Introduction et Cadre Problématique:
- Cette séance fait suite à une précédente intervention sur les inégalités d'exposition au risque du Covid-19 et se concentre sur la vaccination, enjeu majeur de santé publique.
- La vaccination a permis une réduction drastique des contaminations et des décès dans les pays où elle était accessible.
- Cependant, les réticences et les obstacles sociaux à la vaccination diffèrent des autres moyens de prévention.
- L'exemple du Covid-19 sera utilisé pour éclairer la question plus large des discriminations et de la santé.
- Deux dimensions sont essentielles pour comprendre le rapport au risque et à la vaccination : la construction individuelle de la définition du risque et le rapport à la discrimination.
- Sources et Méthodologie:
- L'analyse s'appuie sur plusieurs sources:
- L'enquête EPICOV (épidémiologie et conditions de vie), menée auprès de plusieurs dizaines de milliers de personnes à partir de mai 2020, avec des suivis réguliers. Cette enquête utilise un échantillon aléatoire pour garantir la fiabilité statistique.
- L'enquête Trakov de la Dares sur l'évolution perçue des conditions de travail (janvier-mars 2021, échantillon aléatoire de 15 423 personnes).
- Des recherches qualitatives, notamment des entretiens approfondis menés par Raphaël Chalier auprès de personnes non vaccinées dans le cadre du projet Epicov et Jendy.
- Le Concept de Risque : Approches Objectiviste et Constructiviste:
- Le concept de risque est central en santé publique mais très polysémique.
- Distinction entre deux approches :
- Objectiviste: Considère la définition biologique et médicale du risque comme un postulat. On analyse comment les individus se positionnent par rapport à cette définition institutionnelle. L'hypothèse implicite est que la logique sanitaire est la plus importante pour structurer les attitudes et pratiques.
- Constructiviste: Appréhende le risque comme une construction sociale résultant de la confrontation des intérêts, des savoirs et des pratiques entre différents acteurs sociaux. On cherche à comprendre comment les individus élaborent leur propre définition du risque selon leur position, leurs intérêts, etc.
- Ces deux approches ne sont pas antagonistes mais complémentaires. L'approche objectiviste permet une cartographie sociale du risque, l'approche constructiviste aide à comprendre la construction individuelle du risque et l'adoption ou non de moyens de prévention.
- Des exemples hors Covid-19 (contraception, VIH/SIDA) illustrent que la connaissance du risque seul ne suffit pas à expliquer les pratiques ; le contexte social et les conditions de vie jouent un rôle crucial.
- Discrimination et Santé:
- La discrimination, appréhendée comme une expérience, est une manifestation d'exclusion sociale.
- Hypothèse centrale: Se sentir socialement exclu est un obstacle majeur pour s'inscrire dans une démarche préventive, d'autant plus dans le cas de la vaccination contre le Covid-19 qui appelait à un sens citoyen ("se protéger et protéger les autres"). Le non-recours peut être en dissonance avec ce discours si l'on se sent exclu.
- Cette hypothèse s'appuie sur les travaux pionniers de Michael Pollak sur le VIH/SIDA, qui a montré que "l'homophobie est un facteur de risque de contamination par le VIH". Par analogie, les discriminations peuvent être des facteurs de risque de non-recours à la vaccination pour certains groupes sociaux.
- Définition de la discrimination:Juridique: Renvoie à des pratiques interdites par la loi sur 25 critères et concernant des domaines spécifiques (emploi, éducation, logement, accès aux biens et services publics et privés, santé). Introduite tardivement en France (2001) par rapport à d'autres pays.
- Sociologique: Concept d'analyse de la production des inégalités, invitant à resituer les différences de traitement comme des "rouages de la production ou de la reproduction des rapports de pouvoir structurels qui engendrent des groupes majoritaires et des groupes minoritaires."
- Liens entre discrimination et santé: Ils peuvent intervenir à différents niveaux :
- Conditions de vie néfastes pour la santé: Les personnes discriminées connaissent souvent des conditions de vie qui les exposent davantage aux maladies (discrimination indirecte).
- Difficultés d'accès aux soins: Pour des raisons financières, de l'offre de soins (certains professionnels refusent de prendre en charge certains patients, comme le montrent les rapports du Défenseur des droits), ou en raison d'attitudes et pratiques discriminatoires des professionnels (syndrome méditerranéen/Hispanique panique, homophobie/transphobie).
- Recommandations médicales discriminatoires: Basées sur des biais (ex: biais de genre dans le traitement de l'infarctus du myocarde).
