% of predicted). A quadriceps muscle biopsy performed at age 11 years showed non-specific myopathic changes while a gastocnemius biopsy performed at age 32 years showed prominent mini-cores, increased internalized nuclei (~40% of fibers), type I fiber predominance, mildly increased endomysial fibrosis and focal fatty replacement. Sanger sequencing was normal for RYR1 (full exonic sequencing), SEPN1, LMNA, and COL6A1/2/3.

GenotypingMethod: A heterozygous MYH7 p.Leu1597Arg (c.4790T>G) missense change was detected in the patient and not found in either parent.

    1. CTIP2 is a negative regulator of P-TEFb

      PMID: 23852730

      Gene: MYH7

      HGNC: 7577

In hearts of hypertrophic cardiomyopathic mice, CTIP2 controls P-TEFb-sensitive pathways involved in the establishment of this pathology. Overexpression of the β-myosin heavy chain protein contributes to the pathological cardiac wall thickening. The inactive P-TEFb complex associates with CTIP2 at the MYH7 gene promoter to repress its activity. Taken together, our results strongly suggest that CTIP2 controls P-TEFb function in physiological and pathological conditions.

    1. Mutation that causes hypertrophic cardiomyopathy increases force production in human β-cardiac myosin

      PMID: 23852727

      Gene: MYH7

      HGNC: 7577

"So, how do we stop a genetic heartbreaker like R453C? The identification of a biochemical and biophysical phenotype is the first step. Now that we know that this mutation causes a subtle decrease in ATPase cycling and an increase in force production by changing the stiffness of the myosin motor domain, we can investigate how these changes affect heart function and development."

    1. The discovery in 1990 that a point mutation at residue 403 (R403Q) in the β-myosin heavy chain (MHC) caused a severe form of FHC was the first of many demonstrations linking FHC to mutations in muscle proteins. A mouse model for FHC has been widely used to study the mechanochemical properties of mutated cardiac myosin, but mouse hearts express α-MHC, whereas the ventricles of larger mammals express predominantly β-MHC. To address the role of the isoform backbone on function, we generated a transgenic mouse in which the endogenous α-MHC was partially replaced with transgenically encoded β-MHC or α-MHC. A His6 tag was cloned at the N terminus, along with R403Q, to facilitate isolation of myosin subfragment 1 (S1).

We find in agreement with earlier studies that the R403Q mutation in a mouse α-MHC backbone results in a gain of function, a model that has been widely accepted as the primary cause leading to hypertrophic cardiomyopathy. However, we find that this enhancement does not extend to the R403Q mutation in a mouse β-MHC backbone. Our results do not necessarily imply a universal mechanism for FHC mutations in the β-myosin heavy chain.

CaseHPOFreeText: In the last weeks, patient had frequently been visited by a pediatrician to treat seasonal allergies. Full-body CT-scan was performed to exclude abuse or trauma and did not show any anomaly. The heart (weight: 240 g, longitudinal diameter: 11 cm, transverse diameter: 10 cm, thickness of the anterior wall of the right ventricle: 0·3 cm, thickness of the left ventricle: 1·0 cm) did not reveal morphologic or structural anomalies of ventricular walls and valves. The coronary circulation was right-dominant and presented myocardial bridging (length: 1·1 cm, thickness: 0·5 cm) involving the middle segment of the left anterior descending coronary artery. No other significant elements were found upon macroscopic observation, with the exception of pulmonary edema. Histopathological examination showed patchy interstitial fibrosis, myocyte disarray and waving of the myofibers in the tissue surrounding the tunneled artery. Several areas of interstitial and subendocardic fibrosis in the other samples of the left ventricular myocardium were also observed.

    1. The patient's age suggested familial disease, and testing for genes associated with phaeochromocytoma was conducted. Germ line mutation analysis of the three exons of the VHL gene was performed by polymerase chain reaction and sequencing. A missense mutation c.308C>A (p.Pro103His), not previously reported, was detected in exon 1 (Fig. 3). Figure 3Open in figure viewerPowerPoint Electropherograms showing germ line von Hippel Lindau (VHL) sequence for normal control (a) and patient (b) as described with VHL exon 1 mutation, c.308C>A. At codon 103, the sequencing trace for the normal control (a) reads CCT (proline) but in the patient (b), the mutant allele reads CAT (histidine).