- Dispositifs médico-juridiques et sociaux discriminatoires: L'exemple de l'Aide Médicale d'État (AME) et ses critères restrictifs.
- Le champ des liens entre discrimination et santé est encore peu exploré en recherche, à l'exception des liens entre discrimination et accès aux soins, principalement étudiés dans la littérature internationale sur les populations racisées.
- Réticence à la Vaccination Covid-19 et Caractéristiques Sociales (Données EPICOV):
- Intention de ne pas se faire vacciner (avant disponibilité): Plus fréquente chez les femmes, les jeunes (25-34 ans), les personnes peu diplômées, les personnes aux revenus les plus bas (D1), les natifs des DOM, les personnes appartenant à des minorités racisées. La confiance dans le gouvernement ou les scientifiques était un enjeu majeur.
- Qui était réticent et s'est quand même fait vacciner?: Les personnes réfractaires les plus aisées se sont davantage fait vacciner par la suite que les personnes réfractaires les plus pauvres. Parmi les personnes issues de l'immigration non européenne (1ère ou 2ème génération), celles qui étaient réfractaires sont restées non vaccinées.
- Dynamique de la vaccination en France:Un gradient social très marqué: Les personnes les plus diplômées se sont fait vacciner plus vite et même avant l'ouverture du calendrier vaccinal les concernant.
- Des écarts importants selon le revenu à tous les âges.
- Une opposition nette (sauf chez les plus âgés, bien que des différences existent) entre les personnes nées en France/pays d'Europe et les habitants des DOM/minorités racisées (Afrique sub-saharienne, Afrique du Nord), qui montrent des taux de vaccination cumulés plus faibles dès le début et qui s'accentuent.
- Conclusion sur les taux de recours: Moindre recours à la vaccination chez les personnes en bas de l'espace social et chez les minorités racisées.
- Liens entre Expériences de Discrimination et Absence de Vaccination (EPICOV Vague 2022):
- Se concentre sur les 7% de la population qui ne s'étaient toujours pas fait vacciner fin 2022.
- Profil des personnes non vaccinées fin 2022: Jeunes (18-24 ans, 25 ans et plus), hommes (alors que les femmes étaient plus réticentes au départ, elles se sont davantage fait vacciner par la suite, potentiellement en raison de leur rôle social de prise en charge du "care"), faible niveau de vie, faible diplôme, minorités racisées, habitants des DOM, habitants des petites agglomérations (moins que dans les grandes villes).
- Corrélation avec les expériences de discrimination (dans les 5 ans précédant l'enquête 2022):Le fait de déclarer avoir parfois vécu des expériences de discrimination multiplie par 1,78 la probabilité de ne pas être vacciné (indépendamment des autres facteurs sociaux).
- Le fait de déclarer avoir vécu des expériences de discrimination souvent multiplie par 2,43 cette probabilité.
- La déclaration d'expériences de discrimination est cohérente avec les données d'autres systèmes d'enquête.
- Lieu des discriminations et non-vaccination:Discriminations dans l'emploi: Augmentation significative mais modérée (multiplicateur 1,18).
- Discriminations dans le logement: Pas d'effet significatif.
- Discriminations dans le domaine de la santé: Effet très important, trois fois plus de chances de ne pas être vacciné.
- Discriminations dans les services publics: Effet important (16,4% de non-vaccinés parmi ceux qui rapportent des discriminations fréquentes dans ce domaine, contre 6,8% chez ceux qui n'en rapportent jamais).
- Importance cruciale des discriminations émanant des institutions (santé, services publics) sur le non-recours à la vaccination.
- Résultat majeur: Le lien entre expérience de discrimination et non-vaccination n'est pas spécifique aux personnes appartenant à des minorités racisées. Il est retrouvé dans tous les groupes sociaux, même si l'effet est statistiquement plus massif pour les personnes racisées car elles sont plus nombreuses à subir des discriminations.
- Le mécanisme de lien entre expérience de discrimination et non-vaccination se retrouve dans tous les groupes.
- Analyse Qualitative et Compréhension des Processus (Travail de Raphaël Chalier):
- Les refus de vaccin prolongent souvent des "vexations ressenties au contact des institutions".
- Exemples:Asatou (20 ans, vendeuse, parents maliens, porte le foulard): Sentiments de stigmatisation et d'exclusion ("tu n'es pas la citoyenne parfaite", "on n'est pas des Français inférieurs"). Cette expérience préexistante de racisme et la défiance vis-à-vis des autorités nourrie par les contrôles renforcés pendant les confinements ("expérience du racisme qui est préexistante à la crise sanitaire et qui a été renforcée pendant les confinements") expliquent sa défiance vis-à-vis du discours gouvernemental pro-vaccin.