A case report revealed a 12Y Pacific Island boy with bilateral pheochromocytoma at age 10Y. Genetic analysis of 3 exons of the VHL gene revealed a germline VHL mutation (p.Pro103His). The mother was screened for VHL genetic mutations and the same mutation was found. The mother was not screened for VHL symptoms at the time of paper publication. ACMG: PP4

    2. Genotype-phenotype correlations of 573 VHL patients from 200 kindreds were analyzed and confirmed that higher risk of pheochromocytoma is associated with missense mutations that result in substitution of a surface amino acid or that disrupt the structural integrity of the VHL gene product (pVHL). Age-related risks of retinal hemangoblastomas and renal cell carcinoma were higher in patients with nonsense or frameshift mutations. Southern blot and/or MLPA analysis by whole blood detected this mutation (c.164_165insG) which causes a frameshift and truncation allele. It was recorded here in 1 VHL family with 10 affected members. 7 had retinal angiomas, 3 has cerebellar hemangioblatomas, 2 had RCC and 3 had pheochromocytomas.

    4. Genotype-phenotype correlations of 573 VHL patients from 200 kindreds were analyzed. Higher risk of pheochromocytoma was associated with missense mutations that result in substitution of a surface amino acid (p<=0.004) than for patients with missense mutations that disrupt the structural integrity of the VHL gene product (pVHL) or other loss of function mutations (either germline deletions or truncating mutations). This missense mutation (c.509T>G), affecting residue on the surface of pVHL, was found in a VHL family of 4, 3 with retinal hemangioblastomas, 1 with renal cell carcinoma diagnosed at 33Y, and 1 with pheochromocytoma diagnosed at 29Y. This mutation was reported in the paper with an incorrect cDNA written as c.509T>C, however correct translation should be c.509T>G given the Latif et al. (1993): c.722T>G (Val170Gly)

    1. Germline mutations were identified in 300/469 (63%) of the families tested; 137 distinct intragenic germline mutations were detected.

Germline mutational data on VHL was gathered from various countries to assess variation in germline mutations that produce disease within different populations. Families were grouped according to three possible outcomes of disease: renal cell carcinoma without pheochromocytoma, renal cell carcinoma with pheochromocytoma, or pheochromocytoma alone. SSCP and denaturing gradient gel electrophoresis identified mutations in 300/469 (63%) of families affected with VHL. This germline familial VHL mutation (R82_V84del (c.243_251del)) was taken from the Cambridge, Department of Pathology database, with an unknown number of members belonging to Family ID 35CA. It was reported in the paper as c.456delRVV due to a difference in transcript reference. As reported in Table 3, this mutation is found in VHL families without pheochromocytoma, and with renal cell carcinoma.

    1. We describe five families for which direct sequencing of the coding region of the VHL gene had failed to identify the family-specific mutation. Further molecular analysis revealed deletions involving the VHL gene in each of these families.

ArticleReferenceSequence: Not specified – Assumption: variants assumed to have been documented by the author under reference sequence NM_000551.3

GenotypingMethod: Exons 1, 2, and 3 of the VHL gene and their immediately flanking sequences were amplified by PCR, by using oligonucleotides. The flanking sequences included 90 nucleotides upstream of the second start codon in exon 1 and 45 nucleotides downstream of the stop codon in exon 3. Amplification products were purified and subjected to automated sequence analysis on an ABI 377. The amplification primers were used as primers in the sequencing reactions. Screening for structural rearrangements, including gross deletions, was performed by Southern blot analysis. FISH analysis demonstrated the deletion of the entire VHL gene.

Fundus examinations showed several retinal hemangioblastomas in the temporal area of the left eye. They had previously undergone the operation for cerebellar hemangioblastoma 3 years prior. Case III:2 (this patient’s grandson) was a 6 year old male who had fundus examination that revealed a very small hemangioblastoma in the distant peripheral retina of his right eye, while his left eye was normal. Case III:2 (this patient’s granddaughter) was a 12 year old female whose fundus examinations revealed a small hemangioblastoma in the infratemporal region of the peripheral retina in her right eye. A sizeable hemangioblastoma with 2.5 papilla diameters and prominant feeder vessels were detected in the inferior peripheral retina of her left eye

VariantEvidence: Authors state that the c.712C>T germline familial variant of the VHL gene in this patient is “inferred from the family”. No genetic testing was done on this patient to confirm this. Being that this familial variant is out of bound with respect to the NM_000551.3 reference sequence, it was concluded that this variant is noted in the Latif et al. (1993) reference and was translated to c.499C>T (NM_000551.3).

    1. Genomic DNA was extracted using standard procedures. All VHL coding regions were analyzed by Sanger sequencing. PCR products were sequenced in an automated ABI Prism 3700 sequencer (Thermo Fisher Scientific, Waltham, MA). The following oligonucleotides were used: exons 1, forward 5′-CTAGCCTCGCCTCCGTTAC-3′ and reverse 5′-GTCACCCTGGATGTGTCCTG-3′; exon 2, forward 5′-TTAGCCAGGACGGTCTTGAT-3′ and reverse 5′- CGTACAAATACATCACTTCCATT -3′; and exon 3, forward 5′-TACTACAGAGGCATGAACACC-3′ and reverse 5′- CCCCTAAACATCACAATGC -3′.In the patients with clinical VHL diagnosis but without pathogenic coding variants detected by Sanger, multiplex ligation-dependent probe amplification (MLPA) was performed to investigate large deletions. MLPA was performed as previously described using the SALSA® MLPA® P016 VHL probe mix (MRC-Holland, Amsterdam, Netherlands) [14]. This probe mix contains 29 probes with amplification products between 166 and 427 nucleotides: nine probes for the VHL gene (two or more probes for each exon); six probes for genes located close to VHL (FANCD2, BRK1/C3orf10/HSPC300, IRAK2, and GHRL); and 12 reference probes detecting sequences on other chromosomes. PCR product underwent capillary electrophoresis on an ABI Prism 310 Genetic Analyzer (Thermo Fisher Scientific). Data analysis was performed with Genescan 3.7 (Thermo Fisher Scientific). Dosage quotient areas outside the range of 0.70 to 1.3 were considered abnormal.

    1. c.99_100InsA*

The genetic testing program for VHL disease included 17 families, ten of which had definite VHL disease and seven had possible VHL disease. 17 families (n=109 individuals) were tested for VHL mutations using peripheral blood. Mutation analysis was performed by direct sequencing of the VHL gene. Mutations were identified in 36 patients. Mutations-carriers were screened for VHL disease-related tumors and were offered follow-up annual examinations. Tumor status and phenotypes were documented at initial screening and at follow ups. 6 (3 adults and 3 children) members from family 6 were tested positive with c.99_100InsA mutation. 2 adult and 1 pediatric patients were affected at initial screening. Another adult patient was affected by VHL disease at follow up. The other 2 children were asymptomatic both at initial screening and at last follow up. 13 (7 adults and 6 children) members from family 7 were tested positive with c.99_100InsA mutation. 9 members were affected by clinical VHL disease in total by the final follow up. The other 4 patients (whom were all pediatric) were asymptomatic at last follow up. ACMG evidence codes: 'PP4' because the patients' family history are highly specific for a disease with single genetic etiology.

    1. PatientsThe Chinese family study included five-generations comprising of 38 living family members, of whom nine were affected [Figure 1]. All living members were examined clinically. The clinical and radiographic images were published with the consent of the patient. The study was approved by the research Ethics Committees of Xi’an Jiaotong University School of Medicine. Written informed consents were obtained from all participants.Open in a separate windowFigure 1Haplotypes of the Chinese family affected by Von Hippel-Lindau (VHL). The open and filled blackened symbols indicated unaffected and affected individuals, respectively. The squares and circles symbolized the males and females, respectively. The proband, IV-3, was indicated by an arrow. The haplotyping was conducted using three polymorphic microsatellite markers. The marker order was determined from the Marshfield map and the UCSC Human Genome database (February 2009). The dark boxes symbolized haplotypes that cosegregated with the affected individuals and suggested linkage of VHL to disease in this family. The boxes with slash symbolized the dead family members.

    1. Of 65 VHL families from central Europe, 53 were identified with germline mutations using SSCP analysis of PCR products by peripheral blood. This mutation was found in 2 Croatian, VHL type 1 family members (family VHL 26). Both have hemangioblastomas of the central nervous system, one has renal cysts, and one with pancreatic cysts.

The authors report this as an AF010238.1:c.443insTCT mutation at codon 77 (insertion Ser). This does not match the associating c. and p. - NM_000551.3:c.230_231insTCT matches Cys77_Asn78insLeu., while c.231_232insTCT (p.Cys77_Asn78insSer) would give the ins Ser the authors write, however this doesn’t match the p.. Without confirmation from authors no assumption could be made.

VariantEvidence: Variant is declared as “c.446A>C (N78T)” by the authors, this was treated as a legacy variant and mapped to c.233A>C, N78T. This study found coexpression of Epo and EpoR in 11 RCCs and in 10 benign renal cysts in a spectrum of VHL renal disease suggesting that Epo/EpoR coexpression is present in both benign and malignant stages of renal neoplasia. Six benign cysts stained for Epo but not EpoR. The researchers detected loss of heterozygosity of a wild-type VHL allele in different RCCs with microsatellite markers, D3S1110 and D3S2452, flanking the VHL gene