- Yassine (chauffeur livreur, origine algérienne, contre le vaccin et le passe sanitaire): Parallèle entre le passe sanitaire et le racisme ("c'est un peu comme du racisme"). Cette interprétation est liée à ses expériences antérieures et celle de sa famille (difficultés d'accès aux aides pour son frère handicapé auprès des services publics). Cette expérience alimente une défiance vis-à-vis des services publics et, par conséquent, de la vaccination.
- Suzanne (ancienne commerçante, vit en camping-car, mari ouvrier, a perdu un enfant à la naissance par négligence attribuée aux personnels hospitaliers): Attribue le drame de son enfant et ses difficultés financières ("l'État ne nous aide pas", "on nous a laissé crever comme de la merde") à la défaillance de l'État. Les réticences vaccinales ("c'est pas à l'état de me dire 'Vous recevrez que deux personnes chez vous'") redoublent ces valeurs professionnelles et personnelles déjà hostiles à l'État. L'instauration de la peur a conduit à une défiance vis-à-vis de l'obligation et des effets indésirables.
- Ces exemples montrent que la défiance vis-à-vis de la vaccination est "ancrée dans l'expérience des gens bien avant l'arrivée du Covid et du vaccin".
- Conclusion et Implications Politiques:
- Bien que la majorité de la population se soit vaccinée, cette minorité non vaccinée (7% fin 2022) est importante à étudier. Il faut également être prudent car les enquêtes représentatives n'incluent pas les populations les plus marginalisées (SDF, sans-papiers) qui rencontrent d'énormes difficultés d'accès aux soins.
- Le profil social des non-vaccinés est stable entre 2021 et 2022, suggérant qu'il s'agit d'un non-recours qui s'est maintenu, non un simple retard.
- Ce non-recours s'explique par des "expériences vécues de discrimination qui renvoient à différentes sphères de la vie sociale (l'emploi, santé, etc.)".
- Les discriminations émanant des institutions sociales ou étatiques (santé, services publics) ont le plus de répercussions sur la vaccination.
- Ce lien entre expérience de discrimination et non-vaccination n'est pas limité aux minorités racisées mais se retrouve dans tous les groupes sociaux.
- Les non-vaccinés d'origine populaire ou racisée évoquent des vexations au contact des institutions. Les non-vaccinés de milieux populaires non racisés décrivent un État "prompt à contraindre mais défaillant lorsqu'on sollicite son appui".
- Les inégalités dans le recours aux soins (pourtant gratuits et accessibles) ne renvoient pas seulement à des croyances ou différences culturelles, mais s'expliquent "aussi par des pratiques discriminatoires qui ont contribué à modifier la vision du monde de celles et ceux qui en ont été victimes".
- Le "sentiment d'exclusion sociale est antagonique avec l'idée de s'inscrire dans une démarche de prévention pour soi et pour les autres".
- Ces résultats interrogent les politiques de prévention fondées uniquement sur une logique épidémiologique (ciblant les plus fragiles sans prendre en compte les inégalités sociales et les expériences vécues).
- Il était crucial, dès le début de la crise, de s'appuyer sur les connaissances acquises lors d'autres épidémies (comme le VIH), d'associer les personnes concernées et d'être attentif aux enjeux de rejet.
- "Vous ne pouvez pas imposer cette vision de définition objectiviste du risque... il faut essayer de comprendre comment les gens se positionnent et construisent leur rapport au risque en fonction de toutes leurs expériences passées et contemporaines."
- Ces inégalités de recours à la vaccination (alors que ces mêmes groupes étaient souvent plus enclins à utiliser le gel ou le masque) montrent la complexité des pratiques de prévention et la nécessité d'une analyse sociologique fine. Cela apporte une "autre compréhension de la réalité sociale".
Enjeux pour la discussion future:
Comment mobiliser les connaissances acquises dans la lutte contre le SIDA pour d'autres épidémies? Comment les politiques de prévention peuvent-elles mieux intégrer la dimension des discriminations et de l'exclusion sociale? Comment construire des discours de santé publique qui résonnent auprès de populations ayant des expériences de défiance vis-à-vis des institutions? L'intervention de Jérémy Vart apportera une perspective plus large sur les questions de vaccination et la spécificité de la vaccination Covid-19.
